1 Health Care RN / LPN Make staff assignment sheets for shift. Complete necessary paperwork: Care plan Monthly summaries Physician orders Admission Nursing notes 24-hour report Medication error / unusual occurrences Complete infection control rounds. Administer medications. Provide nourishments for residents. Provide nail care for residents. Shave residents. Perform treatments, as ordered. Supervise dining room during meals. Perform walking rounds to monitor: General resident care and hygiene Turning and repositioning of residents Releasing of restraints Restorative nursing programs Risks to safety Proper use of mechanical lifting devices Place calls to physicians, as needed. Answer phone on units forward calls to appropriate personnel. 1

2 Fax necessary information to pharmacy and physicians. Certified Nursing Assistant (CNA) Monitor residents for incontinence and notify appropriate personnel, as indicated. Sanitize bedpans as needed. (Empty bedpan weighs approximately ¾ lb; with water, 1.5 lbs. Empty foley catheter bags and measure output. Mark enema bags and catheter bags. Clean IV poles and feeding pumps. Clean walkers, wheelchairs, geri-chairs. Toilet semi-independent residents. Assist with charting residents weight and vital signs: temperature; blood pressure; pulse; respirations; etc. (if trained) Answer telephone at nurse s station, general filing of nursing files and records, make copies, shred paper through shredding machine. Help with paperwork: flow sheets, restraint sheets, potty records. Test resident wandering alarm systems / bracelets Clean nourishment refrigerator(s) and cart(s). Check for food in proper containers and discard old. Distribute ice & water in resident s rooms. Distribute nourishments and snacks to residents. Assist with passing / picking up meal trays. Assist with feeding residents. Calculate mealtime percentages. Assist with cleaning Dining Areas after meals. Clean residents faces and hands before and after meals. 2

3 Assist residents with simple personal hygiene needs including: washing face & hands; oral care; hair care; shaving; washing eyeglasses, etc. Provide nail care. Help with Alzheimers Group. Organize residents drawers / closets. Ensure all personal items are labeled. Make beds. (Bed in elevated position so as to accommodate height of employee). Answer residents call lights (within 5 minutes) and notify appropriate personnel, as indicated. Ensure that call light buttons are within reach for residents use. Ensure that all food in resident s rooms is in sealed containers. Distribute clean linen / clothing. Sew for residents. Clean soiled utility rooms. Make sure door is closed, make sure lids are down, restock gloves / DEPENDS, bags. Refill DEPENDS in residents rooms. Check for dirty DEPENDS. Check bio-hazard bags. Replace and empty at end of shift. Hang wet shower and Century tub mats to dry. Ensure shower hoses are hung up; remove soap, lotions and shampoo. Check residents ID bracelets for legibility and accuracy. Replace as needed. Check all nameplates on outside of doors and above bed. If missing, report to nurses. Organize Shower Rooms and Clean Utility Rooms. Ensure adequate inventory. Monitor halls for cleanliness. Ensure carts are stationed on one side of hall (State inspection requirement). Provide 1:1 attention to cognitively impaired residents. (i.e. sensory stimulation, etc.) Deliver residents mail. Read newspaper / magazine articles to residents. Write letters for residents who want to communicate with friends and relatives. Provide residents with backrubs. Check lap blankets to make sure that the resident is covered. Accompany / transport wheelchair (and/or geri-chair-bound) residents to Recreation Activities and/or Hairdresser. Fill lotion / soap bottles in bathrooms. Clean residents combs and brushes. Clean residents denture cups. 3

4 Clean residents eyeglasses. Polish residents shoes. Wipe down over-bed tables and geri-chair trays with alcohol. Transport soiled linen carts to Laundry Dept. Supervise resident smoking, as assigned. Observe employee entrance / exit door to deter theft, and to prevent access of unauthorized personnel. Attend in-service training staff meetings; complete certified nursing training programs (again). Dietary Make juices, drinks, coffee. Fill silverware bags with straw, sugar, knife, fork and spoon. Help on food line. (i.e. put napkin and silverware bag on tray, place drinks on tray, slide tray to next person). Dish up deserts / salad (such as pudding, cake, jello, toss salad). Assist cook in making sandwiches, cut up lettuce, celery, peel hard-boiled eggs, etc. Wash lettuce and tomatoes for use the following day. Water pitcher distribution. Help serve lunch trays for staff. Prepare and transport nourishments at designated hours to proper stations. Set up diet cards in food line / room number order. 4

5 Set up night time nourishments for patients. Clear dishes in dining room Wipe counters, trays, tables. Put nourishments on tray from refrigerator and date with marker. Quality assurance; food audit; tray evaluation. Butter bread and place in bag. Fill cereal bowls with cereal and cover. Write menu on menu boards. Record food / liquid percentages of residents for care plan. Fill condiment containers with mustard, ketchup, mayo, sugar and milk packets. Complete stock room inventory. Write down stock on shelves. Clean / sweep loading dock area. Unnecessary items (paper, soda cans, etc.) should be removed. Revise recipe files. Arrange new / or set up party items. Assist manager with paperwork. Social Services Assist with mailings, labels and updating mailing lists. Mail letters out to resident family members. Collate admissions and marketing packets. Visit with residents and assist with reading and writing letters. Conduct facility tours for new admissions. 5

6 Assist with filing and typing. Assist with name bands and labeling resident items. Conduct follow-up phone calls. Physical Therapy Clean equipment mat tables, parallel bars, etc. Supervise ambulatory residents. Supervise A.D.L. s activities of daily living. This would include dressing, grooming, showering, eating feeding oneself, toileting, personal hygiene. Assist with filing, placing records on charts. Assist and supervise home exercise programs. Assist and supervise daily exercise for healthy persons. 6

7 Recreation Deliver mail to residents. Assist with structured programs. Complete admission surveys. Organize craft sales and bake sales. Pass out monthly calendars to residents. Interview new and returning residents to discover likes, dislikes and abilities. Assist in one-to-one resident sensory stimulation sessions. Assist in serving A.M. coffee. Walk with ambulatory residents. Order supplies with assistance of director. Complete assigned paper work. Telephone contacts to increase volunteer program. Assist with communion list. Reorganize recreation equipment closet. Develop and send invitations to family members and friends of upcoming special events. Oversee residents who are in recreation room while staff continues transporting. Take attendance for each formal group program. Participate in one-to-one programming on the floors, and with games (i.e. checkers, chess, cards, etc.) Lead and assist in formal group programs. Assist in set up of programs. Assist residents in writing letters to friends and relatives. Read to residents. Attend resident care conferences. Pass out food trays. Feed residents. Daily documentation of resident programs and attendance records. Assist in craft programs. 7

8 Clean ceramics. Plan and make monthly decorations. Write articles for the newsletter. Update volunteer records, file applications. Sort out recreation catalogues. Give manicures to residents. Reorganize bookshelves in office and recreation room. Housekeeping Check and replace hand towels, soap and toilet paper. Dust dining rooms and lounges. Vacuum all carpet areas. Wash tabletops after residents and staff eat. Empty and clean wastebaskets. Clean personal cloths, closets. Clean bedside stands. Clean bureau drawers. Sew for residents. Dust and polish of furniture. Assist with ordering inventory items. Assist in helping staff in different departments. (i.e. filing in office; answering phone in front office; paging individuals; use photocopy machine, making copies for different departments 8

9 as needed; help feed residents ** if properly in-serviced; assist with tabulating timecards and scheduling.) Fold and deliver linen. Touch up (clean) walls where needed. Wipe all doorknobs. Wash windows / mirrors / sinks. Clean water fountains. Clean employee break room. Clean refrigerators on nursing units / dining areas. Clean ashtrays. Laundry Sort laundry Pre-treat / spot clean soiled bed linens. Fold laundry Load / empty washers & dryers. Deliver to residents rooms / linen supply closets. Ensure that all personal clothing is properly labeled and label as needed. Organize linen supply rooms / closets. Mend resident s clothing. Iron resident s clothing. Measure waist and inseam on house stock trousers to determine size and mark. 9

10 Visit and review materials for injured workers Assist with creating a mandatory postings board for OSHA, WC and other safety materials Maintenance Conduct environmental rounds. Check all call bells for proper operation and functioning; Check water temperatures in different areas, assuring they meet the degree F. range; Check refrigerators for proper temperatures, not to exceed 40 degrees F.; Check freezers for 0 degree F.) Check fire extinguishers Check for use of extension cords. Collect trash in parking lots / areas outside building using stick (with nail). Conduct electrical inspections in resident rooms and on equipment. Check for cracked / broken outlet covers. Complete Preventative Maintenance (P.M.) on: resident beds faucets, shower control units. cabinet heaters (entrance vestibules). mechanical lifts / wheelchairs / geri-chairs (i.e. clean dirt from wheels). HVAC units. furniture. door closers, lock sets, hinges, latches, push bars. window blinds. 10

11 Wash windows. Repair screens. Touch up walls with paint / repair torn wallpaper. Clean air conditioner filters. Business Office (Nursing) filing on units. Answering the phone. Typing. Filing. Making copies. Faxing. Delivering phone messages. Deliver resident s mail. Read mail or news articles to residents. 11
