1 Direct Supervisor(s): RN Manager Direct Reports: none Collaboration: CMO, Director of Clinical Operations, Health Center Managers, Project Manager(s), CEO, COO JOB DESCRIPTION: Provides relevant nursing care in an innovative, ambulatory care setting. Nursing care is provided under the guidance and supervision of a registered nurse and clinical leadership. Responsibilities include collection of clinical data, care coordination, administration of medications and treatments, and documentation of care provided. Scheduling: Intermittent Saturday hours every weeks along with assigned medical provider. No call. Combines nursing, care coordination, immunizations administration, lab processing, patient education, NO phlebotomy 1) Nursing skills within scope of LPN patient rooming, onsite lab completion, basic testing, nebulizer treatments, injectable medications (within scope of practice), immunizations 2) Care coordination referral/test tracking, patient education and self-management support, and immunization admin to be completed per protocols generated by RN manager built within the scope of practice for LPN in NV 3) Administrative duties a. Potential for front office and scheduling skills with the Practice Manager software b. Community outreach events i. Community events as representative of CHA ii. Public speaking engagements with medical provider and administrator 4) Time allocations a. Clinical days i. Potentially requires front office skill in check-in and discharge ii. Minimum of 2 patients per hour iii. Communications liaison for care team iv. Immunizations v. Lab processing vi. Other nursing tasks assigned b. Non-clinical days i. Care coordination (based upon care coordination protocols referrals/test tracking, communication liaison, patient engagement, etc) ii. Immunization clinic days iii. Patient education appointments and telephonic self-management support 1
2 PRIMARY ACCOUNTABILITIES Achieve Results 1. Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction. 2. Ensures timely and accurate collection of patient data, and makes observations on assigned patients to provide supportive data in the patient assessment. 3. Promotes attainment of specified goals through health education to patients/families, and health care personnel as delegated by the registered nurse. 4. Provides service in a manner that is appropriate for the patient s age; demonstrates knowledge and skills necessary to meet the patient s physical, psychosocial, educational, and safety needs. 5. Ensure all patient charts and related documentation are maintained current and relevant at all time. Operational Excellence 6. Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, PCMH accreditation and related professional standards. 7. Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines. 8. Provides care in a manner that respects the patient s rights and choices in a multi-cultural setting. Relationships 9. Develops and maintains favorable internal relationships, partnerships with co-workers, including health center managers, nursing staff, medical assistant staff, medical providers, care coordinators, and business office staff. 10. Develops and maintains favorable external relationships with vendors, contractors, specialist offices, etc. Stewardship and Professionalism 11. Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times. 12. Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization. 2
3 PRIMARY TASKS & DUTIES 1. Provide direct care to patients as assigned and qualified to perform. 2. Assess, review, and develop nursing plans to ensure maximum patient outcomes 3. Provide counsel, support, advice, and support to patients and their families 4. Complete all patient charting, and ensure all patient records are current and complete 5. Attend meetings, patient conferences, planning sessions, related to quality improvement/assurance, patient care, and other related topics within the health center and CHA 6. Attend seminars and maintain all licensure requirements for continuing education and best practices 7. Participates in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes. ESSENTIAL FUNCTIONS/ KEY COMPETENCIES Demonstrate a high level of skill at building relationships and customer service. Demonstrate interpersonal savvy and influence skills in managing difficult clients and patients. Demonstrate high degree of knowledge and competency in the practice of medicine and associated charting requirements. Requisite skills and ability to perform certain medical tasks as assigned Demonstrate a high level of problem solving skill to better serve patients and staff. Strong attention to detail and accuracy. Ability to utilize computers for data entry and information retrieval. Excellent verbal and written communication skills. Ability to implement, and evaluate operational and administrative processes. Completes tasks as otherwise assigned by supervisor and/or leadership POSITION REQUIREMENTS Education Graduation from an accredited practical or vocational nursing program. Current and Unrestricted LPN license from the Nevada Board of Nursing Current BLS - CPR certification 3
4 Experience Three years of LPN practice experience; family medicine practice or other primary care setting preferred Demonstrated knowledge and abilities of nursing skills Clerical/administrative experience a plus Demonstrated success in customer service or working with the general public, preferably in a medical care facility. Demonstrated success in managing difficult customer situations Significant knowledge of medical practices and procedures in a primary care environment. Demonstrated success and experience with direct patient services and the proper knowledge and use of medical devices and equipment. Spanish language skill preferred Physical/ Environmental Occasional lifting of lbs and pushing of 5-20 lbs Sitting, standing and walking approximately 80% of the time A medium to high level of manual dexterity required Bending and reaching approximately 20% of the time Education and Licensure: I. Graduate with a LPN designation. II. Current CPR certification III. Current and Unrestricted LPN license from the NV Board of Nursing Experience: I. Current work experience in a family medicine practice or other primary care setting preferred II. Demonstrated knowledge and abilities of nursing skills III. Clerical/administrative experience a plus IV. Spanish language skill preferred Job Standard 1: Confidentiality I.
