3 Department of Clinical Pharmacology The mission of the Department of Clinical Pharmacology, Flinders University School of Medicine is to provide national and international leadership in the disciplines of pharmacology and toxicology through excellence in research, teaching, clinical pharmacology service delivery, and professional activities. This is achieved through implementation of the individual strategic plans for research, teaching and service delivery. The Department of Clinical Pharmacology was established as a fully integrated department of the Flinders University School of Medicine and Flinders Medical Centre (FMC) in The Department is located in Flinders Medical Centre (Southern Adelaide Local Health Network), the teaching hospital associated with Flinders University. Staff of the department contribute to the planning, coordination and provision of teaching activities in the Doctor of Medicine program, and to biomedical programs offered by the Faculty of Medicine, Nursing and Health Sciences. In particular, the Department of Clinical Pharmacology coordinates topics within the Bachelor of Medical Science, Master of Nursing, Paramedics and Optometry programs. Staff of the Department undertake cutting-edge research in a number of areas, with particular strengths in drug and chemical metabolism, and cardiovascular clinical pharmacology. The xenobiotic metabolism group comprises John Miners, Kathie Knights, and Peter Mackenzie. In addition, members of the Group have established research collaborations with colleagues worldwide, including Germany, Indonesia, Japan, New Zealand, Sweden, Thailand, United Kingdom, and USA. Attractive projects are always available for honours and postgraduate research students. The clinical applications group, Arduino Mangoni, Andrew Rowland, Ben Lewis and John Miners are active in three main areas. 1. Therapeutic drug monitoring, particularly concentration targeted dosing of cancer treatment. 2. Drug-Drug Interactions in clinical practice. 3. Physiological and population modelling of pharmacokinetics and pharmacodynamics. Collaborative projects are being performed locally with the Departments of Medical Oncology, Gastroenterology, Endocrinology and Diabetes, and Palliative Care. State collaborations are with the Basil Hetzel research institute, Adelaide University and the University of South Australia. National collaborations are with the Centre for Health Informatics at University of Sydney and with the Centre of Epidemiology and Biostatistics at the University of Newcastle. Within FMC, staff of the department contribute to the provision of clinical service to General Medicine and Endocrinology. The Department also has close links with SA Pathology including professional responsibility for the therapeutic drug monitoring service at FMC. Staff of the department have high national and international profiles, many holding office in scientific societies relevant to pharmacology. The Clinical Pharmacology Registrar participates in departmental activities. He/she conducts a research project, appropriate to his/her training, under departmental supervision. He/she is involved in teaching Clinical Pharmacology.
4 Background Clinical Pharmacology is concerned with the rational, safe and effective use of medicines. Clinical Pharmacologists are involved with patient care, service provision, teaching health professionals, medical research, and medicines policy and regulation. Clinical Pharmacology training is usually based in tertiary teaching hospitals. Flinders Medical Centre and Flinders University both have a nationally and internationally recognised Department of Clinical Pharmacology. The department has three Clinical Pharmacologists, one employed by the hospital. (Most members of staff are employed by Flinders University, in academic or grant funded positions). The Department has strong links with laboratory science, pharmacy and medical specialities, and makes substantive contributions to local and national policy through contributions to a number of committees related to medicines use. The Department is thus one of the best equipped in the country to provide training in Clinical Pharmacology. The Clinical Pharmacology Registrar position in the Southern Adelaide Region has substantial benefits to the region in meeting current clinical needs. FMC currently has a consultant allocated to clinical pharmacology services in the region and the registrar represents an important contribution to clinical resources. The safe and cost-effective use of medicines has been identified as a national priority and Clinical Pharmacology is the medical specialty at the forefront of this area. As well as providing clinical services registrars in training represent our future workforce. Training our future workforce Medicines are the major therapeutic tool used in the health system and prescribing is the commonest medical intervention. As the medical speciality underpinning the use of medicines, Clinical Pharmacology has a substantial amount to offer other specialities. A training position in Clinical Pharmacology can provide valuable training experience for trainees in other specialities as well as training Clinical Pharmacologists. Furthermore the presence of a Clinical Pharmacology Registrar increases the opportunities for Clinical Pharmacologists to contribute to staff training and development. The key areas covered in Clinical Pharmacology training are outlined in appendix 1. Service Delivery Clinical Pharmacology is a support speciality for therapeutics, as radiology and pathology are for diagnostics. We provide a service of a polypharmacy review within general medicine, addressing the increasing prevalence of complex polypharmacy seen in patients. - In matters relating to safe and effective use of medicines (Quality Use of Medicines or QUM) the Department of Clinical Pharmacology works closely with Pharmacy. The Clinical Pharmacology Registrar, in conjunction with the clinical pharmacists, has a key role in providing front line support to junior medical staff in day-to-day QUM issues, including problem solving around adverse drug reactions, drug interactions, and polypharmacy, or difficult pharmacokinetic and pharmacodynamic issues. - The therapeutic drug monitoring (TDM) service for specific drugs, e.g. gentamicin, is jointly coordinated by the Department of Clinical Pharmacology and the Pharmacy Department. Provision of a drug concentration alone does not lead to substantially improved therapeutics and clinical support is required. Provision of the clinical support for TDM is a key role of the Clinical Pharmacology Registrar. - Assistance with after-hours clinical care: Hospital staffing is often defined around provision of 24 hour care. The Clinical Pharmacology Registrar participates in after-hours clinical care rosters, for example in General Medicine. - An advocate for therapeutic excellence: Therapeutic skills are largely developed during the years worked as a junior doctor. The value of having a member of this peer group as an advocate for quality therapeutics should not be underestimated.
5 - Support to other departments by involvement in projects related to improvements in therapeutics and contribution to CME is a key component of the role (e.g. collaboration with gastroenterology in establishing a monitoring programme for thiopurine use in inflammatory bowel disease). - The Clinical Pharmacology Registrar makes a valuable contribution to policy and management decision making in various hospital bodies and at the state level with involvement in the South Australian Formulary Committee. They bring the combination of a junior doctor s clinical experience and an understanding of the principles of therapeutics. - Community support in pharmacovigilance and the quality use of medicines: the department would like to extend its role to support primary care and regional services. A registrar would both contribute to this and to free up consultant time for this activity. Research Clinical Pharmacology is involved actively in research and scrutiny of research. The Clinical Pharmacology Registrar is exposed to research skills that other trainees do not, for example by participation in the Clinical Drug Trials Committee. The position contributes to the research output of the Institution and at the same time develops skills likely to serve the Institution into the future. Integrated Care As a speciality concerned with the use of medicines, Clinical Pharmacology has a substantive role to play in the development of service models spanning community and hospital care. Training and development of skills in integrated care are key to the future of our health service. Job Description - Consideration to dual training with another subspecialty can be accommodated. In particular, dual training with General Medicine would be considered favourably and we anticipate this will help with recruitment of General Medicine advanced trainees. - After hours roster participation, on call for the medical subspeciality admissions.

7 The Clinical Pharmacology Registrar (ESTP) will undertake training in Clinical Pharmacology and contribute to clinical practice, education and research in Clinical Pharmacology in the Southern Region. This will be under the supervision of Clinical Pharmacologists according to the conditions of the ESTP funding. The primary focus of the position is to meet the requirements of the RACP training in Clinical Pharmacology. The Clinical Pharmacology Registrar (ESTP) will: Undertake training in and contribute to the provision of the therapeutic drug monitoring service provided by the Department of Clinical Pharmacology. Undertake training in and contribute to research in Clinical Pharmacology. This will be within the combined University and Regional Department of Clinical Pharmacology Undertake training in and contribute to educational programs in Clinical Pharmacology for a range of students and practitioners particularly medical students, physicians and physician trainees. This will be under joint supervision of the Department of Medical Education of Flinders University, School of Medicine. Undertake training in and contribute to medicines information within the Southern Region and wider Australia. This will be in collaboration with the Australian Medicines Handbook (AMH).
