1 Self-assessment checklist for accreditation About this form You need to complete this form if you are a course owner and you are recording the outcomes of a self-assessment of a course (for which you are seeking accreditation) against the Standards for VET Accredited Courses 2011 and other administrative requirements. Please note that a complete self-assessment must accompany an Application for accreditation (initial or renewal). It is advisable that the person conducting this self-assessment refer to the Users Guide to the Standards for VET Accredited Courses, as this will support their understanding and application of the Standards for VET Accredited Courses. This form is based on an Accreditation Evaluation Template that is used by ASQA Accreditation Assessors when they record the outcomes of a course evaluation against the standards. Please be aware that all course owners must: a) review the findings of this self-assessment b) formally note its outcome c) declare that it is a true and accurate reflection of the compliance status of the course by signing the declaration provided. A comments section is provided if you wish to comment further on the compliance of your course against each of the 13 design standards for accreditation.
