6 ONLINE REGISTRATION GUIDE Please read through this guide before proceeding with the online registration. This guide is designed to provide you with sufficient information to have a smooth registration. All qualifiers are to register individually, inclusive of spouses who are qualifiers. To begin: 1) Access the unique registration link which will be sent to your Pruadviser on 5 December 2016 / 6 December ) Click Yes, I will be attending or No, I will not be attending. a. If you select No, I will not be attending, it will be taken as an official response. Should you change your decision after registration closes, it will be subject to group flights and accommodation availability. 3) If you are attending, a. Indicate the number of accompanying person (AP) you are bringing. b. Read and agree to the Privacy Notice. Accompanying Package (Based on twin sharing basis) Airline Adult Child sharing with 1 adult Child sharing with 2 adults (No Bed) Cathay Pacific (CX) $1, $1, $ Singapore Airlines (SQ) $1, $1, $ * Financial Consultants/Agency Leaders who did not qualify for Eat & Shop Hong Kong are not eligible to attend as an accompanying person except for spouse. ACCOMPANYING PERSON (AP) S PACKAGE INCLUDES: A return economy group ticket together with Qualifier on dates pre-arranged by Prudential Accommodation with Qualifier on twin-sharing basis. Coach transfers during the basic incentive period. Meals as specified in the incentive itinerary. Singapore and Hong Kong airport and fuel taxes for incentive period only. Travel insurance for basic incentive period on dates pre-arranged by Prudential. a. The above AP package is valid for the duration of the pre-arranged dates only. b. Child AP package is valid to children age from 2 11 years at the point of travelling. c. Child AP aged 12 and above will have to pay the Adult AP package price. d. Qualifier bringing a Child AP (without bed) must find a roommate who is willing to share the room with a child. e. AP package is subject to change in cost of air ticket, taxes and fuel surcharges. f. 7 SECTION A: SELECTION OF PRU CODE ONLINE REGISTRATION GUIDE Please select ONE PRU code for your flight booking to Hong Kong. Each PRU code is tagged to a specific airline and flight schedule. a. If you are travelling with an AP(s), they will automatically be placed in the same PRU code. b. Allocation of PRU code / airline is based on first-come first-served basis. SECTION B: FLIGHT DEVIATION If you are deviating (post-trip), please indicate your intended departure date. Additional fees will apply for deviation. a. Flight deviation refers to passengers who are not departing Hong Kong with official events dates (16 February 2017) b. Flight deviation is subject to seat availability on respective airlines. c. In the event the airline is unable to accommodate your flight deviation request, you will be required to travel as per group s schedule. Cost of Flight Deviation (Per Adult / Child) Airlines Deviation cost Cathay Pacific From $ Singapore Airlines From $ (Based on min. 10pax travelling on same flight itinerary) *Eat & Shop Hong Kong flights are booked on unique group fare rate and are subjected to strict fare conditions for the entire journey. As such, travelling qualifiers will need to top up the fare difference (deviation charge) which will enable them to travel on their intended dates. Deviation Remarks Flight deviation is subject to seat availability and fare differences in the available booking classes. (Different booking classes have different fare conditions and restrictions such as flight departure time/periods, itinerary change fee, refund amount/duration, baggage allowance, mileages and seat assignment). Flight deviation is subject to airport tax differences on respective airlines. Higher fares will be offered if the lower fares are not available. Deviation on SQ is based on a minimum of 10 persons travelling on the same flight itinerary. If there are less than 10 persons, a higher deviation cost will apply. 10 IMPORTANT INFORMATION VISA REQUIREMENTS Passport Please ensure that your passport has at least six (6) months validity from your date of return to Singapore. Visa Exemption Nationals of Countries listed below may enter Hong Kong without a Tourist Visa for stay up to 14 days: Singapore / Australia / Brunei / Canada / Indonesia / India / Japan / Macau / Malaysia / New Zealand / Russia / UAE / UK / USA. 11 IMPORTANT INFORMATION Amendment after registration closed The deadline to amend flight deviation is Friday, 16 December Request for amendment is subject to seat availability. Any deviation cost and fare difference will be borne by the qualifier. Strictly no change or amendment allowed after 16 December Deposit and Payment for Accompanying Person s Package A non-refundable deposit of S$ per person is payable upon notice of confirmation. Balance payment must be settled by Tuesday, 10 January Failing to do so may result in the non-processing for accompanying person(s). As there are limited seats available for the trip, it will be on a first-come first-served basis. Every effort will be made to accommodate all booking requests. Payment for Single Supplement, Hotel Extension and Flight Deviation Payment for room upgrades, hotel extension and flight deviation has to be made within five (5) days upon receipt of invoice or notice of confirmation, whichever is earlier. Payment can be settled by cash, cheque or credit cards. Failing to do so may result in the non-processing of request for single supplement / hotel extension / flight deviation. Payment by Cheque Cheques should be crossed, made payable to Global Travel Pte Ltd and mailed to. 12 IMPORTANT INFORMATION Travel Insurance Standard plan travel insurance is provided for the basic trip period on the pre-arranged dates only. Please contact QBE of your travel insurance if you are extending your trip. QBE Contact Person: Wendy Goh QBE Contact Number: Change of Incentive Schedule In the event of unforeseen circumstance, Global Travel Pte Ltd, subject to approval from PACS, reserves the right to cancel or amend the dates, timing of convention or itinerary even after bookings have been accepted. No Show fee A no-show fee will apply based on the cost of the basic package and any other cost including the airline cancellation fee. Unused Service No refund or reduction of the package price will be made in respect of unutilised accommodation, meals or any other service included in the package price. Special Requests All special requests for flight, accommodation, and meals are subject to availability. Please indicate these requests in the special remarks column during registration. Flight Seating Arrangements Any request for flight seating arrangements will be submitted to the respective airlines. Please note that the seating allocation is done by the airlines or at the airport. Global Travel will not be liable, should your request is unsuccessful. Page 12 of 13

13 RESPONSIBILITY AND LIABILITY Global Travel Pte Ltd and its associate agents act only as agents for the transportation companies, hotel contractors and other principals. All tickets, vouchers and documents issued are subjected to those terms and contracts under which such transportation and other services are provided. We assume no responsibility for injury, damage, accident, loss, delay or irregularities that may be caused to any person or property unless directly caused by or attributable to Global Travel, its staff or agents. Global Travel will ensure that all services provided are accordingly to the itinerary provided. However, Global Travel, subject to PACS approval, may alter or modify itineraries and hotel accommodation in the interest of convention members, or due to circumstances over which Global Travel has no control. The right is reserved to cancel or withdraw, at any time, bookings made by or for the clients, in which event no liabilities, whatsoever in respect of such cancellations or withdrawal shall fall upon the operator, save only that they shall be refunded to the clients monies paid by him in respect of the booking so cancelled or withdrawn. The transportation companies or firms shall be exempted from all liability in respect of detention, delay, loss, damage, sickness or injury, by whomsoever caused and or whatever kind of recurring of or to the passenger at any time when the passenger is not on board a carrier or conveyance used or operated by the transportation companies or firms concern shall constitute the sole contract between the transportation companies or firms and the purchase of these conventions and/or passengers. All tickets, coupons and orders are furnished and issued subjected in all respect to those terms and conditions under which the means of transportation or other service, provided thereby are offered or supplied, owners, operators, public carriers, managing agents or agents. Global Travel reserves the right to require any person to withdraw from the convention if it is deemed that his act or conduct is offensive to or incompatible with the comfort of other clients and Global Travel shall be under no further liability. CLAIMS All claims against The Company must be made in writing within 14 days after the completion of the conference. All disputes, if any, will be subjected to the Singapore Court of Law. VARIATION OF CONDITIONS These conditions cannot be waived; except in writing by the Managing Director of The Company. THE RIGHT TO REJECT Global Travel reserves the right to cancel or withdraw any itinerary or any bookings made for a passenger or decline to accept or retain any person as a member of a convention, if any such person may appear likely to endanger the health, safety or impair the comfort and enjoyment of others on the convention. Global Travel further reserves the right to cancel reservations if for any reason, any airline, hotel or other contractor refuses to allow the passenger to take part in the convention. Global Travel s sole liability shall be refunded to the client any monies paid less the amount for services utilized, administrative and cancellation fees.
