3 SAFE JOB PROCEDURE Safe job procedures are a control. They provide step by step instruction on how to perform highrisk tasks safely. The Occupational Safety General Regulations requires task procedures for certain jobs if they are applicable to your operation, i.e.: Lock Out, Confined Space Entry, Fuelling Equipment with Internal Combustion Engine, etc. The following pages contain samples of generic safe job procedures. Safe job procedures must be developed to address specific company work activities, tools and equipment. Page 3

5 Safe Job Procedure Procedure For Changing A Propane Cylinder 1. Check for date of manufacture or the last valid inspection date(10 years or less). 2. Visually inspect the cylinder for cuts, gouges, dents and rusting. 3. Test for leakage which may render cylinder defective. Use soap or leak detector(service valve, valve, sight gauge, relief valve, vent valve). 4. Check that the relief valve fitting is approximately 90 O from the locating holes and secure. 5. Check service valve for defects (handle broken, spindle bent, defective or missing O ring or back up ring. 6. Fill valve cover in place (if applicable). 7. Check to ensure valve is closed before connecting to coupler. 8. Open flow control valve slowly (check for leakage). 9. Tag defective cylinder, noting defect and report to a supervisor. Page 5

6 Safe Job Procedure Procedure for Refueling - Trucks and Mobile Equipment STEPS: 1. Park vehicle parallel to the fuel pump. Position the vehicle to have its fuel tank opposite the pump. 2. Apply maxi brakes. 3. If operating a gas engine, shut engine off before starting the fuel pump. 4. When using the card lock system, follow the instruction on the screen. 5. Make sure the pump meter is returned to zero before pumping. 6. Place the nozzle of the hose in the fuel tank of the vehicle, start the pump, fill the tank by depressing the lever on the nozzle. Do not overfill the tank, or use any device to hold the nozzle open while fueling. 7. When the fuel tank is filled, shut off the pump an place the nozzle back on the mount. 8. Record the fuel used on your time card, or get the print out from the card lock system. Turn this information in at the end of the day. 9. Any spills are to be contained immediately. Use oil dry on any small spills. Absorbent pads are to be used on larger spills. All spills are to be reported to the office. 10. Fuel spilled on hands or exposed skin shall be washed off A.S.A.P. 7 Job Procedure: Confined Space Entry for interior drum maintenance/repair Attachments Yes : No 9 # 6 pgs Major Hazards: Toxic Atmosphere, Rescue Considerations Creation Date: July 5, 01 Review Date: Pg. 1 of 3 Task Description: This procedure is intended for entry into the drum for the purposes of maintenance and repair (a confined space environment). Important Note: No one ( this includes workers, supervisors, management, inspectors, visitors and the general public) shall enter into a confined space until: (a) they have been trained in confined space entry and designated competent ( in writing) in this procedure or a management approved alternative procedure; (b) a pre entry hazard assessment (attached) is completed and documented by a worker who is designated as competent to do so; (c) an entry certificate (attached) has been completed and posted by a designated competent worker. Applicable Safe Work Practices: PPE Applicable Personal Protective Equipment: As outlined in the Occupational Safety General Regulations related to individual tasks. Tools: Attachments: (1) Hazard Assessment Form (2) Entry Certificate Reference Procedures : (1) Lock out Tag out Procedure Steps Hazards/Concerns Corrective Action (1) Complete Hazard Assessment Form Note: Pre entry testing and monitoring for air quality is not required for inspection or cleaning. Consideration must be given to all aspects of the tasks to be performed. As per OSGR, part 12, section 130 subsection 9 Consider persons, process, products and environmental conditions If a controlled product is used in the cleaning process, ongoing monitoring is required. (2) Complete and post Entry Certificate Complete all sections Contact supervisor if any assistance is required. Page 7

8 Job Procedure: Confined Space Entry for interior drum maintenance/repair Attachments Yes : No 9 # 6 pgs Major Hazards: Toxic Atmosphere, Rescue Considerations Creation Date: Nov. 10, 00 Review Date: Pg. 2 of 3 (3) Inform supervisor and other affected personnel that you will be working in a mixer drum. (4) Secure vehicle on level surface, set brake and block wheels. (5) Rotate drum to ensure hatch cover is located on the side of the drum horizontal to the roadbed ie: 3:00 & 9:00 Supervisor will verify compliance with procedure. Traffic in the area around the truck. Ease of access and positive air circulation. (6) Lock out the vehicle. Accidental startup of the vehicle during the cleaning operation. (7) Collect all necessary tools and equipment. Inspect hoses for breaks or leaks. Blow off of the hose and or rupture - defective couplings. Communication - Supervisor will co ordinate activity. If done in the yard, vehicle must be placed in secure area away from traffic flow. Place traffic cones approx 4 ft out from entry and exit side, if adjacent to a travel way. Reference Lock Out - Tag Out Procedure@. Inspect equipment replace as required. Inspect electrical cords for frayed sections and breaks. (8) Ensure all noted PPE is in good condition and in place. Wear appropriate respiratory equipment. (9) Remove access hatch. Enter drum through hatchway. (10) Perform work operation. Electrical shock Airborne irritants and/or toxins Falls from vehicle. Ensure electrical cords are in good working condition and not run through wet areas or puddles. Air quality must be maintained. If dust is generated mechanical ventilation must be used. To prevent potential fall, entry should always be through the hatchway not the hopper. Stepladder must extend at least 3 steps above the hatchway. Page 8

(12) At the completion of the job inspect drum, remove tools and supplies and check for other employees. (13) Remove Lockout. Closeout permit. Notify your supervisor and other affected employees that the job is complete and the vehicle is returned to normal service. (14) Submit all documentation to the Supervisor for records management. Secure and/or removal of tools Loss of equipment. Danger to workers in immediate area. Check for other employees and clear the area, energize the vehicle, rotate the drum and repeat lockout procedure. Do not assume you are alone working in the area. Circle check. 10 Personal Safety Padlock Instructions Your safety padlock is for your personal safety and is to be used only for locking out mixer controls. When starting to work on a mixer drum, read and follow the AMixer Drum Lockout Procedure@. This procedure explains how to apply your safety padlock before entering the drum. Where more than one individual is working on a mixer drum, each person shall use his/her own personal padlock to lock the mixer pump control lever. Therefore, the mixer can not be restarted until the last person has completed their work. Page 10

Creation Date: June 11/99 Review Date: Oct. 20/00 Pg. 1 of 4 Task Description: General lockout - tagout procedure for any activity involving work on the drum body or chassis within the immediate vicinity of the drum.* Important Note: No person(s) shall perform the Lockout procedure unless they have been trained and and verified competent by the employer. Training Records will be maintained within the safety system documentation ( personnel training records) and all tags will be maintained on file. Any lock removals by a second party will be treated as an incident and investigated accordingly. Applicable Safe Work Practices: Traffic Control, PPE Applicable Personal Protective Equipment: Hardhat, Safety Boots Tools: Steps Hazards/Concerns Corrective Action (1) Notify your supervisor and other affected person(s) that the mixer is going to be locked out and that no attempt should be made to restart it. (2) Park truck in appropriate area set brake and chock wheels. Set the drum control in the neutral position so the drum remains stationary. Accidental start up. Crush injuries. Traffic flow around the truck. Communication. If done in the yard, vehicle must be placed in secure area away from traffic flow. Place traffic cones approx 4 ft out from entry and exit side, if adjacent to a travel way. Page 11

12 Job Procedure: Lockout - Tagout, London Mixer Attachments Yes : No 9 # 4 pgs. Major Hazards: Moving Parts & Equipment. Creation Date: June 11/99 Review Date: Pg. 2 of 4 (3) Shut off the truck engine and remove ignition key. Roll up cab windows, lock the cab doors and place the truck keys in your pocket. (4) Place a Danger sign on both cab doors. AMan working on the inside or outside of drum. Do not start mixer or rotate drum. Serious personal injury may occur.@ Refer to Figure 1. (5) Note position of the mixer pump control valve arm. Refer to Figure 2. Remove the nut and lockwasher on the control valve arm. Remove the control valve arm and place a Safety Lockout through the splined hole. Lock the Safety Lockout with a padlock along with a ADanger Lockout Procedure in Affect@ tag. Place the padlock key in your pocket. Possible crush injury If more than 1 person is working on the vehicle possible confusion over who maintains the lock. Quality of the lock Communicate clearly. Ensure signs are visible and in good condition. The individual who places the Lockout sign must be clearly identified on the sign. Each person who is working on or in the drum must place their own lock on the Safety Lockout and pocket their own key. Locks made by a reputable manufacturer should be used to preclude the possibility of having inferior locks which may be tampered with or opened by other keys. Page 12

Combination type locks are prohibited. Duplicate keys will be maintained by the employer in a secure location. They must be under the direct control of a responsible person and used only by him in an emergency. At least two people should be responsible for using a duplicate key. (6) A mechanical locking device or mechanism will be applied to the drum roller track and rollers. (7) If drum must be repositioned, and lockout condition interrupted, check for other employees and clear the area, energize the vehicle, rotate the drum and go back to Step 3. Important Employer will detail site specific controls for mechanical lockout. Short cuts cost Follow all steps At completion of job inspect the drum, remove tools and supplies anc check for other employees. Remove the drum securing assembly. Remove padlocks and Safety Lockout and reconnect the mixer pump control valve arm. Remove the cab door danger signs. Notify your supervisor and other affected person(s) that the lockout is complete and the vehicle is returned to normal status. 14 Job/Task Procedure: Jump Start 2000 Dodge Ram Pickup 12 Volt System Attachments Yes : No 9 # 1 pgs Major Hazards: Chemical, Electrical Burns - Explosion Creation Date: March 23/00 Review Date: Pg. 1 of 3 Task Description: This procedure is intended for jump starting the Dodge Truck with a dead battery. This procedure should not be used for other vehicles before consulting the owners manual for vehicle specific cautions. Winter Caution - During cold weather when temperatures are below freezing point, electrolyte in a discharged battery may freeze. Do not attempt jump starting because the battery could rupture or explode. The battery temperature must be brought up above freezing point before attempting jump start. Applicable Safe Work Practices: PPE - Traffic Hazards Important Note: Remove all metal jewelry such as watch bands or bracelets which might make an unintended electrical contact. Applicable Personal Protective Equipment: Safety Glasses, Gloves Tools: Booster Cables, Flashlight for night time application Steps Hazards/Concerns Corrective Action (1) Check the Battery Test Indicator. If a yellow or bright coloured dot is visible in the indicator, DO NOT jump-start the battery. See figure 1 attached. If the indicator is dark or shows a green dot, proceed. Battery fluid is a corrosive acid solution; do not allow battery fluid to contact eyes, skin or clothing. Don t lean over battery when attaching clamps or allow the clamps to touch each other. If acid splashes in eyes or on skin, flush contaminated area immediately with large quantities of water. (2) Park the booster vehicle within cable reach but without letting the vehicles touch. Set the parking brake on both vehicles, place the automatic transmission in Park or the manual transmission in Neutral, and turn the ignition OFF. Traffic in the area around the truck. Do not permit vehicles to touch each other as this could establish a ground connection and personal injury could result. Reference the SWP on traffic hazards. Do not place yourself between a vehicle and on coming traffic. Obtain longer cables if required. Page 14

15 Job Procedure: Jump Start 2000 Dodge Ram Pickup 12 Volt System Attachments Yes : No 9 # 1 pgs Major Hazards: Chemical, Electrical Burns - Explosion Creation Date: March 23/00 Review Date: Pg. 2 of 3 (3) Turn off the heater, radio, and all unnecessary electrical loads. (4) Connect one end of a jumper cable to the positive terminal of the booster battery. Connect the other end of the same cable to the positive terminal of the discharged battery. Improper connection will cause a dead short and cause potential harm to the electrical system Positive terminals are usually red and marked with a plus sign. Clean battery if required and ensure you have the correct post. See attachment - Figure II - # 1& 2 (5) Connect the other cable, first to the negative terminal of the booster battery and then to the engine of the vehicle with the discharged battery. Make sure you have a good contact on the engine. Do not connect the cable to the negative post of the discharge battery. The resulting electrical spark could cause the battery to explode. Ensure you have post properly identified. See attachment - Figure II - # 3& 4 (6) Start the engine in the vehicle which has the booster battery, let the engine idle a few minutes, then start the engine in the vehicle with the discharged battery. (7) Remove the jumper cables, reverse the above sequence exactly. Never permit a person to stand between two vehicles during this procedure. Vehicle movement is possible from collision or failure to ensure gears in neutral. Be careful of the moving belts and fan.
