3 Introduction Every school has its own story to tell. The context in which teaching and learning takes place influences the processes and procedures by which the school makes decisions around curriculum, instruction, and assessment. The context also impacts the way a school stays faithful to its vision. Many factors contribute to the overall narrative such as an identification of stakeholders, a description of stakeholder engagement, the trends and issues affecting the school, and the kinds of programs and services that a school implements to support student learning. The purpose of the Executive Summary (ES) is to provide a school with an opportunity to describe in narrative form the strengths and challenges it encounters. By doing so, the public and members of the school community will have a more complete picture of how the school perceives itself and the process of self-reflection for continuous improvement. This summary is structured for the school to reflect on how it provides teaching and learning on a day to day basis. Page 1
4 Description of the School Describe the school's size, community/communities, location, and changes it has experienced in the last three years. Include demographic information about the students, staff, and community at large. What unique features and challenges are associated with the community/communities the school serves? & Academy is located in Forest Park, Georgia. The legal sponsoring corporation is Prevention PLUS, Inc., a 27 year old community-based 501(c)3 organization that provides a wide range of educational and vocational services and programs to high-risk, low income youth primarily in Clayton County, although students from other counties also attend. The school's average attendance for is 60, with an approximately equal number of male and female students and, 60% African American, 18% Hispanic, 16% Anglo, 3% Asian and 3% multiracial, which is reflective of the community demographics. In 1999, Prevention PLUS, Inc. formed and registered its educational program through the Home School division of Clayton County Public Schools. In 2001, was officially established. In 2002, the school achieved full accreditation by the Accrediting Commission for Independent Study, allowing students to be eligible for the HOPE scholarship. In 2005, the school achieved full accreditation through the Georgia Accrediting Commission and in 2007, full accreditation through SACS (Southern Association of Colleges and Schools). In 2007, Prevention PLUS began providing credit recovery through the Street School Academy to Clayton County Public School students to help them complete missing credits needed for graduation. The organization also established a dual credit/joint enrollment program with Atlanta Technical College, Clayton State University and Atlanta Metropolitan College. YouthBuild services began in Prevention PLUS, Inc. consists of three programs: -, a member school of the Street School Network, functions much like a traditional school setting where students change classes throughout the day. Class sizes are smaller allowing for more individualized attention. Youth may be significantly behind in basic math and literacy skills, low-income, have multiple high-risk factors, adjudicated or aging out of foster care. - The Street School Academy provides Independent Study classes; a dual-credit program option through Clayton State University, Atlanta Technical College, or other postsecondary institutions; and Credit Recovery for public school students. - YouthBuild, a youth and community development program funded by the Department of Labor, assists low-income young people ages complete their high school diploma or GED while learning job skills, and serving their communities by building or rehabbing affordable housing.key Services: Case Management & Advocacy Prevention PLUS adheres to an intensive approach to case management. Case Managers meet with students individually on a weekly basis and in some instances real-time. At the beginning of the school year, each student works with his/her Case Managers to create an Individual Development Plans (IDP). The IDP outlines: demographic information; assessment data (TABE, CAPSOL, career assessment, GCIS, love language, teacher interview assessment); high school credit summary or GED Scores completed to date; short and long term career goals; and, information regarding family, health, transportation, work/financial, personal, academic and program perceptions. The document facilitates action planning in the above mentioned areas including postsecondary placement and credential attainment. At the beginning of each quarter, the document is updated by the student and Case Manager. It also guides the bi-weekly case conference meeting discussions and once per semester Student Review Panel. Workforce Development Prevention PLUS Inc. programs serve as a provider site for the Workforce Investment Act through the Atlanta Regional Commission. As such, students strengthen career skills through workforce development classes, job fairs, college fairs, summer jobs programs, job shadow days and paid/unpaid internships, and job development support. The Career Development Specialist, Case Managers and Postsecondary Coordinator work with students individually and in groups to introduce participants to GCIS (Georgia Career Information System), an online Page 2
5 tool for career preparation, assessments, labor market data, and educational resources. The Prevention PLUS media center and computer labs are equipped with the latest technology, and classroom have additional computers, I Pads, and a Smartboard available to support career development, postsecondary planning and academic coursework. A partnership with Atlanta Technical College affords students an opportunity to earn Supply Chain Management college credits and credentials while enrolled in our programs. Leadership Development Leadership development and community service activities are integrated throughout all programs. Time management, conflict resolution, communication, values clarification, personal budgeting, employment readiness, and postsecondary readiness are topics discussed in leadership workshops and quarterly conferences. In all facets of programming, students may strengthen leadership skills by serving as teacher assistants, conflict resolution leaders, front office assistants, crew leaders on the construction site, and civic service activity coordinators. In conjunction with WIA, students' participate in leadership workshops. Students are assessed regularly on their competency gains and success is celebrated often. Mentoring/Advocacy Mentoring Youth Rites of Passage Experience (MYROPE) was created to build a brand name for Prevention PLUS' mentoring program within the community. MYROPE continues to evolve into a premier mentoring program which has created a curriculum to hone in on life skills that can be applied to the overall goal of helping youth transition from adolescence to adulthood. The curriculum focuses on specific life skills that better prepare youth to compete on a postsecondary and global scale. Post Secondary preparation and Transition A strong emphasis on postsecondary placement begins prior to enrollment and continues with activities listed in the Individual Development Plan (IDP). All students are tracked on an ongoing basis following graduation through postsecondary and/or career placement. Prevention PLUS, Inc. accesses an alumni support network and tracks alumni annually through The National Student Clearinghouse. In addition to tracking and case management, the postsecondary coordinator provides direct assistance with college admission to ensure that the student successfully completes the application/registration process for enrollment. The Postsecondary Coordinator and Staff Advocates work closely with postsecondary faculty staff to establish a solid entrance for students. Follow-up continues with alumni as long as is needed to reduce or eliminate barriers. Prevention PLUS, Inc. changed locations in July 2012, when it relocated to a separate building on the campus of New Covenant Baptist Church, 5881 Phillips Dr., Forest Park, GA, The new location has larger classrooms, a full-size gymnasium and garage area that serves as the YouthBuild construction lab. The new location allows youth from all programs to convene in one building. After 11 years, a change also occurred in the Prevention PLUS, Inc. Executive Director position. The Principal/Director of Programs remains the same in the & Academy. In the past three years, the school has increased its focus on post-secondary education career development and mentoring, resulting in greater postsecondary success. More than 475 students have completed their high school diploma through & Academy since Page 3

Prevention PLUS, Inc. programs target youth who: need additional assistance to complete their high school education; need more individualized instruction and attention; can withstand independent learning environments; require a flexible schedule; or, are low-income, high school dropouts who have multiple high-risk factors. The Mission Statement is to prepare urban youth for post secondary success. The Vision Statement is to build a bridge and expose urban youth to lifelong educational and career opportunities which empowers them to be positive contributing members of society. & Academy has embodied its purpose through offering: leadership development; workforce development; curriculum alignment; computer based math tutorials; block scheduling; tutoring/mentoring; partnerships with Colleges and Universities; Work Ready Certifications; community service; Economic Literacy development; advocacy; case management; job shadowing; paid work internships; job development; standardized test preparation; postsecondary placement and transition support. Prevention PLUS, Inc. contributes to the development of other youth services in the county, such as after school programs, gang violence prevention, drug and alcohol prevention, and other substance abuse and violence prevention coalitions. Prevention PLUS, Inc. is an active member of the Clayton Can Soar to the Top initiative through the Department of Health. The purpose of the initiative is to provide an ongoing opportunity for community stakeholders to work together to improve the conditions of the county, specifically focused on substance abuse reduction among youth. Prevention PLUS, Inc. is sustainable due to deep partnerships with the Department of Human Resources, Atlanta Regional Commission, Community Foundation of Greater Atlanta, YouthBuild USA and others. Over the past six years, Prevention PLUS expanded its funding to include grants focused on mentoring, civic service and postsecondary placement and transition support. Prevention PLUS, Inc. has received various types of government grants and foundation funding and had fully complied with all conditions, while meeting or exceeding all outcome measurements. The organization has grown from an annual budget of $100,000 in to an annual budget of $1.3 million for fiscal year Past and current grants include Federal grants through HHS, HUD and DOL. State of Georgia funding through the Department of Human Resources; Workforce Investment Act (DOL)--thirteen years of funding; the Children and Youth Coordinating Council (OJJDP); and, YouthBuild USA (National School Initiative, Postsecondary Initiatives, AmeriCorps, and Mentoring (OJJDP) funding as subgrantees). Local funding sources include United Way and the Clayton County Commissioners, along with various businesses and private donors. Prevention PLUS, Inc. is the recipient of The National Mentoring Alliance Award (2012), National Schools Initiative-Commitment to Excellence Award (2012) and The NAACP Outstanding Commitment to Clayton County Award (2010).
