Must be entered by a licensed* healthcare professional * Includes credentialed medical assistants! Order must be entered into EHR before any action can be taken on the order 4

7 C10 Clinical Lab-Test Results Incorporate clinical lab-test results as structured data Threshold: > 55% of all clinical lab tests results ordered during the reporting period whose results are in either: A positive/negative format Numerical format Exclusion: Any EP who orders no lab tests where results are either in a positive/negative affirmation or numeric format during the reporting period 7

8 M3 Imaging Results Imaging results are accessible through the EHR Results consist of the image itself and any other accompanying information Images do not have to be stored in the EHR Images and imaging results that are scanned into the EHR may be counted in the numerator of this measure. Threshold: > 10% of all tests whose result is one or more images ordered by the EP during the reporting period Works in conjunction with CPOE PACS that communicate via HL7 will automatically match to order Scanned results will have to be manually matched 8
