2 PURPOSE To provide psychiatric district nurse assessment and treatment for people in the Counties Manukau area who meet service criteria for Koropiko- Mental Health Services for Older People (). To work as a member of the multidisciplinary team, in the community team and in-patient unit, with those clients living in their own homes or those in residential services. To provide nursing input within the treatment of ECT in the absence of the ECT Nurse, this will include coordination of treatment, ensuring that the client if physically suitable for treatment, communication and organisation of related services, and provision of nursing input during treatment. Psychiatric District Nurses at Counties Manukau District Health Board (CMDHB) provide specialist mental health input, using nursing process to assess, implement and evaluate care, Psychiatric District Nurse services are provided in accordance with designated standards of practise and with a commitment to the principles of the Treaty of Waitangi. The Psychiatric District Nurse must be registered in New Zealand as a nurse, hold a current annual practising certificate, and a Bachelor degree in nursing, be a Registered comprehensive Nurse or Registered psychiatric Nurse, and must adhere to the nursing Council standards of Practice.

5 NATURE AND SCOPE OF RESPONSIBILITIES Key Accountability To provide assessment, treatment and intervention for existing and new service users Standards / Achievements To provide assessment, treatment and intervention in consultation with the multidisciplinary team The PDN will monitor the progress of clients in their care. To carry out a keyworking role to allocated clients, coordinating input, and implementing and maintaining required documentation To carry out risk assessment and management of clients To carry out mental state and MMSE/ACE-R examination. To work the multidisciplinary team, client and their family/whanau/carers to resolve mental health issues and return the client to an optimal level of health and functioning. To participate in competency training and annual updates as required. To work as an effective team member, attending clinical and team meetings as required. To work with team members in taking responsibility for effective collaboration with other clinicians internally/ externally, to ensure the client s safety, treatment and continuity of care. To develop and maintain an effective relationship with clients and their families/whanau during periods of involvement with To work in a culturally sensitive manner with

To carry out key tasks in a manner that respects the privacy of service users as outlined in the Privacy Act (1993). Formulate management plans in consultation with the multidisciplinary team, clearly identifying levels of risk and management of same. Provide education related to medication Monitor client s response to treatment as prescribed. To carry out assessment, treatment and interventions that recognise and protect the rights of individuals under the Code of Health and Disability services Consumers Rights (1996) To identify educational needs of clients and family/whanau Provide accurate information in regard to their illness and in a manner that is conducive to the needs of the client Provide information on available community resources. Work with the consumer and his/her carers on symptoms, recognition of early warning signs and relapse prevention (where appropriate) to assist the consumer and his family/whanau to adapt to the demands and challenges of the illness. To be responsible for all aspects of own professional development Attend individual annual performance reviews. Identifying own learning needs, and addressing ongoing personal development. Develop and maintain a professional portfolio.

9 Support and provide nurse cover to the ECT team in the absence of the ECT Nurse Attend training relevant to this role and maintain competence within this Cover planned and unplanned absences of the ECT nurse, carrying out - coordination of treatment sessions; booking theatre time, producing and distributing patient lists, booking of agency nurses as required (with authorisation) - Coordination of client treatment; ensure consent documentation is completed, ensure treatment pathway is followed and that client is medically suitable for treatment - Attendance during treatment to support the treating doctor and communicate with other parties present

10 COMMUNICATION AND INTERPERSONAL SKILLS The person holding this post will be required to interact on a regular basis with a range of CMDHB staff and maintain a close working relationship with both inpatient and outpatient, with staff from A.T&R services and staff across all Mental Health service areas Externally there will be regular contact with : - Consumers - Family/whanau - Informal support networks and community groups- - Rest Home and Private Hospital providers - All other health professionals and agencies involved with the consumer or his/her support networks - General Practitioners Situations may often call for tact, diplomacy and will require information to be handled in a discreet and sensitive manner. In conflict situations will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome. PROBLEM COMPLEXITY Will be regularly challenged by a wide range of complex Mental and Physical Health issues. There will be a requirement to be able to prioritise issues and negotiate time frames, while still providing a quality customer service. The range of problems will be diverse and require solutions customised to meet the circumstances of the client SCOPE FOR ACTION Are empowered to make decisions or recommendations relating to consumer care. Are encouraged to use initiative and problem solving skills to develop innovative approaches to nursing issues. Discretion is required to be exercised in releasing confidential information to the appropriate parties.

11 PERSON SPECIFICATIONS Qualification Experience Minimum Registered comprehensive or psychiatric nurse Current Annual Practicing Certificate Extensive knowledge of mental illness and treatment interventions At least 3 years post registration experience in a clinical setting. Extensive knowledge of pharamacology and older people A commitment to the Treaty of Waitangi and awareness of cultural safety for Maori A commitment to multicultural practice Knowledge of Mental Health Legislation Experience of risk assessment and management Clean & Full NZ Driver's Licence Preferred Comprehensive nurses have achieved the oneyear post registration certificate Evidence of educational development Previous experience within a mental health setting knowledge of the ageing process and its effects A knowledge of mental illness associated with ageing.including different types of dementia,(alzheimer s, Vascular, Lewy body, frontal lobe) Depression, Delirium, Anxiety and other functional and organic mental health problems affecting older people. An understanding of the correlation between physical health and mental wellbeing in the older person Background of working in a variety of clinical settings including mental health for older people Able to formulate achievable and

12 Skills/Knowledge/ Behaviour Ability to assess:- mental status physical well being social circumstances crisis situations and take action to resolve the situation High level of verbal and written communication skills including:- Basic counselling Negotiation/ collaboration and Problem solving skills Recognise own clinical boundaries and professional dangerousness issues and know when to call on support or refer to specialist services A positive contributing team player /member Good communication and interpersonal skills Professional attitude Able to work independently and at measurable rehabilitation goals in consultation with the consumer, family and/or caregiver.
