Hospital services will include the following units: acute admission, community 1

2 based Mental Health Services. As the HSE moves towards an Integrated Care Model of Service Provision, you will be required to work in a hospital setting or a Primary Care/Community setting across all population age groups services (i.e. Adolescent Adult) as service needs develop, or rotate between hospital Primary Care/Community Setting, /or Community Adolescent Mental Health Teams (CAHMS). Reporting Relationship Reports to the Clinical Nurse Manager III (CNMIII) / Divisional Nurse Manager as relevant to the post / as appropriate. Accountable to the Assistant Director of Nursing Director of Nursing. Purpose of the Post The post of Community Mental Health Nurse has a pivotal role in service planning, coordinating, managing activity resources within the clinical area. The main responsibilities are: quality assurance, resource management, staff development, practice development, facilitating communication professional / clinical leadership. Principal Duties Responsibilities Professional / Clinical Manage patient care to ensure the highest professional stards using an evidence based, care planning approach. Provide a high level of professional clinical leadership. Be responsible for the co-ordination, assessment, planning, delivery review of service user care. Provide safe, comprehensive nursing care to service users within the guidelines laid out by An Bord Altranais. The CMHN will practice nursing according to: Professional Clinical Guidelines National Area Health Service Executive (HSE) guidelines Local policies, protocols guidelines Current legislation. Manage own caseload in accordance with the needs of the post. Participate in teams / meetings, committees as appropriate, communicating working in co-operation with other team members. Facilitate co-ordination, co-operation liaison across healthcare teams programmes. Collaborate with service users, family, carers other staff in treatment / care planning in the provision of support advice. Communicate verbally / or in writing results of assessments, treatment / care programmes recommendations to the team relevant others in accordance with service policy. Plan discharge or transition of the service user between services as appropriate. Ensure that the service users others are treated with dignity respect. Maintain nursing records in accordance with local service professional stards. Adhere to contribute to the development maintenance of nursing stards, protocols guidelines consistent with the highest stards of patient care. Evaluate manage the implementation of best practice policy procedures e.g. admission discharge procedures, control usage of stocks equipment, grievance disciplinary procedures. Maintain professional stards in relation to confidentiality, ethics legislation. In consultation with CNMIII other disciplines, implement assess quality management programmes. Participate in clinical audit as required. Initiate participate in research studies as appropriate. Devise implement Health Promotion Programmes for service users as relevant to the post. Operate within the scope of practice seek advice assistance from his / her 2

3 manager with any cases or issues that prove to be beyond the scope of his / her professional competence in line with principles of best practice clinical governance. Health & Safety Ensure that effective safety procedures are developed managed to comply with statutory obligations, in conjunction with relevant staff e.g. health safety procedures, emergency procedures. Observe, report take appropriate action on any matter which may be detrimental to staff / or service user care or well-being / may be inhibiting the efficient provision of care. Assist in observing ensuring implementation of adherence to established policies procedures e.g. health safety, infection control, storage use of controlled drugs etc. Ensure completion of incident / near miss forms / clinical risk reporting. Adhere to service policies in relation to the care safety of any equipment supplied. Liaise with other disciplines in relation to health safety. Education Training Engage in continuing professional development by keeping up to date with nursing literature, recent nursing research new developments in nursing management, education practice to attend staff study days as considered appropriate. Be familiar with the curriculum-training programme for student nurse be aware of the clinical experience required to meet the needs of the programme. Participate in the identification, development programmes for nursing nonnursing staff. Provide support supportive supervision to staff where appropriate. Supervise assess student nurses foster a clinical learning environment. Engage in performance review processes including personal development planning as appropriate. Management Exercise authority in the running of the assigned area(s) as deputised by the CNMIII. Provide the necessary supervision, co-ordination development of nursing staff to ensure the optimum delivery of care in the designated area(s). Manage communication at service level. Provide staff leadership motivation conducive to good working relations work performance. Promote a culture that values diversity respect in the workplace. Manage all resources efficiently effectively. Lead on practice development within the clinical area. Lead implement change. Promote, facilitate participate in the development of nursing policies procedures. Monitor as appropriate lead on proactive improvement. Contribute to the formulation, development implementation of policies procedures at area hospital level. Ensure compliance with legal requirements, policies procedures affecting service users staff. Manage promote liaisons with internal / external bodies as appropriate e.g. intra-hospital service the community. Actively participate in the Nursing Management structure by acting up when required. Maintain all necessary clinical administrative records reporting arrangements. Engage in Information Technology (IT) developments as they apply to service user service administration. 3

Eligibility Criteria Qualifications /or Experience Cidates must be: 1. Registered in the Psychiatric Division of the Register for Nurses as maintained by an Bord Altranais 2. Have at least 5 years post registration experience, 2 of which must be in the Community 3. Must have, on application, a formal recognised post registration education relevant to the area of specialist practice at higher diploma level or equivalent (level 8) on the NQAI framework Have the clinical, managerial administrative capacity to properly discharge the functions of the role Demonstrate evidence of continuing professional development at the appropriate level. Skills, Competencies /or Knowledge Demonstrate the ability to lead on clinical practice service quality. Demonstrate promotion of evidence-based decision making. Demonstrate practitioner competence professionalism. Demonstrate strong interpersonal skills including the ability to build maintain relationships. 6 Community Mental Health Nurse Terms Conditions of Employment Tenure The appointment is pensionable. Whole Time hours are to be confirmed at job offer stage. Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 the Public Service Management (Recruitment Appointment) Act Remuneration The Salary scale for the post is: 47,089-55,852 (LSI) Working Week The stard working week will be confirmed at job offer stage. HSE Circular Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016 applies. Under the terms of this circular, all new entrants staff appointed to promotional posts from Dec 16 th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016). Annual Leave The annual leave associated with the post will be confirmed at job offer stage. Superannuation Membership of the VHSS Superannuation Scheme applies to this appointment. Existing Members who transferred to the HSE on 1 st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those to which they were entitled at 31 st December Appointees to posts in the Mental Health Services which formerly attracted fast accrual of service should note that the terms of Section 65 of the Mental Treatment Act 1945 do not apply to New Entrant Public Servants as defined by Section 12 of the Public Service Superannuation (Miscellaneous Provisions) Act Probation 12 months Designated Officer - Protection of Persons Reporting Child Abuse. This post is one of those designated in accordance with current child protection legislation. You will remain a designated officer for the duration of your appointment in this post or for the duration of your appointment to such other post as is included in the categories specified in the Ministerial Direction. Such officers will, on receiving a report of child abuse, formally notify the Senior Social Worker in the community care area in which the child is living. Infection Control All employees must have a working knowledge of HIQA Stards as they apply to the role for example, Stards for Healthcare, National Stards for the Prevention Control of Healthcare Associated Infections, Hygiene Stards etc. 6
