2 Exercise Goals Test specimen logistics Accessioning of large number of samples for all labs Logistics for handling large number of real world samples (i.e. urine cups) Packaging and shipping for L2 and L3 labs Test analytical capability of Level 1 and 2 laboratories Test communication between: LRN-C laboratories EPA and CDC networks Test data reporting between: LRN-C laboratories EPA and CDC networks

5 Exercise Preparation 9 participating labs x 100 samples each 30 ml urine x 900 samples = 30 L urine!!!! 5 urine pools: 4 blank urine pools 1 urine pool for spiking 3 spike levels, 2 agents, 5 replicates 16 exposed samples/lab

8 Specimen Packaging, Shipping, and Accessioning Successes: All packaged and shipped specimens arrived at proper temperature All Level 1 and 2 laboratories successfully handled receipt and acquisitioning of specimens Challenges: Broken evidence tape due to dry ice Security of specimens within building

9 Analytical Capability Capability Successes: No batch rejects based on bench QCs Good accuracy and precision for positive samples All positive for Total Arsenic by Level 2 labs (78), also confirmed positive for SBMTE and CVAA (CA-SPHL) 0.3% false positive for SBMTE (CA-SPHL) 0.3% false negative for CVAA (CA-SPHL) Bonus: expanded capabilities of Level 2 labs Network capabilities provided support when lab capabilities down

10 Analytical Capacity Capacity Successes: All Level 2 labs with capability were able to run and report results within 24 hour from receipt Level 1 labs provided mutual surge

11 Data Reporting Successes: Use of LRN Results Messenger by Level 2 and Level 1 programs to report jurisdictional sample data to the CDC Oregon LIMS and the 3-State MOA AK, ID, WA sent arsenic and CVAA result data to OR SPHL in a specified format so as to allow the data to be uploaded into the Oregon LIMS system CA-SPHL did the same when reporting SMBTE and CVAA confirmation results Challenges: Sharing data between Level 2 and Level 1 labs

12 The Unexpected: High Urine Arsenic Levels in Earmarked ND Samples CDC used 5 separate urine pools for exercise samples Urine pool #2 had an average arsenic level at 50 µg/l, the action level for confirmation shipping to Level 1 Lab 85% of Idaho and 10% of Nevada specimens contained urine from pool #2 The Geometric Mean of Total Arsenic In US Population is 8.30 µg/l (NHANES 4 th Report)

13 Lessons Learned Capability and capacity must be addressed up front on the first call Limitations in RM when reporting 2 nd party sample results to the primary party Contact information on LRN-C website not accessible by all Instrumentation software is in need of an upgrade
