The customer, to subscribe the agreement A, (annual supply) should do: 1. Tick the box ANNUAL SUPPLY AGREEMENT. 2. Fill the form, indicating in the box amount/ the number of series of samples desired for the (i.e., indicating 3 on the product FT 001 means that each month will be shipped to your lab 3 full series of this product). 3. Specify on page 8 of 9, the dates when to receive the desired products and their amount. 4. Send the subscription model filled and signed by /fax to Laboratorio Standard Latte. B) SINGLE ORDER AGREEMENT It considers the of the chosen products only for the times indicated by the Customer. The customer to subscribe the agreement B (single order) should do: 1. Tick the box SINGLE ORDER AGREEMENT. 2. Fill the form, indicating in the box amount/ the number of series of samples desired for the. 3. Specify on page 8 of 9, the dates when to receive the desired products and their amount. 4. Send the subscription model filled and signed by /fax to Laboratorio Standard Latte. Whatever is the chosen agreement it is mandatory to fill, in each part, table on page 8 of 9. It must be signed by the legal representative of the company. Each agreement is intended valid from January 1 to December 31, Is it possible to require more series of products or different series of products if communicated by /fax, by filling and signing this agreement form. Any other kind of communication is not considered valid. Is it possible to deny the agreement subscribed in every moment if communicated by /fax. Pagina 1 di 9
2 SHIPPING CONDITIONS ASSOCIAZIONE ITALIANA ALLEVATORI The raw milk are preserved with Bronopol and Sodium Azyde, both at a concentration of 0,02%. All the samples, before, are refrigerated at 3 C ± 2 C. Although to ship the samples are used polystyrene boxes and ice package, the cold chain can t be totally guaranteed. Samples must be delivered within 48 h after the shipment. Shipping is done by using express courier: UPS, TNT Traco, TNT Traco AEREO and SDA. If the Customer has already subscribed to a different express courier, he should send to LSL the account number and he will be charged only for packaging costs. Customers should contact LSL if samples don t arrive within 72 h after the shipment. Otherwise LSL will charge the full costs to the Customer. PRIVACY AND FURTHER COMMUNICATIONS Laboratorio Standard Latte (LSL) can modify, for operative reasons, the schedule of the reference materials and Proficiency Test. The LSL will inform the Customer in advance by /fax. Laboratorio Standard Latte has the due to guarantee the privacy about the customers information. Technical information about the samples are available online on: PAYMENT CONDITIONS The payment has to be regulated, by bank transfer, within 30 days from the invoice, which is sent at the end of the month (IBAN code is reported on the invoice). However, LSL may have the faculty to ask for an advance payment contemporary to the customer s order. If the Customer subscribes for more products for the same day, only one cost will be charged. In case of a missed payment, Laboratorio Standard Latte, has the faculty to block the further shipments, until the payment has been solved. The VAT is regulated according to the law.
