1 EVALUATION INFORMATION SUMMARY OF SCALE FOR THE PROPOSAL Behavioral Health Services for Sarasota and Desoto Counties ITN 212 # 2 Each of the criteria for this ITN has a score value from -1, with 1 being superior and being no value. Superior (1 points): Vendor demonstrated extensive competency, proven capabilities, and an outstanding approach to the delivery of services. Good ( points): Vendor demonstrated clear competency, consistent capability and a reasoned approach to the delivery of services. Adequate ( points): Vendor demonstrated fundamental competency, adequate capability and a basic approach to the delivery of services. Omitted ( points): Vendor has so poorly responded or has not responded at all, that a clear understanding of the response is not possible. The scores for each criterion are added together to generate the Total Score for that particular topic. A weighted value is assigned to each topic. The maximum points given to each topic will be based on the formula: Total Score x Weighted Value = Maximum. All of the individual topic scores will be added together to derive the Total Proposal Score. The Total Proposal Score for all evaluators will be added together and averaged to generate the Average Score for each vendor. Vendors will be ranked based on the Average Scores. 2
