The Team Lead is respnsible fr the day t day peratinal case management, assessment and delivery f specialist health and scial care interventins fr adults with learning disabilities perating within a culture f cntinuus service imprvement and safety t ensure prfessinal standards and best practice are embedded thrughut the team. The Team lead will prvide excellent leadership, directin and effective management f the team s resurces. As a member f the service management team ensure all team members understand, embrace and deliver t the strategic aims f the service. 2 KEY ACCOUNTABILITIES SERVICE PROVISION AND DEVELOPMENT 1. T prmte the rights, interests, needs and chices f service users, carers and ther interest grups in the planning, delivery and reviewing f health and scial care services. 2. T be respnsible fr the delivery f a cmmunity health and scial care service t adults with learning disabilities and their carers; t assess the quality and parity f decisins and t ensure cnsistent quality and targeting f the service t clients. 3. Ensure that the team perate in a cllabrative and cnstructive manner that delivers t the shared bjective f ffering the mst cst effective and apprpriate supprt slutin that meets the agreed utcmes t maintaining independence and prmting gd health utcmes. 4. Tgether with the Head f Service prmte high quality multi-disciplinary practice in the delivery f services thrugh mnitring, planning and evaluatin negtiating and agreeing service bjectives, perfrmance standards and indicatrs. 5. T be practive in develping liaisn mechanisms with relevant agencies. Identify develpment ptins fr addressing inequalities and cntribute t strategies that imprve access t services t peple frm disadvantaged grups. 6. T be aware f and have a technical understanding f all relevant legislatin and guidance and t translate as apprpriate int lcal plicy, prcedure and guidance. 7. T develp systems and prmte lcal and natinal initiatives such as Valuing Peple, Nw, Our Health Our Care, and the Barnet Learning Disabilities Partnership Bard Strategic Plan 8. T ensure that the service prvided by staff is in accrdance with the plicies, strategies and guidance f CLCH, BEH and LBB 9. T lead and cntribute t research and service develpment ensuring practice meets the need f peple with learning disabilities and their families, based n the values and bjectives utlined in Valuing Peple (2001) and Valuing Peple Nw (2009) 10. T gather strategic infrmatin t infrm the develpment f service initiatives/departmental guidelines/pilt prjects, t infrm current practice and future cmmissining 11. T mnitr the perfrmance f the department regularly t ensure that perfrmance indicatrs are being met. T wrk with planning and perfrmance department n this. T implement plicy t ensure that team members recrd data n time and crrectly. 12. Jint wrk with health and ther key agencies; ensuring implementatin f Care Prgramme Apprach, risk assessments, persn centered planning and cmpliance with clinical gvernance. 13. Manage and mnitr the team budgets and apply the principles f sund financial practice necessary t perate within the defined financial cnstraints prmting a culture f value fr mney. 14. Supprt and prmte persnal budgets and direct payments t reinfrce the principle f client chice. Agree persnal budgets in accrdance with the scheme f delegatin and in cmpliance with financial regulatins. 15. Take respnsibility fr prmting and safeguarding the welfare f adults at risk wh cme int cntact with the service, in line with 16. Barnet s prcedures. T ensure that safeguarding prcedures are implemented at all times, accurately recrded within time limits with utcmes. 17. 3 18. T carry ut audit and benchmarking t ensure that the Learning Disabilities Service is meeting the Service, LBB,CLCH and BEH MH bjectives. 19. T ensure all Serius Untward Incidents are apprpriately investigated and that any recmmendatins are implemented. 20. T ensure that all cmplaints and ther feedback frm service users are dealt with in clse liaisn with the Cmplaints Department and that necessary actin are taken t imprve the Learning Disability services as a result. OPERATIONAL MANAGEMENT AND DEVELOPMENT 1. Ensure the team perates in accrdance with plicies and prcedures f Adult Scial Services CLCH and BEHMH, and that the team members cmply with prfessinal standards, plicy and practice and the relevant statutry framewrks. 2. T regularly supervise case managers/assessment and Enablement fficers/ health prfessinals and administrative fficers either by direct invlvement r by delegatin. 3. Select/recruit and prvide inductin fr all new team members. 4. Prmte gd staff mrale by carrying ut staff appraisals and reviewing prfessinal develpment including the training needs f team members and t arrange t meet these needs 5. Deal with all persnnel and perfrmance issues including health and safety/disciplinary/sickness mnitring matters fr all team members. Liaisn with cre persnnel/persnnel cntractr and ccupatinal health cntract hlder. T wrk with HR in the CLCH and BEHMHT fr all health persnnel issues. 6. T authrize care packages up t agreed limits and screen fr suitability f supprt minimizing the need fr residential/nursing care. T ensure that all supprt plans are presented t the Learning Disability Panel fr authrizatin. 7. Supprt the drive fr excellence in custmer care thrugh identifying and helping reslve any cmplaints and service issues that may arise. 8. T c-rdinate and reslve cmplex situatins with regard t prfessinals interventins, tgether with the ther Team Managers in the service. 9. T adpt and take frward the cuncil s cmmitment t Valuing Diversity thrugh the recruitment and training f staff, and the cmmissining and mnitring f service prvisin. 10. T cntribute t the training and develpment fr the service. Ensure that there are a wide variety f pprtunities fr learning and persnal develpment, including effective inductin prgrammers fr new staff, team building events and pprtunities t review pririties fr service delivery. 11. T be respnsible fr all aspects f management, develpment and leadership within the service area. This includes respnsibility fr effective cmmunicatin and sharing infrmatin. 12. Maintain sund perfrmance management systems and perfrmance indicatrs t regularly mnitr team and individual perfrmance against targets and custmer expectatins and take the necessary early crrective actin t achieve this. 13. Safeguarding LBB, CLCH, BEH Trust are cmmitted t safeguarding and prtecting children and vulnerable adults. All emplyees have respnsibility fr safeguarding and prmting the welfare f adults, children and yung peple. This applies t emplyees with bth direct and indirect cntact with children and families. All emplyees must undertake training in safeguarding children and vulnerable adults but will have different training needs t fulfil their respnsibilities depending n their degree f cntact with vulnerable grups and their level f respnsibility. Cnfidentiality and Data Prtectin Emplyees will have access t cnfidential infrmatin and will be required t ensure that the highest level f cnfidentiality is maintained at all times, adhering t all plicies relating t cnfidentiality. Emplyees are required t btain prcess and/r use persn identifiable infrmatin in a fair and lawful way. 5 Access t Scial Services and Health Recrds All staff wh cntribute t service user recrds are expected t be familiar with, and adhere t CLCH s, LBB s and BEHMH Recrds Management Plicy. Staff shuld be aware that patients recrds will be the subject f regular audit. In additin, all prfessinals are advised t cmpile recrds n the assumptin that they are accessible t service users in line with the Data Prtectin Act All staff that have access t service user recrds have a respnsibility t ensure that these are maintained and that cnfidentiality is prtected. Health and Safety All staff are required t cmply with the requirements f the Health and Safety at Wrk Act and ther relevant health and safety legislatin and lcal plicies and prcedures. All staff are required t make psitive effrts t prmte their wn persnal safety and that f thers by taking reasnable care at wrk, by carrying ut requirements f the law r fllwing recgnised cdes f practice and c-perating with safety measures prvided r advised by CLCH, LBB and BEHMH Trust t ensure safe wrking. Managers are respnsible fr implementing and mnitring any identified risk management cntrl measures within their designated area/s and scpe f respnsibility. In situatins where significant risks have been identified and where lcal cntrl measures are cnsidered t be ptentially inadequate, managers are respnsible fr bringing these risks t the attentin f the apprpriate Cmmittee if reslutin has nt been satisfactrily achieved. All staff must cmplete infectin cntrl training at inductin, and all staff whse duties invlve patient cntact must als cmplete infectin cntrl refresher training annually.
