1 Prior Learning Assessment and Recognition (PLAR) Course Credit Challenge Student Application Package Name of Applicant: School: Date: Prior Learning Assessment and Recognition (PLAR) is the formal evaluation and credit-granting process whereby students may obtain credits for prior learning. Prior learning includes the knowledge and skills that students have acquired, in both formal and informal ways, outside secondary school. Students may have their knowledge and skills evaluated against the expectations outlined in provincial curriculum policy documents in order to earn credits towards the secondary school diploma. Only credits for Grade 10, 11, or 12 courses developed from provincial curriculum policy documents published in 1999 or later may be challenged through the PLAR process. A maximum of two credits in any one subject discipline are permitted. A student cannot be granted a credit for a course where a credit has already been granted for a later grade of the same subject discipline. The following courses may not be challenged through the PLAR process: Any course that has been attempted in the previous 4 years, Grade 9 courses, transfer courses, locally developed courses (1L, 2L, IDC), cooperative education courses, ESL/ELD (if the student has completed ENG course credits). Steps in the PLAR Challenge Process: 1. Student submits this completed application package, before October 31, of the applicable school year to Peak Centre Academy. 2. PLAR committee reviews applications for eligibility and sufficient evidence. 3. Applicant is notified of the committee s decision by the end of November. 4. Required PLAR teacher component is organized. 5. Successful student PLAR applicant works with PLAR teacher, between January and April, to plan, and complete tasks to provide evidence of student learning for all remaining overall expectations (70% term tasks), including the 30% final summative task. 6. The PLAR teacher maintains the PLAR Record of Assessments and Evaluation, and determines the final grade. 7. The PLAR Record of Assessments and Evaluation, with final course grade, is provided to the guidance counselor, to allow the principal to grant the credit. 8. Peak Academy enters the course credit and final grade onto the official student transcript (OST). The original PLAR Record of Assessments and Evaluation is filed in Ontario Student Records (OSR), with a copy filed with the Guidance Counselor. 1

2 PLAR Application Package Prior Learning Assessment and Recognition (PLAR) The following components of the PLAR Application package must be completed and submitted to Peak Centre Academy prior to October 31st of the current school year. 1. PLAR Challenge Application Package This application package must be completed and signed, by the student, the required references, parent, guidance counselor and school principal. Please ensure you retain a copy of all documents for your personal files. 2. Ontario Student Transcript (OST) Your Ontario Student Transcript may be obtained by contacting the Peak Centre Academy Guidance Counselor. A copy of your most recent OST must be included in the PLAR application package that you submit. The Ontario Student Transcript provides an official record of the Ontario secondary school credit courses successfully completed by the student. 3. KNOWLEDGE AND SKILLS (Evidence of Learning) PROFILE The Knowledge and Skills Profile provides a rationale, evidence of prior learning and references to support this evidence. This profile is developed by the student, along with support from references, and includes clear evidence of learning for each of the overall expectations of the course being challenged. It also provides the rationale for seeking a credit using the PLAR challenge process. The Knowledge and Skills Profile consists of: a) Rationale The student describes the rationale for seeking a credit using the PLAR challenge process. The student must include a description for how this course credit will help fulfill their educational goals, in terms of their remaining high school years, and any possible post-secondary plans. Unique circumstances, including any individual education plan (IEP) information may be described here, including a need to have a more flexible schedule. Knowledge and Skills Profile RATIONALE FOR PLAR CHALLENGE b) Evidence of Relevant Learning The student is to provide a description, with clear evidence of successful learning, aligned to the overall expectations of the course being challenged. Any samples of original work being submitted must be referenced in this section, including alignment to appropriate overall expectation. This description must be submitted on the form titled Knowledge and Skills Profile EVIDENCE OF RELEVANT LEARNING. c) Reference(s) There must be two reference letters submitted. These may be from a teacher or instructor who has worked with a student in an area related to the course being challenged, and provide support for the specific evidence of relevant learning provided by the student. The reference(s) must be submitted on the form titled Knowledge and Skills Profile REFERENCE. 4. Individual Education Plan (IEP), if applicable 2

Knowledge and Skills Profile: Rationale, Evidence of Relevant Learning with samples, References Signatures for Application I am aware that a passing or failing mark resulting from a challenge for credit for a Grade 11 or 12 course will be entered on my Ontario Student Transcript and that a passing or failing mark or a withdrawal resulting from a challenge for credit for any Grade 10, 11 or 12 course will be entered on my PLAR tracking record and maintained in my Ontario Student Record. All credits granted will be entered on the Ontario Student Transcript. I am aware that the PLAR challenge process will include rich assessment tasks (balanced between written work and demonstration, as appropriate for the subject) worth 70 percent of the final mark, and other types of assessment worth 30 percent of the final mark. I am aware that my skills and knowledge will be evaluated against the expectations outlined in the appropriate provincial curriculum policy document. I am aware that a maximum of four credits may be granted through the challenge process for courses in Grades 10 to 12, with no more than two in any one discipline. I certify that the information given on this application and in any documents attached is correct, complete, and fully discloses all prior PLAR challenges. I hereby acknowledge and confirm my application to the PLAR challenge process and understand that submission of this form to Peak Academy is a commitment to the completion of the assessment tasks for the course indicated above.
