3 State Performance Measures (SPM) SPM 1: Percentage of women who smoke in the last three months of pregnancy SPM 2: Percentage of children, ages 2 to 5 years, receiving WIC services with a Body Mass Index (BMI) at or above the 85th percentile SPM 3: Rate of birth per 1,000 female adolescents aged years SPM 4: Percent of children, ages 1 through 17, who had a preventive dental visit in the past year

PERINATAL/INFANT HEALTH SPN 1: Decrease infant mortality NPM 03: Percent of very low birth weight (VLBW) infants born in a hospital with a Level III+ Neonatal Intensive Care Unit (NICU) NPM 04: A) Percent of infants who are ever breastfed; B) Percent of infants breastfed exclusively through 6 months NPM 05: Percent of infants placed to sleep on their backs ESM Percentage of birth facilities with documented level of care consistent with AAP guidelines using the LOCATe tool ESM Number of birthing hospitals designated as Baby Friendly. ESM Percentage of mothers who report receiving breastfeeding information from a healthcare provider. ESM Percentage of birthing hospitals who have a safe sleep policy.

5 CHILD HEALTH ADOLESCENT HEALTH CYSHCN HEALTH CROSS-CUTTING/LIFE COURSE SPN 3: Increase access to SPM: Percentage of children, ESM Number of trainings comprehensive health care ages 2 to 5 years, receiving WIC provided to child care providers, SPN 8: Increase child nutrition services with a Body Mass Index home visitors, and other early and early childhood obesity (BMI) at or above the 85th learning professionals on Ages & prevention percentile Stages. NPM 06: Percent of children, ages 10 through 71 months, receiving a developmental screening using a parent completed screening tool SPM 4: Decrease teen pregnancy and teen birth rate NPM 10: Percent of adolescents, ages 12 through 17, with a preventive medical visit in the past year SPN 2: Increase access to health care/medical homes NPM 11: Percent of children with and without special health care needs having a medical home SPN 5: Increase health insurance coverage NPM 15: Percent of children ages 0 through 17 who are adequately insured SPM: Rate of birth per 1,000 female adolescents aged 15-19ars SOM: Rate of pregnancy per 1,000 female adolescents aged years SPM 4: Percent of children, ages 1 through 17, who had a preventive dental visit in the past year ESM Number of clinic sites engaged in youth-centered care quality improvement cycles. ESM Number of practices receiving education or technical assistance about medical home/family-centered care. ESM Percentage of currently insured Children with Special Health Care Needs (CSHCN) whose insurance is inadequate.

6 Infant Mortality CoIIN Update Safe Sleep Partnership with all MS birthing hospitals to distribute safe sleep books and work on safe sleep policies. Partnering with the Direct On Scene Education (DOSE) program to train local EMTs and implement the program across the Jackson-Metro and surrounding counties. Education and training through the Sisters United Graduate Sorority Program. All six public health educators now trained in safe sleep by the MS SIDS and Infant Safety Alliance. Health Educators will educate the public on safe sleep in various settings Infant Safe Sleep Conference

7 Infant Mortality CoIIN Update Through partnerships MSDH: First in-state Certified Lactation Counselor Training. First Baby Café in Greenville, Mississippi ( Washington County). Provide cribs to needy families. Through partnering with our WIC program: Mississippi WIC is currently using Pacify, a mobile application used to provide breastfeeding peer counselors and mothers access to professional lactation support (lactation consultants, registered dietitians, and registered nurses) via video consultation. Mississippi WIC is funding a social marketing campaign entitled Breastfeeding As A Superpower consisting of the development and initiation of radio and television commercials to be aired statewide in the month August for National Breastfeeding Month.
