1 Hospital Engagement Network CDC/NHSN Join Group and Group Data Template Joining the Group in NHSN To join the group in NHSN, you will need to login to the CDC/NHSN tool and navigate to the Group, Join page. Group ID: Group Joining Password: HREThen123 (password is case sensitive) Click on the Join Group button A warning message will pop up. Click OK to proceed.

2 Group Data Template Once you have joined the group, you will be taken to the web page in which you need to review the data fields that HRET has requested from your hospital. Once you have agreed to accept the template, you will be asked to confer the rights of your data to HRET. HRET HEN CDC/NHSN Data Template The screen shots below outline the information in the template that HRET has created. Please review the fields before clicking on Accept or Back. The General section reviews the patient information. HRET has selected to get patient data without any identifiers.

3 The Infections and other Events (Not specific to MDRO/CDI) section reviews the Event, timeframe and if the data requested is inpatient, outpatient or both. HRET has selected to receive the following data elements IF your hospital submits them to CDC: BSI Bloodstream Infection (CLA), Inpatient only, starting January 1, 2010 through the end of the project. These are BSI in all location types and all locations. If your hospital only collects BSI for ICU, your data will be included in the collection. CLIP Central Line Insertion Practices, Inpatient only, starting January 1, 2010 through the end of the project. These are CLIPs in all location types and all locations. If your hospital only collects CLIP in the ICU, your data will be included in the collection. PNEU Pneumonia (Vent), Inpatient only, starting January 1, 2010 through the end of the project. These are Ventilator Associated Pneumonias in all location types and all locations. If your hospital only collects VAPs in the ICU, your data will be included in the collection. SSI Surgical Site Infection, Inpatient only, starting January 1, 2010 through the end of the project. These are all Surgical Site Infections in all location types and all locations. If your hospital only collects SSI for CBGC, your data will be included in the collection. SSI Surgical Site Infection, Outpatients only, starting January 1, 2010 through the end of the project. These are all Surgical Site Infections in all location types and all locations. If your hospital only collects SSI for AVSD, your data will be included in the collection. UTI Urinary Tract Infection, Inpatients only, starting January 1, 2010 through the end of the project. These are all UTI (Cath) in all location types and all locations. If your hospital only collects UTI for ICU, your data will be included in the collection.

4 Denominator Data for Events this section is used to request access to procedure denominator data for the procedure-associated modules. Antimicrobial Use; Summary Data for Vaccinations; MDRO/CDI Events; MDRO/CDI Summary Data (Denominators) these are sections HRET is not requesting data from. MDRO/CDI Process & Outcome Measures Inpatient only, starting January 1, 2010 through the end of the project Hand Hygiene or Gown and Gloves Process Measure data. Once you have reviewed the fields, at the bottom of the template review, clicking on the Accept button means your hospital has accepted the template and your data will be shared with HRET. Clicking on the Back button means that your hospital does not accept the template and your data will not be shared with HRET.
