1 Midwest Is Tops for Top Hospitals, Says Report Of the nation s 50 top hospitals in terms of patient outcomes, 21 (42%) are in 7 Midwestern states, according to new hospital ratings released today by Healthgrades, a healthcare quality reporting group. And of the nation s top 20 cities with the lowest in-hospital mortality rates, 12 are in the Midwest, and none are on the West Coast or in New England. The new Healthgrades rankings build on an earlier report this year that listed the top 262 (5%) of hospitals when it comes to risk-adjusted mortality and complication rates for 27 common conditions and procedures involving Medicare patients. Conditions range from sepsis to bowel obstruction. Procedures include hip replacement, back and neck surgery, and carotid surgery. Hospitals are graded for each condition and procedure with stars 5 for outcomes that are statistically better than expected, 3 for outcomes that are as expected, and 1 for outcomes worse than expected. The 262 institutions with the highest average score comprise Healthgrades Distinguished Hospitals for Clinical Excellence in Healthgrades bestows its best-hospital designation on the elite within the Clinical Excellence category. To make it into what the group calls "America s 50 Best Hospitals," an institution must earn the award for at least 7 consecutive years. The top 50 represent 1% of the nation s hospitals. The 100 best hospitals, in Healthgrades methodology, represent the top 2%. They have earned the Clinical Excellence award for at least 4 consecutive years. Large hospitals are 3 times more likely to fall into the top 50 or 100 categories than small or medium-sized hospitals. For-profit hospitals account for roughly 16% of all hospitals, but only 6% of the top 50. Table 1. 4 4 of 5 The Odds Are With Patients in Dayton, Ohio The list of top 20 cities for the lowest in-hospital mortality rates from Healthgrades is based on "core based statistical areas" (CBSAs) used by the US Census Bureau to refer to individual cities as well as broader metropolitan regions combining several cities. Healthgrades only considered CBSAs with at least 4 hospitals, one of which must have been eligible for a Clinical Excellence Award. The group calculated an average, risk-adjusted mortality rate across 16 procedures and conditions based on 5 years worth of Medicare data. The in-hospital mortality rates for Healthgrades top 20 CBSAs is lower on average for the 16 procedures and condition than the national average of 6.52%. Number one on the Healthgrades list is Dayton, Ohio, with an in-hospital mortality rate of 4.68%. It was 1 of 4 Ohio cities or metro areas in the top 20. Table 2.
