Services provided for Myanmar citizens The Embassy of the Union of Myanmar in Brussels has been providing the following services for Myanmar Citizens: (a) Renewal of Passport (b) Renewal of Myanmar Passports for Students (c) Issuance of New Passport (d) Renewal of Certificate of Identity ( C of I ) (e) Issuances of New Certificate of Identity ( C of I ) / Travel Document (f) Attestation of Powers of Attorney (g) Attestation of Certification (h) Other Matters (i) Matters needed to be referred to the respective Ministry (a) Renewal of Passport (1) Passport original and photo copy (2) Up to date receipt of income tax (3) Renewal fees, Euro (80) for three years. Note : Myanmar Passport holders are required to renew their Passports (2-months) before the date of expiry. (b) Renewal of Myanmar Passports for Students (1) Application letter addressed to the Ambassador, Embassy of the Union of Myanmar. (2) Letter of attendance / confirmation of student status from the respective school ( also to mention the duration of study required) (3) Photo copy of Alien Registration Card (4) Photo copy of Passport ( every stamped pages ) (5) Two color photographs with soft copy of JPEG format (6) Personal history form ( C.V. form ) (c) Issuance of New Passport (1) Application letter (2) Copy of all pages of the Passport (3) Income tax payment must be up to date when applying for a new Passport (4) Two color photographs with soft copy of JPEG format (5) Personal history form ( C.V. form ) visa@embassyofmyanmar.be

6 (6) New Passport Fees Euro (80) Note : Myanmar Passports holders and their dependents are eligible to apply for a new Passport when there are only a few pages left. (d) Renewal of Certificate of Identity ( C of I ) (1) Application letter by parents / guardian (2) Up to date income tax receipt of parents/ guardian (3) Photo copy of C of I (4) Two color photographs with soft copy of JPEG format (5) Renewal fees Euro (40) (e) Issuance of New Certificate of Identity ( C of I ) Application for children (1) Application letter by parents / guardian (2) Up to date income tax receipt of parents/ guardian (3) Copy of current C of I ( if applicable ) (4) Personal history form ( C.V. form ) (5) Two color photographs with soft copy of JPEG format (6) Issuance fees Euro (40) Application for detainees (1) Application letter of true story written in Burmese language (2) Original passport (or) copy of passport / Citizenship Card / National Registration Card (3) Personal data form for detainees (4) Two color photographs with soft copy of JPEG format (5) Issuance fees Euro (40) (f) Attestation of Powers of Attorney (1) Revenue stamped document. (2) Signature of 2 witnesses. (3) Particulars of witnesses such as Passport No, NRC No / ID No and address, tel/fax nos, mobile no, must be stated. (4) Person authorizing General Power / Special Power must come to the Embassy in person and sign before the consular officer (original passport needed ). 7 (g) Attestation of Certification Various documents such as Birth Certificate / Marriage Certificate / Degrees / Diploma / Driving license... etc needed for attestation / certification should be notarized by a registered Notary Public in Myanmar and endorsed by the Ministry of Foreign Affairs ( MOFA ) in Myanmar, prior to submission to the Myanmar Embassy. (h) Other Matters (1) For First Time Payment of Income Tax (1) Company letter stating employees' monthly basic salary. (2) Application Letter addressed to the Ambassador, Embassy of the Union of Myanmar. (3) Two color photographs (4) Personal History Form ( C.V. form ) (2) Loss / Damage of passport (1) Application letter stating detail of loss / damage (2) Police report ( for lost only ) (3) Copy of passport (4) Income tax payment must be up to date when applying for a new Passport (5) Personal history form ( C.V. form ) (6) Two color photographs with soft copy of JPEG format (7) New Passport Fees Euro (320) (8) The new passport will be issued when approval received from the Passport Issuing Board in Myanmar (i) Matters needed to be referred to the respective Ministry (i.e. Passport issuing board, Ministry of Home Affairs, Ministry of Immigration and Population) (1) Inclusion of new born child in mother's Passport (2) Change of status (3) Separation of child from mother's Passport and issuance of new Certificate of Identity ( C of I ) (4) Inclusion of a child who holds C of I to mother's Passport (5) Change of status to dependent of foreign husband or wife (6) Change of status from seaman to shore job and from shore job to seaman. (7) Application for C of I (or) Travel Document for those who residing illegally in abroad without valid travel document, residence permit.
