2 Chlorhexidine preop bathing/showering has been recommended by the CDC since 1998 and has increasingly become a standard of practice for prevention of surgical site infection. Infection Control Subcommittee has recommended the use of Chlorhexidine gluconate (CHG) wipes for our patients at CRH. Patient Safety is a priority! 2

3 Cleansing of the skin removes dirt and debris Bathing prior to surgery is essential PAT-patients will be instructed to shower the night before and the morning of surgery In patients-should be bathed within 12 hours of surgery For patients with poor personal hygiene, make sure the skin and skin folds are clean Do Not use lotions/powders after bathing prior to surgery 3

4 Abnormalities of the skin can place the patient at risk for infection and can result in the surgery being cancelled If any of the problems mentioned below are identified, they need to be reported to the surgeon and control room (ext. 5264) ASAP Assess the patients skin for: Rash/open areas/cuts/lesions/bug bites, etc at or around the surgical site: head, neck, chest, abdomen, back, joints, legs, groin, etc Skin rash or fungal infection in skin folds Potentially infected IV site or procedure site 4

5 Provides rapid bactericidal action against a broad spectrum of microorganisms Demonstrates continued antimicrobial activity for up to 6 hours after application Alcohol free and also contains aloe and glycerin (safe for skin) Rinse-free Scrubs away debris and also protects the prepped area Each cloth contains 500mg of CHG 5

6 Allergy to Chlorhexidine or any other ingredient in the product Do not use for lumbar punctures or in contact with the meninges If the patient has an open wound; dressed or undressed Skin Rash If the scheduled procedure indicates any type of infection Minor procedures i.e., lumps, bumps (tumors, lipomas) Do not use as a general skin cleanser NOTE: Do not use the cloths on your face, ears, eyes, nose, mouth or private parts (genital area). If the cloths do touch these areas, rinse them well with clean, warm water to prevent irritation CHG wipes are not effective against C-Diff 6

7 Selected patients that meet the criteria! Every patient (that meets criteria) with an external incision, starting at the neck or below will receive full body CHG wipes (3 packages of 2). 7

8 Nurses: RN s, LPN s The nurse can delegate this task before or after the skin assessment is completed The patients and/or family/friend/significant other can use the wipes after fully instructed on how to do so Care partners with proper training Remember: Privacy is very important for our patients. Please drape or cover the patient and knock on the door before entering. 8

9 Follow diagram shown Wet the surgical site area and let it air dry for 1-3 minutes Do not wipe dry Wipe 1 is for neck to belly Wipe 2 for both arms Wipe 3 for belly and hips Wipe 4 for legs Wipe 5 for back Wipe 6 for lower back and buttocks If the patient has cast/splint; cleanse down to the cast/splint and stop. If the patient has an open wound, skin rashes; dressed or undressed DO NOT use CHG wipes. Discard each cloth after a single use After package has been opened discard any unused cloths Store CHG wipes flat Store between 68-77* F Avoid excessive heat above 104* Do not microwave 9

10 AdHoc-Preprocedure Information Form The CHG wipes need to charted separately from the Chlorhexidine shower or bath complete Chart in other for completion of CHG wipes. Preparations are being made to start a process in changing the Patient Preparation field and the blue wording in the screen. 10
