2 Aims and objectives The purpose of this study was to shake the healthcare management to encourage Medical Imaging Technologist to justify portable radiography and make them authorize to play important role in radiation protection as well. Additionally, it aims to sort out the reasons behind an increased number of unjustified portables x rays.moreover, there should be proper training of internal medicine's residents to teach them about justification and harmful effects of ionizing radiations. Some appropriate teaching sessions should be organised for medical imaging technologist to tell them the importance of their role in justification of single medical exposure. Its of dire importance to educate nurses and healthcare assistants on preliminary basis as well. Monitoring is a definitive tool for improvement in avoiding unnecessary portable x rays. Methods and materials A descriptive study conducted in which data collected from February to September In this time period, 1071 portable x-ray tests were ordered. However, ICU patients' X- rays are excluded since they are being performed in early morning daily. Radiology consultants and residents helped in this study along with medical imaging technologist (MIT). For this study, a discussion made for unjustified portable X-rays ordered and tried to lower their number to improve practice. Results Out of 1071 portable x rays ordered, 102 were cancelled by elaborating justification rules with concerned residents (Figure 1). However, (2.4%) unjustified portable x-rays were performed because of different reasons that would be discussed later. Images for this section: Page 2 of 7

4 Conclusion The significance of referral pattern guidelines enhancing compliance with justification highlighted in this study. Conclusively, the tools that facilitate justification are awareness, appropriateness and audit (the three A's campaign) required to be developed. The key players of this campaign would be Radiologist, MIT and IM residents. Moreover, there are 3 basic parameters on which justification of portable x rays can be done.first, quality is compromised due to limitations of portable machine's sensitivity and specificity. It is evident that image quality and contrast of the radiograph is not adequate [Figure 2]. Second, extra or unnecessary dose. i.e., irradiating not only to the patient but neighbor bed patient and the operator as well. It is stated in literature that radiation dose is higher by portable x ray machine than that of performed by digital X-ray machine in the radiology department. Patients' rooms are not lead shielded and to achieve better quality of radiograph more exposure factors are given. A comparison is shown in table 1. Third is cost effectiveness. If the patient is paying its not fair to irradiate with x rays and charge for it too. Images for this section: Page 4 of 7
