5 154 SALVIA & MOUTAN Dān Shēn/Mǔ Dān Pí Salvia miltorrhiza & Paeonia suffruticosa Dān Shēn Mǔ Dān Pí bitter, slightly cold acrid, bitter, slightly cold activates Blood, removes Blood stasis, cools Blood, clears the Heart, nourishes Blood, calms shen clears Heat, cools Blood, activates Blood, disperses Blood stagnation, removes deficiency Heat Combined action Cools Blood, stops bleeding, removes Blood stasis, generates new Blood. Indications Heat in the Blood with yīn deficiency, continuous low fever. Irregular menstruation, amenorrhoea, dysmenorrhoea, postpartum bleeding. Skin patches, haematemesis, and epistaxis due to Heat entering in the yíng-blood system. Red, swollen and painful joints due to Heat-bì syndrome. Combinations For low fever, add Biē Jiǎ/Dì Gǔ Pí/Qīng Hāo. For irregular menstruation, add Chuān Xiōng/Dāng Guī. For high fever, combine with Huáng Lián Jiě Dú Tāng. Can be added to Táo Rén/Hóng Huā and Sì Wù Tāng for menstrual problems due to Blood stasis complicated with Heat. Dosage The typical proportion when added to a constitutional formula is 6:1 formula to addition (15 grams in grams of formula). If a stronger effect is desired more can be added. When used alone the dose is three grams twice daily. Cautions L and contraindications x L None noted. x None noted. YELLOW EARTH DECOCTION Huáng Tǔ Tāng Yellow Earth Decoction Source: Essentials from the Golden Cabinet (Jīn Guì Yào Lüè, 200) Keywords: gastrointestinal bleeding and dysfunctional uterine bleeding Ingredients Terra Flava Usta (Zào Xīn Tǔ, ignited yellow earth), Glycyrrhiza uralensis (Gān Cǎo, liquorice), Rehmannia glutinosa (Gān Dì Huáng, dried rehmannia), Atractylodes macrocephala (Bái Zhú, atractylodes), Aconitum carmichaeli (Fù Zǐ, aconite), Equus asinus (Ē Jiāo, ass-hide glue), Scutellaria baicalensis (Huáng Qín, scute) Major therapeutic action Warms yáng, strengthens the Spleen, nourishes Blood and stops bleeding.
6 56 PEONY & CITRUS FORMULA Tòng Xiè Yào Fāng Important Formula for Painful Diarrhoea Source: Collected Treatises of Zhāng Jǐng-Yuè (Jǐng Yuè Quán Shū, 1624) Keywords: digestive problems with an emotional component, irritable bowel syndrome Ingredients Paeonia alba (Bái Sháo, white peony), Atractylodes macrocephala (Bái Zhú, atractylodes), Ledebouriella divaricata (Fáng Fēng, siler), Citrus reticulata (Chén Pí, citrus) Major therapeutic action Harmonises the emotions and digestion, strengthens the digestion, and stops diarrhoea. Biomedical action Antispasmodic, analgesic, astringent. Indications Emotional problems coupled with digestion problems. A variety of symptoms may occur with this pattern, most typically gastrointestinal pain with altered bowel habits. The main feature is the clear relationship of onset or aggravation of symptoms to the emotional state of the patient. With the appropriate key symptoms this formula can be used to treat disorders such as irritable bowel syndrome and ulcerative colitis. Key symptoms for use chronic or recurrent diarrhoea, or alternating diarrhoea and constipation cramping abdominal pain relieved by defecation borborygmus, flatulence, loss of appetite, bloating Differentiation Sì Nì Sǎn acts primarily on the liver, and has significantly less intestine soothing activity. Better in general for the upper gastrointestinal effects of the emotions affecting digestion. Xiāo Yáo Sǎn: Xiāo Yáo Sǎn is best for the effects on the gynaecological system. Combinations With severe abdominal bloating, add Zhǐ Shí & Jié Gěng. With symptoms of anorexia, postprandial abdominal distension, lethargy, and a sallow complexion, combine with Sì Jūn Zǐ Tāng. With abdominal pain, add Jīn Líng Zǐ Sǎn. With watery diarrhoea, add Gān Jiāng & Ròu Guì. With severe irritability, depression, pain or distension in the hypochondrium, and dysmenorrhoea, add Chái Hú & Bái Sháo. Dosage The typical dose is three grams twice daily, before meals. In severe or acute cases, or the early stages of treatment (the first two weeks), a % increase in dose may be used, then reduced as the treatment takes effect. Cautions L and contraindications x L None noted. x Contraindicated in the early stages of a febrile illness, or when there is residual pathogenic influence remaining; viral or bacterial.
