60 OBSERVATIONS DH Anantnag Dental X-ray films and 10 x12 X-ray films were purchase for Rs from SBS traders Anantnag when the rates quotated by the dealer was not lowest as per the comparative statement X-Ray developer was purchased from 3B Medicine Services Anantnag for Rs per unit when the rate quotated by the same dealer was Rs per unit only. Dental equipments has been purchased for Rs from SN traders Anantnag without performing codal formalities. Optical Ocular Lens were purchased from Intra Ocular Care Vododra for Rs without performing codal formalities.
61 PHC B. Kalan Block Bijbehara OBSERVATIONS Condt.. Genset has been purchased out of RKS funds for which quotations were called on 10th March The dates mentioned on quotations are 15th March 2014, Supply order was placed on 20th March 2014 but the date mentioned on bill of cost is 15th March 2014, that is 5 days prior to date of supply order. PHC Ashmuqam Block Mattan, Air Conditioner has been purchased for labour room for Rs , but the rates quotated by the dealer were not lowest. CHC Kokernag Block Larnoo Both the RBSK teams were in the HQ till AM. Driver of team A said he has to pick up doctors from their residence which gets late. Tour plans were not available. SC Panzgam Block Larnoo The service delivery for ANC and Immunization is getting affected as the concerned ANM has to visit NTPHC Kherpora on alternate Wednesday for ANC checkups and Immunization.
62 OBSERVATIONS Condt.. District Shopian In District Hospital,JSSK drugs need to be purchased in consulation with the Gynecologists, as Gynecologist prescribe those medicine which are not available at the hospital under JSSK. FMPHW of Sub centre Memander had been attached to sub-centre Reshi Nagar, due to which the target of ANC is not been achieved as there is MMPHW posted. Sub Centre Zawoora was closed on dated at 12:30 pm. In PHC Berthipora, Medical officer incharge,dental technician, Basic Health Worker was present rest of staff was not available at 10:30 am. Sub centre ANMs are put at night due to which routine work of Sub Centre suffers.
63 OBSERVATIONS Condt.. District Pulwama In NTPHC lethpora, the registers of MCTS were not properly maintained. In PHC Khalil, no IEC is displayed outside the building or around the area. In CHC Tral, Blood storage is not established. In CHC Tral, labour room is not having autoclave,led lights. Proper management of Biomedical Waste Management is not available at all level. RCH registers of Sub centres were not properly filled. ANMs need to be oriented on RCH register.
64 OBSERVATIONS District Kulgam Condt.. Blood Storage Unit at CHC Qazigund, which is also an accidental hospital has not been established yet. Segregation and disposal of BMW is very poor across all the facilities in district Kulgam. Children referred under RBSK are not provided free diagnostic tests and medicines. Gynaecologist is not available at CHC Qazigund & CHC Yaripora. Most of the cases from these FRUs get referred to MCH Anantnag. Codal formalities for procuring drugs/ supplies and other equipments are not being observed as per the financial guidelines and Rate Contract of District Purchase Committee of District Kulgam at most of the health facilities. One firm S.Z. Scientific Traders, Anantnag was found at many institutions supplying drugs and equipments at higher rates than approved by District Purchase Committee, Kulgam. At CHC Qazigund, various items like baby warmer, Phototherapy Units, OT lights, Suction Apparatus, etc. amounting to Rs /- were procured from M/s S.Z. Scientific Traders Anantnag vide bill No & 8408 dated without taking approval from the RKS, Committee and without calling any quotations for the same products.
65 OBSERVATIONS District Kulgam Condt.. At CHC Yaripora, Inj. Epidosin approved by District Purchase Committee, Kulgam to Chemi Health Diagnostics, per ampoule has been purchased from Rainbow Traders (Bill No dated: per ampoule plus 5% VAT. At PHC Qaimoh, 2 no. X-ray tray purchased out of RKS funds from S.Z. Scientific Traders which is not approved by DPC, per tray vide bill No In the same bill, episiotomy set has been per set whereas approved rate of DPC, Kulgam is Rs.650/- per set. However, at PHC Frisal, X-Ray tray has been has been purchased from Dawood Medical per tray only. At CHC DH Pora, as per the comparative statement of five firms for procurement of Semi Automatic Biochemistry Analyzer (BTS-350), purchased from R.K Enterprises for Rs /- which is higher than the rate quoted by M/s S.Z. Scientific Traders Anantnag (Rs /-) having the same specification, which is against the financial norms. MOs & other staff are doing night duties for 3 consecutive days and are taking night off for next 4 days. ANMs posted at SCs have been put on night duties, which affects the functioning of SC.

68 National Health Mission, J&K Appointment of Concurrent Auditors of District Health Societies for the financial year , shall be made as per guidelines of Govt. of India and direction from the office of Mission Director, NHM.(in this regard letter has issued on 25/8/2014) Annual Audit of Rogi Kalyan Samities of District Health Society Bandipora, Baramulla, Ganderbal, Kargil and Kupwara pending. However, reports are still awaited from all the District Health Societies of Kashmir division. Tax Deduction Account Numbers (TAN) is yet to be obtained by the District Health Society Kargil and Pulwama. Observations of Concurrent Auditor for 4 th quarter of , were shared with District Health Society Budgam, Baramulla, Ganderbal, Kulgam, Kargil, Leh, Pulwama, Srinagar and Shopian. Replies of awaited from said districts.
69 National Health Mission, J&K Functionality of 102 Ambulance Services: Performance of M/s Esteemed industries (agency hired for implementation of 102 service in the state) with regards to the fitting of GPS devices in the ambulances & functioning of 102 Call Centre is very poor. DHS(K) has been asked to take action against the concerned agency and ensure the full implementation of the service.

71 National Health Mission, J&K Functionality of Rashtriya Bal Swasthya Karyakram: Establishment of DEICs need to be expedited as none of the 09 approved DEICs is functional till date. Pharmacists under RBSK have not been hired by District Anantnag, Baramulla, Ganderbal, Pulwama & Shopian till date. Vehicles hired under RBSK are not being managed properly as the tendering has been done at District Level and their payment is also made at District Level which leads to the misuse of vehicles beyond working hours as BMOs have no control over the vehicles.
72 National Health Mission, J&K Functionality of National Urban Health Mission: NUHM has not been rolled out in the State till date. Information regarding Status of planning and mapping of the cities, progress in Human resource recruitment and capacity building, status of civil works, location and progress in establishment of UPHCs & KIOSKs and the DPRs and the time of completion for the upgradation of 2 UPHCs into UCHCs. has been solicited by GoI which needs to be furnished at an earliest.

MDR District Pulwama has reviewed only 2 Maternal deaths out of 7 MDs reported during this FY Child Health National Health Mission, J&K No progress for SNCU sanctioned in SKIMS Medical College Bemina.
74 National Health Mission, J&K RKSK The Report of ARSH which should come by 5 th of every month is always delayed.. With reference to the letter no SHS/JK/NHM/ dated no funds have been recouped on account of sale of sanitary napkins to State Health Society till date from district Baramulla and Bandipora.
