1 Mid-South Food Bank will not accept the following offenders for community service: Robbery or petty theft, assault, indecent exposure or any other sexual offense, or arson. All court ordered community service workers must agree to the following policies and obey the following rules: Working hours are from 8:30 AM until 3:30 PM, Monday through Thursday for a full day s credit (6.5 hours). Mid-South Food Bank will only accept workers who can start at 8:30 AM. No other start time is acceptable. You must arrive before 8:30 AM or you will be asked to return another day. If you choose to leave before 3:30 pm or if the Food Bank runs out of work before 3:30 pm, you will be given credit for actual hours worked. The Food Bank does not guarantee that we will always be able to provide work. No service hours are available on Fridays or weekends. If you are pregnant, for safety reasons you will not be able to work at this site. Do not bring anything with you when working at the Food Bank that won t fit in your pockets. Cell phones are to remain in the car & are only to be used outside of the Food Bank building. 1. Enter & exit through the front door only. Court Ordered Workers Must: 2. Sign in when you arrive & sign out when you leave. You may work at a different Food Bank location. You are required to also sign in & out at that location, then return to 239 Dudley to sign out for the day. 3. Meet the volunteer coordinator in the kitchen at 8:30 am for your daily assignment. 4. Return to your designated work area promptly after breaks: Morning break is 10:00 am 10:15 am, lunch is 12:00 pm 12:30 pm, and afternoon break is 2:00 pm 2:15 pm. 5. Make sure your supervisor knows where you are at all times. (Your supervisor is the main staff member working in the department you are assigned to each day.) 6. Wear your name tag at shirt pocket level at all times. Name tags are to be left in the designated box by the sign in/out book each day when you leave. 7. The Food Bank telephone is only to be used in case of an emergency and only with the permission of your supervisor. 8. Treat all others at Mid-South Food Bank with respect.

2 Dress Code For your safety you must wear shoes that provide maximum protection for your feet with a rubber bottom, and good ankle support. Mid-South Food Bank staff and volunteers must provide a positive, presentable image to our donors and volunteers as they visit the Food Bank. Clothes should be comfortable but presentable. Cleavage and waistlines cannot be exposed. Unacceptable clothing and footwear Flip flops Sandals Ballet slippers House shoes Crocs Slip on shoes High heels Tee shirts with inappropriate language or pictures Tank tops Crop tops Sweat pants Pajamas Pants not worn at the waist Scarves, hats (other than baseball caps) Dresses and skirts Leggings Acceptable clothing and footwear Tennis Shoes Boots Steel toed shoes Shorts that come to the knee Tee shirts Shirts with sleeves Jeans Pants belted at the waist line Baseball caps Rules That Can Result in Dismissal 1. Cell phones are only to be used outside of the Food Bank building. If you have a cell phone with you, it must be turned off or on mute and out of sight. 2. Failure to sign in and sign out will result in no credit given for time worked.

3 3. Writing false time on the sign-in is cause for dismissal and no credit given for time worked. 4. Do not loiter (hang out) in front of the Food Bank building. 5. Smoking is only permitted 20 away from the back dock of the building. Do not dispose of cigarettes on the ground. 6. After being assigned to a work area, you are not allowed to leave the premises except during lunch break. 7. You are not allowed to eat or take any food or other items from Mid-South Food Bank. You may bring your own food and eat it in the break room only. 8. You are not allowed to conceal or carry any type of weapon or controlled substance. 9. If you are found to be under the influence of any substance, you will not be able to work at this site. 10. No profanity or inappropriate conversations will be tolerated. 11. No radios, headphones, MP3 players at anytime Please understand that if these rules are broken, you will be asked to leave immediately and will be reported to the proper authorities. Your hours will not count for the day. In some cases you may not be allowed to return to Mid-South Food Bank and will have to complete your community service hours elsewhere. 4 Release and Hold Harmless Statement I understand that my participation as a volunteer for Mid-South Food Bank is purely voluntary, and by signing below, I agree to release and hold harmless Mid-South Food Bank, its representatives, officers and employees from any and all liabilities, claims, obligations, judgments, suits, costs, damages, expenses, attorneys fees, or any other form of legal liability which may be incurred or paid as a result of any property damage or destruction, personal injury (including death) or any other damages of whatsoever nature and kind, arising from, on account of or related to my use of Mid-South Food Bank s facilities.
