1 FOOD BANK/HOME DELIVERED MEALS I. DEFINITION OF SERVICE Funding for Food Bank/Home-delivered Meals that may include: The provision of actual food items Provision of hot meals A voucher program to purchase food May also include the provision of non-food items that are limited to: Personal hygiene products Household cleaning supplies Water filtration/ purification systems in communities where issues with water purity exist Appropriate licensure/ certification for food banks and home delivered meals where required under State or local regulations No funds used for: Permanent water filtration systems for water entering the house Household appliances Pet foods Other non-essential product II. DESCRIPTION OF SERVICE SERVICE PERFORMANCE MEASURE/METHOD MONITORING STANDARD LIMITATIONS Funding for Food Bank/Homedelivered Meals that may include: The provision of actual food items Provision of hot meals A voucher program to purchase food May also include the provision of nonfood items that are limited to: Personal hygiene products Household cleaning supplies Water filtration/purification 1. Services supported are limited to food bank, home-delivered meals and/or food voucher program 2. Indicates actual services provided, client eligibility, number of clients served, and level of services to these clients 1. Types of non-food items provided are allowable 2. If water filtration/purification systems are provided, community 1. Services provided by type of service, number of clients served, and levels of service 2. Assurance that Ryan White funds were used only for allowable purposes and Ryan White was the payer of last resort 1. Assurance that Ryan White funds were used only for allowable purposes and Ryan White was the payer of last resort 1. Compliance with all federal, state, and local laws regarding the provision of food bank, home-delivered meals and food voucher programs, including any required licensure and/or certifications 2. Indicates use of funds only for allowable essential non-food items Ryan White funds are not to be used to purchase permanent water filtration systems for water entering the house, household appliances, pet foods, other non-essential products III. FOOD BANK/HOME DELIVERED MEALS SERVICE COMPONENTS Program Outcome: 75% of clients show improved Activities of Daily Living (ADL) as a result of increased nutritional status, hygiene or cleaning supplies at 6 and 12 months. Indicators: Number of requests completed for food bank/nutrition/ home delivered meals Number of requests at reduced urgency (emergency need, frequency) Service Unit(s): Number and dollar amount of filled requests for Food Bank/ Meal Vouchers/Cleaning supplies 2

3 Standard of Care Outcome Measure Numerator Denominator Data Source Goal/Benchmark I. Structure Food bank/home-delivered meals include the provision of actual food or meals, essential hygiene items, household cleaning supplies and vouchers to purchase meals. Provider has criteria, including allowable extenuating circumstances to determine if a client is eligible for food service. II. Process Service provider assists client in seeking alternate sources to obtain food bank/ home delivered meals/cleaning supplies. III. Outcome Service provider ensures clients accessing food services are enrolled in AOMC for HIV treatment as documented by 2 or more CD4/VL measurements and/or AOMC visit in an HIV care setting in the measurement year Services are provided to stabilize clients and improve nutritional status and ADLs. Clients are screened for need of Food Services. Agency has documented criteria to determine eligibility for food service such as wasting syndrome, little or no financial means, recent or transitional homelessness with no regular means of self-support, and access to only one meal a day (i.e. soup kitchens, shelter) Planning sessions occur with clients regarding alternate source investigation for food bank/home delivered meals/cleaning supplies. 4 IV. DATA REPORTING Part A service providers are responsible for documenting and keeping accurate records of Ryan White Program Data/Client information, units of service, and client health outcomes. Reporting units of service are a component of each agency s approved work plan. Please refer to the most current work plan, including any amendments, for guidance regarding units of service. Summaries of service statistics by priority will be made available to the Planning Council by the Grantee for priority setting, resource allocation and evaluation purposes.
