3 The updated Market Position statement (MPS) outlines key achievements that have been commissioned and delivered by Hertfordshire County Council in partnership with the Clinical Commissioning Groups (CCGs) and other key stakeholders in The key stakeholders include: the voluntary and community sector, community health partners and the private sector. The MPS also examines future market opportunities and commissioning intentions and should be used alongside our strategic documents 123 to provide the necessary information to local, regional and national suppliers to allow them to make informed strategic business decisions

The market that we wish to facilitate Hertfordshire County Council (HCC), East & North Hertfordshire ENHCCG and Herts Valley HVCCG are committed to working together, alongside other key stakeholders, to achieve good outcomes for the services we deliver in a way that is personalised, well led, provides value for money and that we are in a strong position to promote a sustainable and diverse market place. For further details on this please refer to Commissioning for Better Outcomes. 4 We want to encourage and engage with providers to consider ways in which, through working together, we can help prevent common mental health conditions and promote the recovery and resilience of those who do require treatment from services. Market opportunities include services that demonstrate being: Person centred and outcome focused Inclusive Well Led Promotes a diverse and sustainable market Service users to have more involvement in design and service delivery Ensuring Carers voices are heard and that their needs are being met Promotion of peer support and other community based activities Personalisation and Think Local Act Personal 4 principles are at the heart of the service design and delivery Works in an integrated way across the health and social care partnership To work towards a whole system approach where clear and robust data information can be shared in a safe and secure way to support future commissioning of services To ensure that people with mental health issues are accessing services that are appropriate to their level of needs, achieves their outcomes and that all options are being fully maximised with partners Ensuring the service is best value for both the person and funding authority People understand what they are buying e.g. support hours Promoting a diverse market, including recognition of the role of the voluntary and community sectors 4

5 Wellbeing, Prevention and Personalisation We want every service we commission and work with to promote and enhance individuals wellbeing. The Care Act 2014 establishes this as a principle and gives a broad definition of what wellbeing means 5. This includes personal dignity, physical and mental health, control over dayto-day life, and ability to participate in work, education and recreation. All services should therefore work holistically with individuals to support their wellbeing as far as possible. We see prevention as another key principle that sits alongside wellbeing. We want to develop services that promote individual wellbeing by preventing, reducing or delaying the need for health and social care, in many cases by intervening early before problems escalate. Key examples include avoiding hospital admissions, such as through falls prevention or avoiding carer breakdown, often by working in partnership with voluntary and community sector organisations. All services should therefore proactively identify and develop approaches to prevention and early intervention. We see the personalisation of services as key to achieving this. We recognise that individuals often know what is best for their own care and wellbeing. We want to commission services that work proactively with individuals to understand their needs, wants, interests, and ambitions, and that build on individuals strengths and assets in providing care and support for them. Achievements Key service developments within the integrated health and social care services in 2015 were: Increased access to psychological therapies in primary care Introduction of mental health clinical triage within the Hertfordshire Constabulary Force Control to improve the response to people experiencing a mental health crisis Improved psychiatric liaison services to accident and emergency departments at Lister and Watford Hospitals Introduction of Dementia Carers Health and Care Practitioner across the County. Future Commissioning Intentions & Development Opportunities Market Opportunity Some examples of forthcoming tender opportunities will be development of a range of talking therapies that compliment Improving Access to Psychological Therapies IAPT provision and improved community based activities. Commissioners will be seeking a wide range of providers, under the Any Qualified Provider framework, to deliver psychological therapies in primary care. The commissioning aims identified in the MPS 2015 continue to inform current commissioning intentions: Promote healthy eating, support and reduce feelings of social isolation, improve self-worth Promote and enable health and fitness, and reduce smoking and drinking. Help service users to manage their health and mental wellbeing and personal safety. I ncrease the use of personal budgets. Develop outcome focussed, co-produced specifications, where appropriate, for new services. Co produced services have been shown to deliver: An improved sense of belonging to local groups and networks Reduced stigma Increased skills and employability Reduced need for emergency health care Improving physical and mental well-being. 7

8 Market Opportunity Opportunities currently exist to work with providers to test flexible approaches to service provision that meet personalised needs based on patient identified outcomes. Providers will however need to have the ability to support people with complex and multiple health and related needs. 8 New opportunities for county-wide services will be formally procured by the partner organisations and directly by the Community Wellbeing commissioning team for services that address the preventative agenda. Providers, including those from the voluntary and community sector, will have the opportunity to develop, inform and compete in a range of tender opportunities with a focus on improving mental health and emotional wellbeing. 3.1 Personal Health Budgets East and North Hertfordshire Clinical Commissioning Group (ENHCCG) is currently working with Herts Valleys Clinical Commissioning Group (HVCCG) to provide Personal Health Budgets (PHBs) to people with Continuing Health Care, who have since October 2014 had the right to have a Personal Health Budget. The CCGs intend to expand PHB provision to other patient groups including those with Learning Disabilities, Mental Health and Long Term Conditions (LTC). The CCGs approach to expanded provision is to be delivered over a 5 year period. This will be phased so as to allow the CCG to pilot initiatives before rolling out other patient groups. The expansion of provision to other patients will be detailed in the PHB local offer to be published in April PHBs are intended to give people more choice, control and purchasing power over their healthcare and support. New opportunities for county-wide services

9 ENHCCG and HVCCG will in addition be working with the Local Authority to develop integrated processes to cater for individuals with health and social care budgets presenting further opportunities to provide integrated personalised health and social care services across the local economy. Individuals, or one of the statutory agencies working on their behalf, could also be seeking care home placements to address complex care needs e.g. dementia (with challenging behaviour) including respite care. For many people placements will not be a home for life but part of a pathway designed to equip individuals with the skills they need to live independently. Providers contributing to this pathway will be expected to ensure care and support plans are properly implemented and that their services are recovery focussed. There is also an ongoing need for improved access to a wider range of good quality accommodation options for people with well managed mental health conditions who will have personal budgets to fund support to maintain their mental health. ENHCCG & HVCCG integrated personalised health and social care individuals with health & social care budgets Market Opportunity Opportunities to provide bespoke services for individuals to purchase using their personal budgets may also arise, e.g. Personal assistants, short term flexible services for relapse crisis prevention and step-up, step-down purposes. Providers will want to ensure they have a visible presence in the market place and appropriate information is readily available to inform individuals who need to choose a service provider. Providers will also want to engage with existing brokerage services. 9

10 Market Opportunities Providers will want to demonstrate that they can work in an integrated approach across health and adult social care to meet the needs of people with mental illness. 3.2 Develop services to support people with a Mental Health condition The Council has entered into a commissioning partnership arrangement with the two Hertfordshire Clinical Commissioning Groups (CCG s) East & North Herts and Herts Valley which established the integrated commissioning of health and social care services for mental health. The Council has also entered into a separate partnership arrangement with Hertfordshire Partnership Foundation Trust which established the integrated delivery of health and social care services for mental health. The aims of the Partners in establishing these integrated commissioning and delivery arrangements are to: Operate a single process to assess the needs of service users, to manage and deliver their care and to eliminate overlaps in provision that currently exist between health and social care; Provide services in a more coordinated way by allowing different professions to work within a single management structure and by arranging assessment and care management from a single statutory provider; Support service users and carers to access services that are appropriate to their needs, subject to eligibility, regardless of whether the funding originates from an NHS or Local Authority source; 10

11 4. Mental Health and wellbeing in Hertfordshire Across England, one in four adults will experience a mental health problem in any given year. Within Hertfordshire there are an estimated 172,558 adults aged experiencing some form of mental ill health. Between there has been an increase in diagnosis of adult mental health illness in Hertfordshire of between 4%-15% - see graph 1. (Source: Public Health England) The steady increase in diagnosis is expected to continue to grow and projections show that over the next 10 years the increase in mental health prevalence will rise at an estimated 6%. The demand growth will be driven by a number of factors: early diagnosis of young people and transition to adult services; increased awareness of mental health conditions and community lead reduction in stigma attached to mental health (building on the success of campaigns like Time To Change) and Hertfordshire s Year of Mental Health July July 16. Table 2 Prevalence of mental health diagnoses - Hertfordshire % / / / / / /15 England Hertfordshire There has been a significant increase in Dementia diagnoses from with an increase of 15%. Current estimates suggest 15,000 people over the age of 65 are living with dementia in Hertfordshire. Increase % Dementia 7% 12

14 4.2 Mental Health Conditions Mental health conditions that require specialist treatment from a mental health provider are less common equating to approximately 2% of the adult population. In Hertfordshire these statutory services are delivered by Hertfordshire Partnership Foundation Trust (HPFT). Conditions can include persistent depression, bi-polar disorder, schizophrenia and some forms of personality disorder. Within this group of people, approximately 6% will have more than one mental health disorder. Our Spend Table 6 Voluntary, Community & Independent Sector Services for Mental Health 2014/15 Expenditure 5.1 Prevention Support and improving emotional wellbeing Services that are commissioned from the voluntary, community and independent sector focus on: prevention, support and improving mental wellbeing. 16 Annual spend m Support people to find and maintain accommodation 5.2 Mental health accommodation We spend over 1.3m annually to provide support to people to find accommodation and to help them cope with maintaining their accommodation. 5.3 Housing Support We spend nearly 1m per year on accommodation for people with mental health conditions in a variety of residential settings, including supported living. 5.4 Financial Challenge Each of the partner organisations faces significant financial challenges and will seek to work with providers to manage capacity in line with their financial resources. Each partner organisation will seek the engagement of providers to support the design and delivery of high quality, cost effective service pathways. Market Opportunities Providers will wish to consider a variety of ways to monitor and improve the quality of the services they deliver, such as: Involving people who use the services, their carers and families and use their feedback to improve the quality of the service. Identifying good practice and benchmark the service with others Improving outcomes for individuals Recognising that the people using services and their carers are experts in their own lives and are therefore essential partners in the design and development of services. Investing in appropriately trained, qualified and competent staff that are well supervised and managed.

17 5.5 Quality and Monitoring 5.6 Residential monitoring Service safety and quality is extremely important to HCC and its partners. HCC works closely with the CCGs, the Herts Care Providers Association (HCPA), Hertfordshire Partnership Foundation Trust (HPFT) and the Care Quality Commission (CQC) and other partners to deliver an ambitious programme of continuous improvement in services for people with mental health. Our ambition is at least 80% of care homes in Hertfordshire achieve East of England contract standard of Good or above. 80% Our Ambition Care Homes Rated Good or Excellent Currently 69% of care homes in Hertfordshire are rated Good or Excellent. This means that Hertfordshire is ranked 37 in the Country for the number of Good care homes, despite its providers being the most expensive in the Country. We have adopted the Association of Directors of Adult Social Services (ADDASS) East of England Regional Standard contract for adult social care and housing support services in the East of England, which provides a consistent framework for managing quality. To support effective commissioning partners lead on an ongoing programme of service quality monitoring with our partners from the independent, statutory (both health and social care) and the regulatory sectors. Care Homes in Hertfordshire 69% Rated Good or Excellent 17

18 18 January accessing mental health provider HPFT 5.7 Mental Health Placements As of January 2016, approximately 428 people were accessing mental health accommodation placements through our main mental health provider Hertfordshire Partnership NHS Foundation Trust (HPFT). HPFT commission these placements and also monitor the quality of the placements following the Hertfordshire Multi Agency Protocol for Managing Care Quality in Adult Health and Social Care Commissioned Provision which incorporates the East of England monitoring tools. Monitoring of nursing home, residential and supported living placements is done jointly with HCC. Table 7 Mental Health Placements January 2016 Long Stay Placements 252 Total Placements 428 Supported Living Placements 176 Over the past year HPFT have carried out a programme of reviews of people accessing mental health placements and found: 30 people who require supported living 50 people currently in supported living who are ready for general needs housing 20 older adults who would benefit from sheltered accommodation.

19 All of these individuals have a mental health diagnosis, but their condition is very well managed and they no longer need the support from their current placements. If move on accommodation could be identified each individual s personal budget allocation would be established to provide for any support required to maintain their mental health. In addition HPFT directly provide inpatient services for people with dementia who meet NHS Continuing Care eligibility criteria. HPFT will be looking to the market to provide approximately 80 of these beds. Market Opportunities To provide approximately 80 beds 5.8 Mental Health Recovery Support Services We have a number of contracts in place with a range of providers across the county to provide floating support to help people maintain their housing and to manage daily living. The majority of the support focusses on assisting people with financial management, debt management, rent arrears, housing benefit and other related items. All districts and boroughs across the county will have access to Mental Health Recovery (floating) support provision. We also commission services from the voluntary, community and independent sectors which focus on prevention, support and improving mental wellbeing. Statutory social care support for adults with mental health conditions is determined by eligibility criteria. Hertfordshire County Council has delegated this responsibility to HPFT via a Section 75 agreement. 19

20 Hearing the voice of people with Mental Health issues In recognition of the importance of hearing the voice of people who use services and those who may use services, as part of our future commissioning intentions we will be consolidating current contracts in order to tender for a county wide Service User Voice network during We expect this network to include organisations supporting people with Mental Health issues. 21 6. Accommodation requirements for people with mental health conditions We want to work with our providers and stimulate the market to encourage more properties suitable for people with a mental health condition, focusing on the recovery model; this includes accommodation settings for rehabilitation, residential settings. A major aim of recovery will be to enable people to move on from these settings to supported living or independent housing. The particular demand across the county for accommodation is for one bedroom flats/studios or selfcontained provision. Our key aims are to focus on, improving choice, improving quality and ensuring good supply. In order to achieve this we firstly need to have an understanding of: What accommodation provision currently exists Its condition and suitability What needs to be done to improve it Where there are gaps in accommodation provision What should be done to fill the gaps Hertfordshire County Council is well placed in understanding the property conditions. In 2015 the council carried out property reviews of all HCC owned accommodation. The review identified that 21 properties were registered as supported living accommodation for people with mental health conditions. Part of the property review is to ascertain how we could make best use of the council s assets. We estimate there are 50 service users residing in the 21 properties, we want to work with our partners to understand better how we can achieve supporting service user s aspirations move to independent housing. Market Opportunity We are keen to work with our partners to consider alternative models of housing support and care which would maximise making best use of the council s assets and enhance individuals opportunities on where they can chose to live. 22 6.1 Accommodation Boards In 2015 HCC established dual district Accommodation Boards with our 10 district partners and stock-transfer housing providers. One of the key drivers for the Boards is to ensure that the accommodation related priorities of key partners and people with lived experience of care and support needs are promoted in local planning. Through the establishment of the boards we aim to develop a county wide Accommodation Strategy to be launched in autumn 2016 that addresses the following: The promotion of independent living and prevention or delay of escalation of need; Equity in service access and delivery of the right quality standards and value for money; Enabling timely access to the right accommodation. Enabling individual choice and control 22

23 6.2 Future commissioning intentions for mental health accommodation 1. Reduce the use of traditional residential care and group home settings and increase alternative housing and support options. 2. We want to explore the potential to decommission some of the councils in-house mental health provisions by working in partnership with agencies to future proof our housing stock 3. The opportunity to work with our District Councils and review their housing profile for mental health provisions, this will form part of the local commitment under the Mental Health Crisis Care Concordat 4. Work with the private rented sector to secure affordable and sustainable tenancies for people with a mental health condition. 5. The opportunity to work with providers and consider new developments for mental health services this includes step down provision 6. Having a better planned approach in discharging of patients and housing them appropriately including returning them back home with appropriate support 7. To work with providers and have a seamless void process to enable efficient and timely allocation, which matches the needs of clients 8. To inform future investment and service provision 23

24 7. Future of Preventative Services Target Preventative and recovery based 7% services Promoting = 6% of the population Emotional Wellbeing Commissioners are keen to target preventative and recovery based services focused on promoting emotional wellbeing; we are looking for opportunities to continue developing this area and support innovative ideas. We have just recommissioned a county wide Complex Needs service from the voluntary sector that can support the whole person in their recovery from crisis, providing support and expertise on a wide range of issues including accommodation issues and substance misuse, but we continue to develop our thinking around responding to or preventing crisis in the first place. Market Opportunity One area of consideration is the development of a Wellbeing College and this is linked in with wider thinking about the five ways to wellbeing 7 for all adults. We are currently taking a co-production approach by talking to people about what courses would assist with their wellbeing in terms of mental and physical health. County Wide Complex Needs Service Voluntary sector provides support and expertise on a wide range of issues

25 8. Future of personal budgets and self-directed support The introduction of personal budgets and self-directed social care support is underway within HPFT. Also, Personal Health Budgets have been introduced in each of the Hertfordshire CCG s for people receiving NHS continuing care and pilot projects to roll out personal health budgets to other client groups are underway. Future of Brokerage services We want people to have greater influence, choice and control over how their social care needs are met. To help achieve this, anyone with a personal budget and eligible for financial support will be offered the option of having a broker work with them to identify community resources that will meet their social care needs. Using a person-centered approach brokers will design and implement a support plan with the individual. Some people will choose to have an independent broker while others will want to arrange their services with their care manager. Support for people experiencing mental health crisis Hertfordshire s Crisis Care Concordat declaration has been signed by all statutory agencies in the County, together with a range of voluntary organisations working in mental health and/ or substance misuse. The Crisis Care Concordat signatories have agreed an Action Plan which is available on the Crisis Concordat website ( and identified the following priority areas: Ensuring the effective use of S136 suites (places of safety) and implementing strategies to reduce inappropriate use e.g. introduction of street triage Developing methodologies to improve knowledge and information sharing between Concordat partners built on the engagement of people with lived experience Housing supporting people with mental health problems to access and maintain appropriate accommodation. Self-Funders Market The self-funder market is a growing market and improvements we are making to our commissioned services in terms of quality and choice should be consistent for all people in Hertfordshire who need care and support services, regardless of whether they are funded by HCC. Due to responsibilities under the Care Act the council are required to have a better understanding of this area of the market. We are working closely with Herts Care Providers Association and their wider membership to help us gain a better understanding and knowledge of, the self-funder market in Hertfordshire. We aim to map and calculate the size and value of the self-funder market in 2016/17 and will continue to extend our partnerships with providers to include those not directly commissioned by HCC, to identify those who may require local authority funding in the coming year as well as to be prepared for the cap on care costs due to be implemented in Please visit hertsmpe for more information on all of the Hertfordshire Market Position Statements as wel as further reading on HCS legislation, strategies and drivers. 27
