2 Stream 1: For people who experience difficultly navigating stressful life circumstances or with symptoms of Depression and / or Anxiety: eg through bereavement, trauma, relationship difficulties, questioning identity; and with or without concurrent problematic substance use. A large group of people in the community with the most common problems present different challenges for service delivery. This group requires highly accessible interventions that focus particularly on anxiety, depression and/or problematic alcohol, nicotine and cannabis use. Primary care services (Community Health/General Practice etc) are assisted by both Primary Mental health and Early Intervention teams from the specialist Mental health sector and Drug and alcohol services to provide professional development as well as secondary and tertiary consultation. Stream 2: For people who experience severe substance use problems with or without mental health problems of lesser severity and complexity. The main service response will be provided by staff with high level drug & alcohol skills and training who are also dual diagnosis capable. People treated by this sector experience a significant incidence of relapse (both mental health problems and drug & alcohol use) and services incorporate some long term psychosocial interventions in order toassest opele overcome addictions. People in this group may have significant mental health problems associated with the drug and alcohol use. These can be managed effectively with consultation from, and in some instances transfer to, specialist meant health services. Short term shared care arrangements may be put in place to respond to crises. Poly substance use increases the complexity of presentations. Part of the function of clinicians in drug and alcohol services is to provide support through co-consultation, secondary consultation and training and education to the Tier 1 services. Stream 3: For people who experience severe mental health problems with or without problematic substance use. People with serous a complex problems, which include a severe mental illness with or without problematic substance use, require an integrated response that takes account of all aspects of their illness. Services should bring together the best in well research models of primary care, psychiatry, rehabilitation and addiction medicine. A blend of integrated / collaborative treatment will frequently be necessary to ensure the most effective possible response to people with multiple complex needs

3 Collaborative Partnerships: Practitioners and service who require shared care, secondary consultation, traing and support. Also includes maintenance and review of protocol partnership and processes. Secondary consultation pertains to the provision of clinical advice and support to health professionals at their request. In secondary consultation, the client is NOT seen. Secondary consultation routinely occurs on both intra and inter agency bases. Training refers to education activities: Conducted between services (one service prodinvding education to another service) Recruited together (events for all service staff) Delivered together (dual agency delivery to external parties) Centrally promoted and planned Support refers to Provision of debrief opportunities where required Opportunistic joint product development etc

7 Service Name: Bridging the Gap Community Services Inc Male support and resource centre- object is to reduce destructive activity which impacts on the welfare of families and the community Aim of Service: generally by addressing domestic violence, drug and alcohol abuse, gambling and dislocation problems that occur in region Tel: Fax (02) Intake Phone/ Organisation 3/160 Pacific Highway, Male members of families Nil To reduce the incidents of domestic violence and assist and support males to address their personal issues and resolve those issues. Counselling is provided in areas of anger management, drug and alcohol abuse, financial management and special counselling and help following relationship breakdowns. 11 The Drug and Alcohol Specialist Advisory Service Service Name: (DASAS) DASAS is a 24 hour telephone service to support doctors and health professionals exclusively with any assistance they may Aim of Service: need with diagnosis and treatment of patients with alcohol and drug issues. Organisation Tel: (02) (Sydney) Fax (02) 24 hours a day 7 days a week Intake Phone/ (For NSW regional and rural callers) Doctors, nurses, and other health professionals DASAS is a free telephone service for health professionals, funded by NSW Health and managed by St Vincent s Hospital Alcohol and Drug Service in Sydney. DASAS advises on the clinical diagnosis and management of patients with alcohol and other drug-related problems, and is available 24 hours a day, 7 days a week. Advice can include the safe management of intoxication and withdrawal of patients for many types of drugs, both legal and illegal; advice on the management of other clinical issues that may arise from drug and alcohol use, such as during pregnancy; advice on the interactions between medications and illicit drugs; referral and treatment options around NSW; and advice on methadone and other opioid treatments. Aim of Service: Organisation The Ted Noffs Foundation s programs are not band-aid solutions.
