Please specify if you would be interested in purchasing a puppy, teenager, or adult (occasionally we have adults or teenagers available). Do you plan on breeding the Dachshund that you purchase from us? Are there any children in the home if so what are their ages? I understand signing this contract does not guarantee purchase of a Splendor Farms puppy or to be put on a waiting list. If you are approved we will keep in contact with you about which puppies we have available (if any), when upcoming litters are expected (they will also be posted on our website, when litters are born, and where you stand on the waiting list.

3 Thank you for your interest in a Splendor Farms puppy. We will contact you as soon as possible. AGREEMENTS Are you aware most of our puppies are sold as pets and must be spayed or neutered by 7 months of age? Proof of the spay or neuter procedure must be provided to us by the age of 8 months. (This allows 1 month for you to send the paperwork to us.) Are you willing to send in this proof for our records? Do you agree to comply with our puppy contract? Upon approval of your application you will be required to place a $100.00, deposit to be considered as an adoptee for one of our puppies. Deposits are non-refundable and will be applied to the total purchase price of your puppy. Are you aware that the deposit is non-refundable? Deposits are not refunded unless your application for adoption is denied. Do you agree to this? Any monies paid towards a puppy are non-refundable. If you change your mind about a puppy, no money will be refunded. You can put the paid money towards another puppy. Do you agree to this?

4 I understand that the balance for the puppy is due no later than 8 weeks of age. If balance is not received, my puppy may be offered to another family for adoption. Splendor Farms agrees to keep the puppy up to 11 weeks of age. If it becomes necessary for the puppy to stay beyond this age for your convenience, there will be a $10 per day boarding fee. If the puppy next set of vaccinations/worming is due prior to pickup these will be administered by Splendor Farms at your cost, i.e. vaccinations/$25.00, worming/$ Do you agree to this fee if it becomes necessary? CONDITIONS OF SALE Seller guarantees that at the time of delivery, the dog was check by a licensed Vet and is in good health. Should dog be found to be in poor health of a serious nature that is diagnosed as permanently life altering by a veterinarian, Seller will provide Buyer with a replacement dog from another litter after dog is returned to seller at Buyer's expense, or a full refund of the purchase price of dog only after dog is returned to Seller at Buyer's expense. Shipping costs or additional vaccination/worming costs will not be refunded. The following conditions apply to the sale of the aforementioned dog: The Buyer has THREE (3) days from the time of receipt of dog to have dog examined by a veterinarian. Any request to return the dog to the Seller for a full refund must be made within this time period and may only be requested should the dog be found to have a health issue of such serious nature that the quality of life of the dog will be shortened or seriously altered.

5 Minor health ailments including, but not limited to, fleas, worms, or stress induced diarrhea aka coccidia, due to the puppy's transition from the Sellers home are not reason for dog to be returned to Seller. Proof of examination and diagnosis must be provided to Seller and Sellers vet, should Buyer wish to request returning dog for a refund. We do strive to produce a certain look but we do not guarantee size, color, or appearance. This warranty will be terminated immediately if dog is not examined within the allotted time. A warranty against all serious life altering genetic disease is provided for one year after the date of sale. Should a genetic disease be discovered during this time that will greatly shorten the life of, or greatly alter the quality of life of dog, Seller will reimburse Buyer the veterinary costs up to the purchase price of the dog, if dog is kept by Buyer, OR provide a replacement puppy to Buyer from a similar litter type from first litter with availability. Two independent veterinary examinations and reviews must be provided to Seller documenting the genetic health issue. Reimbursement of veterinary fees is provided to Buyer once copies of the medical reports are received by Seller. Reports must state the treatments provided, and fees for the treatments. Warranties shall apply only to original purchaser. Transfer of dog ownership shall void all such provisions of this agreement. Buyer agrees to first contact Seller should it ever become necessary to rehome dog. Seller will assist in re homing the dog, Buyer agrees to never relinquish the dog to an animal shelter, rescue organization or puppy mill. Under no circumstances shall the Seller be liable to the Buyer or to any third party for any consequential incidental or special damages resulting from or in manner related to the dog. Upon execution of this contract all veterinary care and its

6 associated costs are the sole and exclusive responsibility of the buyer. MAINTENANCE Buyer agrees to maintain this dog in good health, provide routine preventative health care including, but not limited to, ear, nail, & teeth care, inoculations and internal and external parasite treatments for fleas, tick & heartworms. Veterinary records must be provided to breeder if requesting refund or replacement. If any of the above preventative care has not been provided, health warranty will be void. Buyer also agrees to provide, at minimum, basic obedience training for dog by a certified trainer or by Buyer, with the use of books, videos, of verbal advice by a certified trainer. TEMPERAMENT Buyer understands that all puppies exhibit certain undesirable behaviors (nipping, chewing, barking, marking, etc). Buyer is responsible for appropriate training of puppy to correctly teach acceptable behaviors, and is responsible for proper socialization of puppy. Should Buyer not be successful at training these behaviors on their own, Buyer agrees to seek professional help from a certified trainer should it become necessary. Seller will not be responsible for puppy's temperament once the puppy has left the kennel. We require that you have your puppy spayed or neutered within the first 7 months of their birth date. Failure to do so or to provide documentation to this affect by the age of 8 months will void the health guarantee and could possibly result in legal proceedings. FULL AND FINAL AGREEMENT The parties agree that this writing represents the entire Agreement between them and that no other representations have been made

7 regarding the dog described above. I agree to the items listed in the conditions of sale. I have answered all of the above questions truthfully. I understand that if any intentionally false answers have been given, Splendor Farms reserves the right not to sell to me. By signing below, I agree to the re-home and spay/neuter policy as explained above. I also agree to the Health requirements and guarantee as specified in the conditions of sale.
