Terms of Service ViewQwest Fibre Broadband + Entertainment Bundle Promotion a) ViewQwest Fibre Broadband + Entertainment Bundle Promotion (SITEX 2016) i) Should subscribers terminate their subscription, regardless of any reason from the day of sign-up to contract expiry date they shall be liable for early termination charges, total monthly subscriptions for the remaining contract, remaining free months and termination penalties as followed: - ASUS RT-AC1200G+ Router: $199 (w/gst) - ASUS RT-AC88U Router: $429 (w/gst) - ASUS RT-AC5300 Router: $469 (w/gst) - ViewQwest 4K Media Player: $188 (w/gst) ii) This promotion is not available to existing ViewQwest subscribers who are less than 21 months into their contract. iii) ViewQwest Entertainment Pack is not available for purchase as a standalone package for existing ViewQwest subscribers. iv) Free 9 months is applicable for 1Gbps and 2Gbps Single Network Fibre Broadband with every sign-up of ViewQwest Entertainment Pack, with the monthly subscription payable as followed: - 1Gbps Fibre Broadband + Entertainment Bundle $39.90/mth for first 9 months, thereafter $89.80/mth for remaining 15 months - 2Gbps Single Network Fibre Broadband + Entertainment Bundle $39.90/mth for first 9 months, thereafter $109.80/mth for remaining 15 months v) First 9 months off subscription for 1Gbps and 2Gbps Single Network Fibre Broadband will be reflected in the invoices for the 1st to 9th months of the contract. vi) Service for ViewQwest Fibre Broadband + Entertainment Bundle Promotion is provided to subscriber for 24 months, and thereafter the subscription will continue automatically upon subscription expiry as followed: - 1Gbps Fibre Broadband + Entertainment Bundle $89.80/mth - 2Gbps Single Network Fibre Broadband + Entertainment Bundle $109.80/mth vii) ViewQwest is not liable for the subscription amount upon subscription expiry should subscriber choose not to re-contract after 21 months into the contract. viii) Freebie is given free with every sign-up of ViewQwest 1Gbps and 2Gbps Single Network Fibre Broadband + Entertainment Bundle Promotion (24 months contract). Chosen item is non-exchangeable. Customer is limited to either free ASUS RT-AC1200G+ Router, free ViewQwest 4K Media Player or other top-up options stated in the Service Application and Agreement Form for 1Gbps and 2Gbps Single Network Fibre Broadband + Entertainment Bundle Promotion as part of the bundle promotion. Price difference between all the options is non-exchangeable for billing credits or any other products and services. ix) Selected items will be delivered on day of installation for new 2Gbps Single Network Fibre Broadband + Entertainment Bundle Promotion sign-up. For 1Gbps and 2Gbps Single Network Fibre Broadband + Entertainment Bundle Promotion recontract customers, the items will be delivered within 1 month of signup subjected to availability. In any case of stock shortage, another appointment will be scheduled to deliver the items. ViewQwest reserves the right to replace the router model with another model of same value. x) Avertek (authorised dealer of Asus Router in Singapore) is the partner of ViewQwest of this promotion and is responsible for all device warranty. xi) Service activation fee of $53.50 will be billed to customer within 1 week after sign-up. xii) ViewQwest reserves the right to deduct the premium charges as shown above from the Credit/Debit card submitted should subscriber terminate their contract regardless of any reason. xiii) ViewQwest reserves the right to amend the Terms & Conditions and reject any application without prior notice. xiv) Requests stated in the Comments section are subjected to management approval, and shall be rejected without prior notice. xv) Other Terms & Conditions apply. b) Installation of equipment (1Gbps Fibre Broadband + Entertainment Bundle Promotion) i) After NetLink Trust activation date, relevant equipment will be delivered to customer for self-installation. Onsite installation is chargeable at $80 per trip for both weekdays and Saturday (i.e. ViewQwest engineer will be sent to premise to assist with installation). ii) Should there be any high-loss issue (indicated by a red alarm light), customers are to call in to inform ViewQwest. ViewQwest will then arrange for an onsite visit to check on the issue. There will not be any onsite installation charge should it be genuine high loss issue. iii) If equipment is faulty upon delivery, ViewQwest will arrange to replace the equipment. Arrangement to replace the equipment will not be chargeable. iv) It will be at the discretion of ViewQwest to charge an onsite fee if the onsite is scheduled by the subscriber for rectification of issues that are not due to ViewQwest s fault. v) Customers will have to receive the equipment within 5 working days after we have sent out the items. If the equipment is not received after 5 working days, customers will have to self-collect the equipment from ViewQwest Bukit Timah Showroom. vi) NinjaVan is the official courier service of ViewQwest for the delivery of the items. Should you have any enquiries regarding the delivery of your item, kindly contact NinjaVan at support_sg@ninjavan.co or call vii) Tracking ID will be available for your information after your item is received by NinjaVan. viii) ViewQwest only provides support for routers indicated in the Service Application and Agreement Form. No assistance will be provided for any other unsupported routers. ix) Billing will start 1 day after successful delivery of the relevant equipment. c) ViewQwest Entertainment Pack i) Subscriber will enjoy the following services with every sign-up of ViewQwest Entertainment Pack: - 24 Months Eleven Sports Network Subscription - One free unit of ViewQwest 4K Media Player - 24 Months Freedom DNS - 24 Months OneVoice Subscription ii) All the services above are non-exchangeable for cash or billing credits, is non-refundable and non-transferrable. iii) Unless otherwise specified by ViewQwest, ViewQwest Entertainment Pack service will be provided to subscriber for 24 months. Upon contract expiry, free services included in ViewQwest Entertainment Pack, with the exception of ViewQwest 4K Media Player, will continue on a monthly basis at $39.90/mth, in addition to the monthly subscription rate for the subscribed Fibre Broadband Plan. iv) Upon contract expiry, subscriber can choose to cancel ViewQwest Entertainment Pack, retaining Fibre Broadband at the monthly subscription stated herein upon endorsement. v) Eleven Sports Network Subscription - Eleven Sports Network will commence from the date that your service is activated, which will be informed via - Details on how to access Eleven Network Subscription will be indicated in the Ready For Service . - There will no refund or cancellation for any delayed match streaming. - You are entitled for 3 LIVE and 3 delayed matches per match week. - For technical support, you can contact Eleven Sports via at info@elevensportsnetwork.sg, live support chat on and hotline number (Operating Hours: Weekdays 3pm to 1am and Weekends 9am to 1am). - Please visit for more details on programme listing. - Terms and Conditions by Eleven Sports Network apply. vi) ViewQwest reserves the right to amend the Terms & Conditions and reject any application without prior notice. vii) Terms and Conditions for the various services apply. d) Aftershock Discount i) Customers must produce a copy of the completed ViewQwest Service Application and Agreement Form and Official Receipt when purchasing Aftershock notebook/desktop to enjoy the $100 off and 3 years extended warranty (1 for 1). ii) The $100 discount for all Aftershock notebook/desktop can be redeemed at AfterShock booth during SITEX 2016 from 24 Nov to 27 Nov iii) If the $100 discount is not redeemed during SITEX 2016, customers will have to visit AfterShock Headquarters to redeem it. The $100 discount is only valid for 2 months from date of Official Receipt and is non-transferable. e) Refer-A-Friend Program i) Referral program applies when an existing customer refer a new customer to sign up for any 2-year ViewQwest fibre broadband plan. ii) Referral section must be filled up with valid name and NRIC of existing customer. iii) $40 ViewQwest E-store voucher for new and existing customer will be issued via upon service activated by the new customer. iv) Subscription for any 2-year ViewQwest fibre broadband plans are subjected to other Terms and Conditions as stated in the application form. v) Terms & Conditions for ViewQwest E-store voucher apply. vi) ViewQwest E-store Terms & Conditions apply. vii) ViewQwest reserves the right to amend the Terms & Conditions. f) ViewQwest 4D3N UK Trip Lucky Draw i) ViewQwest 4D3N UK Trip Lucky Draw is opened till 30 Nov 2016 to customers who had signed up for ViewQwest 1Gbps and 2Gbps Single Network Fibre Broadband + Entertainment Bundle Promotion. ii) Prize for ViewQwest 4D3N UK Trip Lucky Draw will only be awarded to customer who had (a) submitted fully completed and endorsed Service Application and Agreement Form, and (b) whose service is successfully activated. iii) ViewQwest 4D3N UK Trip shall only include the cost of: - A pair of economy return airfares from Singapore to a city in United Kingdom; - 4 Days and 3 Nights accommodation in the said city; - Standard admission tickets for two adults to catch a Premier League match in Manchester, 2017 (exact match schedule subjected to change) iv) Any other costs and expenses not expressly stated shall not be included in the Prize nor shall be reimbursable or claimable by ViewQwest. v) ViewQwest is not liable for any delay in NetLink Trust activation for Fibre Termination Point. vi) By participating in ViewQwest 4D3N UK Trip Lucky Draw, participants agree to abide by the Terms and Conditions of this Lucky Draw. vii) ViewQwest has the absolute discretion to determine the eligibility of a participant participating in the Lucky Draw or receiving the Prize. viii) For more Terms and Conditions on ViewQwest 4D3N UK Trip Lucky Draw, kindly visit

Terms of Service Installation of connectivity a) All amounts quoted in this contract are in Singapore dollars, and include prevailing GST or other government charges. b) The following original documents must be submitted to ViewQwest at i) Service Application Form, duly completed and signed ii) Photocopy / Scanned copy of your photo identification document (front and back) as indicated in this form (the subscriber must be at least 18 years old for Singaporeans and Permanent Residents) iii) If the subscriber is a foreigner, the subscriber must be an Employment Pass Holder, Work Permit Holder, Student Pass Holder, Long-Term Visit Pass or Dependent Pass Holder (with a minimum validity period of 12 months) and at least 18 years of age. c) For all Fibre Broadband plans, you will be charged any miscellaneous fees, including without limitation the following, where applicable and may be incurred by you, in respect of each Service Address. In case of loss, defects, damage, misuse, acts of God, accident or unauthorised alteration/repair, replacement of the ONU will be chargeable. e) There are no Sunday installations available. f) ViewQwest reserves the right to replace any hardware, with an equivalent in terms of performance, without prior notice to the subscriber. g) ViewQwest service installation date must be booked within 14 days of the NetLink Trust activation date. h) ViewQwest pays Netlink Trust upfront for the FTP installation and the subscriber commits to reimburse ViewQwest through the Fibre Termination Point installation Monthly installment plan, if selected instead of the one time payment. In the event that the subscriber terminates their subscription before the end of this contract, the remaining charges shall apply. i) Approval of applications, accompanied with all correct documentation, will be notified via . j) ViewQwest will not be responsible for routers purchased from third parties nor internal wiring related to the provision of ViewQwest services. k) In the event that a subscriber opts to use an existing router that is supported (please refer to a list of supported routers: ViewQwest undertakes to install it free of charge. If the router is not a supported model, ViewQwest undertakes to install it on a best-effort basis, and will charge a fee upon successful installation. A successful install is defined as the availability of Internet access via the router s Ethernet port(s) as well as internet access via WiFi (if available on the router). l) Billing will be monthly in advance. The subscriber hereby agrees to pay ViewQwest for all charges incurred on their monthly invoice. All charges are calculated and billed as per listed here (valid at the time signing this form). m) Subscribers who wish to terminate the Service after the 24th month shall inform ViewQwest s Customer Service 7 days in advance of termination. The termination will only be effective upon return of all hardware/equipment provided (ONU, Patch Cable and power adapter) in good order and working condition to ViewQwest s showroom at 200 Bukit Timah Road, Singapore , failing which ViewQwest may in its option deem the Service (and any plans thereof ) to be continuing at the full stipulated charges. n) Subscriber shall return all leased hardware/equipment (ONU, Patch Cable and power adapter) in good working order and condition to ViewQwest, failing which, the equipment will be considered lost and the subscriber shall be liable to pay the applicable charges (as stated in clause 2c). o) ViewQwest may cancel or suspend the subscriber s use of the service, without notice, should the subscriber become negligent for improper use of the service or for failing to pay their outstanding invoice charges. A reactivation charge must be paid in advance should the services be required again. p) ViewQwest reserves the right to modify or amend the terms of the Service Application Form and any other agreements made with the subscriber. q) Unless this contract is replaced by a new service contract of an equal or greater value for contract duration less than 24 months, termination of this contract for any reason before the expiry of the agreed contract period will result in the subscriber being liable to pay the remainder of the contract service fee until the contract expiry date. 7 days written notice must be submitted for any termination. r) ViewQwest s General Terms & Conditions can be downloaded from our website, s) All Value-added services (except OneVoice TM Residential) can be subscribed to / terminated, at any point, regardless of the period of this contract. t) From time to time a free gift may be out of stock on the day of installation of the service. In such cases, ViewQwest undertakes to obtain the item and provide it to the subscriber within a reasonable time. Service provision and billing will be unaffected by this delay. 3. Terms of service OneVoice TM Residential a) Subscribers must commit to OneVoice TM for a minimum of 12 months. Termination of OneVoice TM for any reason before 12 months is completed will result in the subscriber being liable to pay the remainder of the OneVoice TM contract service fee. 30 days written notice must be submitted for any termination. b) All incoming and outgoing local calls are free. c) IDD services are activated on OneVoice TM plans by request. d) In the event that a subscriber decides to take up the OneVoice TM service despite having less than 12 months remaining in their ViewQwest contract, the subscriber is still liable to pay the remainder of the OneVoice TM contract service fee upon termination of the subscription. 4. Terms of service ViewQwest 4K Media Player a) Warranty period is 1 year from the date of commencement of this contract. b) Any hardware fault within this 1 year warranty period entitles the subscriber to a 1-to-1 exchange of the Viewqwest 4K Media Player. To utilize the 1-to-1 exchange, the ViewQwest 4K Media Player, with full set of accessories and box, must be brought to ViewQwest s showroom at 200 Bukit Timah Road, Singapore c) A chargeable dispatch service is also available wherein ViewQwest undertakes to perform the 1-to-1 exchange at the subscriber s premises (as stated in clause 2c). d) The ViewQwest 4K Media Player device allows access to media channels only. The availability, quality and content of the media channels is the sole responsibility of the channel owners. 5. Terms of Service Freedom DNS a) Freedom DNS grants access to certain geo-blocked websites around the world. ViewQwest neither owns nor is responsible for content on said websites. Content may be added/removed at any point at the discretion of the respective website/app owners. b) ViewQwest shall not be responsible for any fees incurred or subscriptions to any website/app accessed through Freedom DNS. c) The listed websites/apps accessible through Freedom DNS are subject to change. ViewQwest reserves the right to add/remove support for any website/app at any time without prior notification. 6.
