3 Code Blue: Cardiopulmonary Arrest Code Blue Adult or Pediatric + Facility/Building + Floor/Dept Code Blue Team responds
4 Code Red: Fire RACE Rescue: Get everyone out of the danger area Alarm: Pull alarm, report the exact location to the Operator Contain: Close all door and windows Extinguish: Only if can be done safely/or Evacuate Pull: Pull the pin PASS Aim: Aim the nozzle at the base of the flame Squeeze: Squeeze the trigger slowly Sweep: Sweep from side to side at the base of the flame Locate these items in your work area: Fire exits Fire alarms Fire extinguishers
5 Code Yellow: Internal, Internal Emergency Key Words: Water, Electrical, Telephone, IT Systems, etc. If situation does impact your area: Activate back-up plans, downtime procedures Reduce services Evacuate (if in immediate danger) If situation does not impact your area: Remain alert for additional information
6 Code Yellow: External, External Disaster Large influx of patients into the Emergency Department Know your Department s responsibilities and actions when this Code is announced 1. Report to your Department/Unit 2. Be assigned to the ED or other areas 3. Be assigned to the Labor Pool
7 Code Orange Internal: Hazardous Materials Spill or Release Stay away from the area unless you are the responding team Evacuate the area if necessary Stop traffic from entering the area Close doors, guard entrances
8 Code Orange External/Code Orange-Decon: Decon Team Activates Code Orange External May consider Sheltering-in-Place If shelter-in-place is implemented, entrances to the facility will be restricted to one access point for decontamination Code Orange-Decon ULH ED Decontamination Team Activates
9 Tornado Warning Use only when there is an actual TORNADO WARNING The operator will announce The National Weather Service has issued a tornado warning for Jefferson County (or other county as appropriate) until XXX time (time will be announced by The National Weather Service. All areas should implement the Tornado Warning Policy, which includes moving visitors, patients and staff to interior locations.
10 Code X - Evacuation Operator will announce Code X + building + floor(s) with special instructions to evacuate from floor to floor or announce a complete evacuation of the facility The first evacuation attempt should be to evacuate horizontally into a safe area on the same floor Complete evacuation of the facility should only be used as a last resort when it is determined that horizontal or vertical evacuation into another location cannot provide a safe environment for visitors, patients and staff. ** Not used at University Hospital and the Brown Cancer Center **

15 Emergency Medical Assistance ULH Specific Code EMA: Emergency Medical Assistance We must provide emergency care to anyone on our campus (sidewalks, parking lots, clinics, etc.) Code EMA Pink: Emergency Medical Assistance involving a pregnant or laboring woman Stay on the line!!! The operator will ask you specific questions Is the patient awake? Breathing? Speaking? Pregnant or in labor? ** Not paged overhead only a special team responds **
16 Medical Emergency Team (MET) JHMC Staff dials 7777 and states MET Team needed, Room Hospital operator sends out page via LiveProcess MET Team Code SMEH Staff dials 7777 and states MET Team needed on Unit The operatory puts group page out to the MET Team and pages overhead MET Team to Unit
17 Code Pink: Missing Adult or Child Notify Operatory/Switchboard ( 7777, 20 or 5000 ) Immediately notify security and provide them with a description of the missing patient Employees should secure doors, stairs, elevators or any point of entry/exit from their respective areas Security and Administration will determine if the police should be notified Code Pink: Infant Abduction Watch for suspicious women (typical abductor) carrying large bags or purses
18 Code Black: Possible Explosive Device on Campus Notify Operator/Switchboard ( 7777, 20 or 5000 ) Operator will announce Code Black + Building + Floor/Unit/Dept If you take a bomb threat call try to gather as much information as possible, such as: Where is the bomb located When is the bomb going to explode What kind of bomb is it What does the bomb look like Characteristics of caller (male/female, young/old, accent, tone, background, does voice sound familiar, etc.) Follow directions of Security and local Law Enforcement Do Not Move Anything
19 Code Gray: Security Assistance Needed Notify Operatory/Switchboard ( 7777, 20 or 5000 ) Panic buttons notify Security directly Operatory announces Code Gray + facility/building + floor/unit/dept Remove patients/visitors/employees from harm if possible
20 Code Silver: Active Shooter Notify Operator/Switchboard ( 7777, 20 or 5000 ) Code Silver + facility/building + floor/department/unit Provide the following information: Location of the incident or direction of travel of the shooter Description of the individual(s): sex, race, stature, clothing, type of weapon(s), etc. Lock/barricade doors and windows Remain in locked or barricaded areas until the All Clear is announced
21 Code 200 Our Lady of Peace Specific Code 200 Call 7777 and request announcement: Code location House manager will respond immediately to assess patient care situation and determine if EMS is required
22 Code 100 and 100B Our Lady of Peace Specific Code 100 Call 7777 and request announcement: Code location or activate the transponder system which sends a direct signal to the hospital switchboard operator. Assigned code 100 employees with training in Safe Crisis Management (SCM) will respond to area immediately. Physical intervention is only to be used when other least restrictive interventions have failed within the SCM cycle. Code 100b Call 7777 and request announcement: Code 100b + location or activate the transponder system which sends a direct signal to the hospital switchboard operator. Only implemented when the Code 100 team feels additional personnel are needed to handle the individual situation.

24 Hazardous Materials Right to Know Hazardous chemical: You have a right to know any chemical that is a physical or health hazard in your workplace Hazardous chemical inventory list: A list of all the chemicals in the department. You will be instructed on the chemicals you utilize by your manager.
25 Hazardous Materials Right to Know Safety Data Sheets Name, alternate names Active ingredients Fire and explosion data, fire fighting methods Reactivity data, any other substances that react Spill or leak clean-up procedures Health hazards data, what to do if exposed First aid information Recommended PPE
26 Electrical Safety Electrical equipment is the leading cause of fires in the hospital. The Hospital has 2 electrical power systems; normal and emergency The Emergency Power system is identified by the red outlets All electrical equipment entering the Hospital must be checked for electrical safety Never us an extension cord or another power strip to lengthen the reach of a power strip! Follow the policy on power strips and extension cord usage Use only UL listed equipment
27 Radiation Safety Radiation Safety = Time, Distance and Shielding Minimize time around source Maximize distance from source Increase shielding near source (lead apron or walls) Tips Avoid areas with radiation symbol Dispose of radioactive materials properly Pregnant employees avoid exposure
