Rebate claim form (different form for each appliance) would be given to customer by seller. Each claim form will have a unique ID number and dealer will enter customer profile (name, address, etc) and manufacturer and model number. Customer signs, mails claim form and sales receipt to fulfillment center within 30 days. After 30 days, any unclaimed refunds would be available to others. Fulfillment company has an 800 call center for customers on status and other questions. Consumers and dealers would access information and forms via the Energy Office web site. At the site would be an overview of the process, access to forms (dealers would have protected access area), and other relevant information. Each application must be accompanied by original receipts and/or invoices that include the date of purchase, cost, and equipment make and model. No applicant can receive more than one rebate for a single appliance, or a gas furnace, per residential address. IIntended for replacement products only. NYSERDA will NOT be accepting applications for the Great Appliance Swap Out until the February 2010 launch during President s week. Under the approved plan, consumers could receive rebates for purchasing eligible appliances individually or in a bundle. Appliances will qualify only if they have earned the ENERGY STAR label, meaning that they are up to 30 percent more efficient than standard models on the market. Consumers may receive a larger rebate by purchasing three eligible appliances that meet standards issued by the Consortium of Energy Efficiency (CEE) that are higher than ENERGY STAR standards. Under the approved plan, customers purchasing appliances would qualify for a rebate of $75 ($105 with documented recycling) for ENERGY STAR qualified refrigerators, $75 ($100 with documented recycling) for clothes washers and $50 ($75 with documented recycling) for freezers. Rebates are available for dishwashers when they are purchased as part of a threeappliance package (refrigerator, dishwasher, clothes washer), which may qualify for a $500 rebate ($550 with documented recycling).

11 NORTH CAROLINA Total Funding: $8,848,616 Purchase Dates: April 22 - April 25 Clothes Washers - ES $124 Dishwashers - ES $94 Refrigerators - ES $177 Freezers - ES $132 Water Heaters (begins June, 2010) - ES $200 Required. North Carolina specifies that the following products will require recycling. North Carolina law lists the following items: refrigerators, ranges, water heaters, freezers, unit air conditioners, washing machines, dishwashers, clothes dryers and other similar domestic and commercial large appliance. 15% POS rebate on qualified appliances. Eligibility Rules Only North Carolina residents will be eligible to participate in the Program. Each North Carolina resident/household can apply for any/ all of the ENERGY STAR appliance rebates in any/all of the rebated categories. Rebates will be limited to one rebate per ENERGY STAR white goods appliance per household. Up to two HVAC units per household can be rebated; this number coincides with the participating utility partner s existing HVAC rebate programs. Consumers must be replacing an existing appliance and they will agree to recycle the existing appliance in order to qualify for the SEEARP rebate. Rebate Process for ENERGY STAR Household Appliances The third party provider will administer the point-of-sale rebates for consumers who purchase ENERGY STAR household appliances from retailers. Retailers will work with the third party provider to electronically transmit information that confirms the sale, verifies the model number and identifies the purchaser for re-imbursement of executed rebates. Upon receiving the batched sale documentation, the third party provider will provide rebate re-imbursements. Retailers, with more than one participating store, will be sent one check or electronic payment to that retailer s headquarters for administering/distributing to the participating stores as they see fit. The payment re-imbursement will be sent to the retailer within seven to ten business days following receipt of verification. Rebate Process for ENERGY STAR Heating, Air Conditioning and Water Heating The third party provider will send consumers, who purchase ENERGY STAR HVAC and W/H units, rebates following the confirmation of the installation. The utility s rebate to the consumer (where applicable) will be in addition to the ENERGY STAR rebate (that is the same statewide) provided by SEEARP.

12 NORTH DAKOTA Total Funding: $615,000 Purchase Dates: April 2010 Refrigerators - ES $150 Replacement of old appliance required (disposal invoice required) Online registration and mail-in rebate. Consumer Eligibility Rules Must be a resident of North Dakota. Limit of one refrigerator per applicant. Limit of one refrigerator per household. Must be an ENERGY STAR qualified refrigerator. Must be a full size, 7.75 cubic feet refrigerator or larger. Consumers must either have a qualified retailer pick up an old full size refrigerator or take an old full size refrigerator to a qualified inert waste landfill. Application Procedures Upon the start date of the program, consumer logs on to application website ( Register for the rebate by filling out required information on the application and then print out the rebate form. The rebate form will have a unique identification number that will be used to associate one consumer with one rebate. Only the name registered with the unique identification number can apply for the rebate. Purchase an Energy Star qualified refrigerator. Consumers may purchase from a qualified retailer. Qualified retailers will dispose old refrigerators. Or, consumers may purchase from a qualified or non-qualified retailer. Consumer will dispose old refrigerator at an inert waste landfill. Fill in required product information on rebate form; attach product recite or invoice and copy of consumer s state identification. If consumer disposes of refrigerator, disposal invoice must be provided as well. Mail the completed rebate form with attachments to NDO- REEE. NDOREEE will review the rebate forms that are received and if they qualify, will send the consumer a prepaid debit gift card. Old appliances must be collected and hauled away for proper disposal or recycling in accordance with all federal and state regulations. A list of recommended recyclers will be provided by the state. Mail-In. Rebates will allow for on-line reservations at POS. Funds are first come first served. White Goods Rebate Schedule (Energy Star refrigerators, dishwashers, and clothes washers) - White goods must be purchased between March 19, 2010 and April 12, 2010 (dates are subject to change based on number of rebates claimed and availability of funds); and Water Heater Rebate Schedule (Energy Star high--efficiency gas storage water heaters, and electric heat pump water heaters) - Water heaters must be purchased between March 19, 2010 and May 17, 2010 (dates are subject to change based on number of rebates claimed and availability of funds). Consumers should expect to receive rebates in a timely fashion from a predetermined third party administrative rebate processor. Processing times are estimated to take between four to six weeks. Rebate payment processing will be performed via a mail-in rebate, which will be mailed back to the consumer in the form of check, or prepaid checking card (e.g. Visa or Mastercard ). The State of Ohio will select a third-party administrator who will handle the rebate processing requests as well as customer service and customer inquires. Rebates will allow for on-line reservations at the either the point of sale, or at the consumer s home or other internet connection through a secure portal that will track and inform consumers of remaining budgets. Oklahoma Total Funding: $3,495,000 Purchase Dates: March 1 Clothes Washers - ES $200 Refrigerators - ES $200 Room Air Conditioners - ES $50 Water Heaters - ES $50 Ground Source heat pumps - ES $250 Must replace old appliance Oklahoma residential consumers will be eligible for rebates in Oklahoma, and there is a maximum of one rebate per appliance and two rebates per household. Appliances bought prior to the official start date will not qualify for the rebate. This is an appliance replacement program, designed to take inefficient appliances out of service and replace them with high-efficiency models. Old appliances must be removed from the property in order to qualify. First-time buyers of these appliances also are ineligible. The new appliance must replace an old appliance.

14 OREGON Total Funding: $3,636,000 Purchase Dates: December 15 Gas Furnaces - ES: 70% up to $2000 Heat Pump Equipment - ES: 70% up to $2000 Required Online registration and Mail-In. The CAP agency representative will inspect and determine if your heating system is in need of replacement, determine the type of heating system (brand and model) to be installed and arrange for a CAP-certified contractor to install the new system. Once the system is installed, it will be inspected. The Oregon Department of Energy rebate will pay the CAP agency directly for the equipment (70 percent up to $2,000). The CAP agency will also pay the contractor directly for the remaining costs including installation. The homeowner will be asked to sign a release for the funds, but will not need to pay for either the equipment or installation. The Commonwealth of Puerto Rico will implement a voucher rebate program to help residents replace older, inefficient appliances with new ENERGY STAR qualified appliances. Vouchers for consumers to redeem at participating retailers will be distributed through all 78 municipalitiesties of Puerto Rico, based on population. The program is tentatively scheduled to begin in mid-january 2010, with vouchers valid for 30 days. If funds remain, an additional round will begin the end of May until funds are depleted.

The rebate provider will promote the rebates through the RI Office of Energy Resources and Utility websites and retail outlets will provide information to residents on existing utility and municipal appliance recycling programs. Since there are limited funds available, rebates will be on a first-come-first-serve basis while funds last. The telephone number will tell consumers how much money is left in the program. In some cases ARRA rebates might be combined with local utility or retailer rebate/discount programs. SOUTH CAROLINA Total Funding: $4,298,000 Purchase Dates: March 31 Until funds run out Clothes Washers - ES: $100 Dishwashers - ES: $50 Heat Pumps - ES: $500 Refrigerators - ES: $50 Room Air Conditioning Units - ES: $50 Water Heaters (Gas Condensing) - ES: $400 Water Heaters (Electric Heat Pump) - ES: $400 Water Heaters (HE Gas Storage) - ES: $100 Gas-Condensing Water heater - ES $400 Electric Heat Pump Water Heater - ES $400 Hi Efficiency Gas Storage Water Heater - ES $100 Required. Customers who purchase a new appliance must turn in the old item. Stores and contractors participating in the rebate program must agree to recycle all old appliances in accordance with standard industry practice. Recycling is required to ensure that inefficient appliances are removed from the power grid. Appliances purchased in-store will receive an instant rebate at point of sale. Installers of whole-house equipment like water heaters or HVAC will reserve rebates online and provide customers with mail-in forms. Rebates will not be retroactive they will only apply to purchases made after the program launches. For appliances bought in a store, the rebate will be applied instantly when the customer goes to the register. Installers of whole-house equipment such as water heaters and HVAC systems will reserve rebates online and then provide customers with mail-in forms. An electronic system will track each rebate issued. The rebates will be available only to South Carolina residents making purchases from South Carolina stores or contractors. The program will start March 31, 2010 and will continue until all funds are exhausted.

Appliances must be purchased on or after March 1, and each household will be eligible to receive one rebate for each type of appliance. TENNESSEE Total Funding: $5,963,000 Purchase Dates: April 22 Air source heat pumps - ES $250 Room Air conditioners - ES $40 Consumers receiving rebates under the state s program may also be eligible for additional financial assistance through TVA s existing residential efficiency programs. Appliance rebates will be available to Tennessee citizens only. Applicants will be expected to demonstrate eligibility via a valid Tennessee driver s license, for instance. It is also important to note that the program is intended to replace older, inefficient appliances. Consumers receiving rebates under the state s program may also be eligible for additional financial assistance through TVA s existing residential efficiency programs. Only qualifying Energy Star heating and cooling units that are purchased on or after the official start of the program will be eligible. There will be no retroactive rebates for appliances purchased prior to the launch.

Customers applying for Cash- Back Rewards on refrigerators, freezers, or central air conditioners must provide proof that their old unit was taken out of service and properly recycled. Customers have several options to provide this proof. Work with a program-approved retailer to pick up the old appliance when they deliver the new one. Program-approved retailers have agreed to take old appliances picked up through the program out of service and recycle them. Use an existing recycling service to recycle the old refrigerator or freezer. Eligible services include municipal programs, utility programs, and private recycling services. Central air-conditioners must be turned over to the contractor installing the new central air-conditioner. Complete and submit Mail-In rebate from retailer. The rebates will not be retroactive. Purchases made prior to January 1, 2010 will not be eligible to receive Cash-Back Rewards. Consumers will receive a mail in reward application at the time of purchase directly from their retailer or contractor. Within 30 days consumers will need to fill out the form, attach a copy of the purchase receipt and submit it to the address indicated on the form. Consumers will receive a check for their Cash-Back Reward within 6 to 8 weeks. Cash-Back Rewards are only available to residential customers who are replacing an existing appliance that will be installed in a Wisconsin residence. Busi nesses, including landlords, are not eligible. Rewards are for replacement appliances only. Appliances installed in new construction are not eligible for rewards. The program is available to all residential customers regardless if their utilities participate in Focus on Energy. For customers whose utilities do participate in Focus on Energy, eligibility for both rewards is possible if the equipment qualifies for both SEEARP and Focus on Energy. Consumers purchasing multiple qualifying appliances for their residence can receive Cash-Back Rewards for each purchase. Cash-Back Rewards are available to residential customers. Businesses, including landlords, are not eligible. In the event funds are exhausted prior to February 2012, Focus on Energy will announce a program end date. All qualified purchases made prior to the program end date and submitted within the required time frame will receive a Cash-Back Reward.
