2 Deck Registration: Entry Limits, Fees and Deadlines: athletes who meet the qualification criteria. No swimmer will be permitted to compete unless the swimmer is a member as provided in Article 302. This meet will be run according to current USA Swimming rules and regulations. Swimmer s age on the first day of the meet determines age group for the entire competition. No deck registration will be accepted; swimmers must be registered at time of entry. This meet will be limited to the number required to comply with the 4 hour rule. Teams will not be split. Individual swimmers may enter up to SIX (6) individual events, swimming no more than THREE (3) individual events per day. Swimmers may enter any event for which they have achieved a USA motivational B time standard. Swimmers may not enter any event in which they have achieved a 2016 Utah Swimming Long Course State Time. Bonus Events: Each swimmer will be allowed up to two (2) bonus events. Bonus events MUST be designated as such during the entry process. No times (NT s) may NOT be used as bonus events, the 400 free cannot be a bonus event. No Times (NT s) are NOT accepted. Surcharge per participating swimmer: $9.00 Individual Event Entry: $5.25 All fees must accompany entries and are nonrefundable. One team check is recommended. Make checks payable to OTAC. Entries will be accepted starting Monday, June 20, 2016 at 12:00pm (noon). All entries must be received (in the hands of the below designated person) by 7:00 PM, Saturday, July 2, No deck entries will be allowed. Individuals/Teams mailing the USI Official Entry form found on the sanctions page of the Utah Swimming website ( must make sure that each form is signed and provide time verification for each entry. Entries are not accepted without said verification.

3 Entries are not considered confirmed until the team entry representative receives official confirmation from the meet entry contact. If the team entry representative does not receive an confirmation regarding your entries within 24 hours of the entry deadline, please contact the meet entry contact immediately. Coaches should bring a hard copy of electronic entries to the meet to address any entry issues. The person submitting a team s entries is the team entry representative and the received implies the official signature of the team entry representative. Said person will be the only person to make changes and will be responsible for all fines and penalties resulting from inaccurate times and unregistered swimmers. Entry Rules: *Whether entries are submitted electronically or on paper, by the action of entering the swimmer in the meet the submitter, whether the swimmer, coach, parent or team representative, certifies that the swimmer is a registered member of USA Swimming and that all times stated in the entry are true and correct. *The submitter further assumes responsibility for any false or incorrect times, and understands that all listed entry times are subject to Utah Swimming Time Verification Procedures, and that a fine of $20.00 shall be paid to Utah Swimming, Inc. for each incorrect time, unless corrected before the entry deadline. It is understood that an automatic call before the review section shall be the result for a third entry time that cannot be verified. The submitter further agrees that a penalty of $ shall be assessed to anyone entering a swimmer in a meet sanctioned by Utah Swimming, Inc. who is not registered with USA Swimming at the time of entry, upon it being proven that the swimmer was not so registered. Entries must be submitted in the course in which they were achieved. Converted times should NOT be used. All times submitted must be the swimmer's best time. Event seeding will be in the following order: conforming LCM, non-conforming SCY, nonconforming SCM A mandatory scratch down may apply to this meet if the time line exceeds 4 hours for 12-Under swimmers, subject to the provisions of USA Swimming Rules and Regulations F. If a scratch down is necessary, teams will be reimbursed for said scratched events. Meet Format, Check-in and Scratches All events will be timed finals. Time trials will not be offered at this meet. Swimmers may enter with any USA Swimming achieved time. NTs will NOT be accepted. Positive check-in is required for the 400 IM and 400 Freestyle. Swimmers must check in for the 400 IM on Friday by 3:00pm. Swimmers must check in for the 400 Freestyle by 10:00am on Saturday. Swimmers who do not check in will not be allowed to swim that event. All other events will be negative check-in. All 400 IM and 400 Freestyle swimmers must provide their own timers. All scratches must be ed to the Meet Director by 6:00 PM the night

4 before the session being scratched. Swimmers who do not scratch and miss their event will not be allowed to swim that event. Warm-up: USA Swimming and Utah Swimming safety procedures will be enforced throughout the meet. Warm-ups are subject to the following procedures: Warm-up: Coaches have the responsibility of insuring that all their swimmers are instructed in warm-up procedures and that the swimmers follow all Utah Swimming safety procedures. All swimmers will enter the pool from a sitting or 3-point position. No pushing, splashing, running or any other general horseplay. Swimmers engaging in such activities will be asked to leave the pool. All USA Swimming athletes must be under the supervision of a USA Swimming member coach during warm-up, competition, and warmdown. The Meet Director or Meet Referee may assist unaccompanied swimmers in making arrangements for such supervision, but it is the swimmer s responsibility to make such arrangements prior to the start of the meet Dive Certification: All lanes of the pool will be circle swimming only. During the last 15 minutes of warm-up lanes 2 and 7 will be opened for diving starts. Areas of the pool designated may be used for warm-down. Warm-up and warm-down areas may be used but limited to 10 minutes prior to or following a race. Any swimmer entered in the meet must be certified by a USA Swimming member-coach as being proficient in performing a racing start or must start each race from within the water. When unaccompanied by a member-coach, it is the responsibility of the swimmer or the swimmer s legal guardian to ensure compliance with this requirement D Restrictions: Deck changes are prohibited I Use of audio or visual recording devices, including a cell phone, is not permitted in changing areas, rest rooms or locker rooms H Adaptive Swimming Scoring: Awards: Results: Adaptive swimming: In accordance with USA Swimming rule & Article 105.1, the Meet Referee has the authority to accommodate swimmers with disabilities in accordance with USI P&P The coach or person in charge of team entries must notify the meet referee and meet entry contact of accommodations requests at the time of entry submission. No Scoring of Events. Medals will be awarded 1st 3th place in each individual event, with Ribbons being awarded 4th-8th place. Age Groups: 10 & Under, 11-12, 13-14, and 15 & Over. Events may be swum as combined age groups but will be awarded according to the preceding age groups. Final Results will be available on the Utah Swimming web site in a (HTML) printable format and in a downloadable Team Manager (Cfile.zip) format. Results will not be mailed.

5 Deck access: Deck access is limited to swimmers, coaches, officials & meet workers. Officials and coaches must wear their USA Swimming ID cards to be allowed on deck. Officials: Hospitality: Timers: Parking: Web Site: Officials from visiting teams will receive a $15 gift card for each session worked. Officials from OTAC will have their meet fees waived. Administrators, please let me know which officials you will have coming from your team so we can plan accordingly. There will be food and drinks available at all sessions for officials and coaches. Teams with more than 10 swimmers will be asked to provide timers. Lane assignments will be sent out prior to the meet. Swimmers must provide their own timers for the 400 IM and 400 Freestyle. 7 Orem Fitness Center rules for swim meets No food on deck for any participants. Any food must be consumed upstairs near the viewing gallery. If a parent is bringing something for the team or a participant it must remain upstairs at all times. Officials and coaches who are not able to leave deck may have limited food (no glass). Participants and spectators must keep to the pool and viewing gallery only. If you would like to use the rest of the facility you may purchase a day pass or use a current pass if you have one. Each participant and spectator who does not wish to purchase a day pass will be given a stamp at the door to indicate they are only attending the meet. All participants and spectators are expected to adhere to the Code of Conduct. Any violations will result in immediate removal from the facility. City of Orem Recreation Department CODE OF CONDUCT The City of Orem Recreation Department has established a Code of Conduct which applies to all persons participating in any way in any Orem City Recreation Department programs or facilities. All participants, volunteers, coaches, patrons, and spectators are expected to know, promote, and adhere to the following standards of conduct. General: 1. Promote and encourage good sportsmanship by demonstrating positive support for players, coaches, officials, participants, and employees. 2. Treat everyone with respect, regardless of race, color, age, sex, religion, national origin, disability, or skill level. Putting down or demoralizing others is not acceptable. 3. Adhere strictly to the rules of the game/facility (including official s or employee s decisions) and do not attempt to change or manipulate those rules in order to win or for personal benefit. 4. No smoking, drinking, or being under the influence of alcohol or drugs will be allowed in the facilities or on playing fields. 5. Abuse or harassment of players, volunteers, coaches, officials, spectators, or employees in any form will not be tolerated. Bullying is a form of harassment which is also prohibited. 6. Foul, abusive, profane, or vulgar language is prohibited and will not be tolerated. 7. Fighting is prohibited. Fighting includes physical (hitting, pushing, touching, getting in the face of ) or verbal (threatening, making fun of, calling names, cursing, arguing).

8 8. Remember that youth programs are for the kids [to learn fundamentals, techniques, sportsmanship, teamwork, and have fun]; they are not for adults! Therefore, all kids deserve a right to fairness at all times. 9. Conduct yourself in a manner that would be an example and a role model to youth under your direction or in the vicinity. 10. Coaches agree to support and work with assigned team throughout the designated season, and agree to hold regular practices (at least once a week) and show up to scheduled games. 11. [Respect for equipment, field, facility, etc.] Any equipment issued does not become personal property and all equipment must be returned in good condition to the City at determined time. 12. Promote team work, physical conditioning and teaching skills appropriate to the sport. Do not teach, encourage, or allow rough or dirty tactics of play. Anyone who does not comply with this policy, or who conducts himself or herself in any unbecoming manner, will be subject to disciplinary action including (but not limited to) verbal or written warning, being asked to vacate the premises, suspension, or expulsion from the program, class, facility, or field and associated privileges will be terminated without refund. Financial restitution may be required when appropriate. Criminal violations will be prosecuted to the fullest extent of the law
