19 HRET HEN Sustainability Strategy Going ALL IN for 2015 and beyond! Powerful improvement in many areas over the last few years AHRQ CUSP CLABSI, CUSP CAUTI, CMS HEN Meaningful, dramatic and accelerating improvement which equates to patients lives saved, harms reduced and costs avoided Hospitals that are all in working to improve across the board in multiple projects thru multiple strategies have produced powerful results. HEN has provided incredible platform and network to continue the work Considering above, HRET is planning to launch the All In Improvement Network in 2015

20 HRET HEN Sustainability Strategy All In Improvement Network/Triple Aim Collaborative (Final name TBD) Open to all hospitals at no charge Will have multiple HEN-like topics Hospitals CEOs and leadership must commit to data submission and working on multiple topics HRET will provide resources for data submission/analysis, subject matter experts, change packages, webinars and host 3 to 4 face to face meetings in 2015 and 2016

(National Baseline=20%) READMISSIONS REDUCTION BREASTFEEDING INCREASE Better Healthcare FAMILY VISITING 24/7 Example AIM: 100% of hospitals will have non-restrictive visiting hours for families in all units (National Baseline=86%) BEDSIDE ENGAGEMENT TOTAL PATIENT HARM REDUCTION Lower Cost APPROPRIATE USE OF IMAGING AIM: Overall reduction of MRI use by 10% for acute low back pain and 10% for CTs for headaches in the ED APPROPRIATE BLOOD MANAGEMENT APOLOGY AND DISCLOSURE
