2 Your privacy. Hostplus is seeking to collect personal information from you so that we may apply your contribution to your or your spouse s account. The personal information we are seeking to collect from you is your name, address, date of birth, contact details and your spouse s name, address, date of birth and details. We need to collect the requested personal information from you to complete your request today. If you do not provide this information, we will be unable to action your request. It is your responsibility to inform your spouse that you have provided Hostplus with their personal information. The Hostplus privacy policy is available on the Hostplus website at hostplus.com.au/privacy and includes information about overseas disclosure of personal information, how you and your spouse can access and seek correction of your personal information as well as make a complaint about a breach of privacy. Hostplus usually discloses your personal information to our administrator Superpartners, mail houses, our insurer Metlife Ltd and the ATO. Superpartners may disclose your personal information to overseas recipients.
3 How to make a contribution. You can use the contribution slips in this booklet to make personal contributions or spouse contributions. For personal contributions. Step 1: Step 2: Step 3: Step 4: Complete the front of the slip. Make your cheque out to Hostplus and write your membership number on the back of the cheque. Send your completed slip and cheque to Hostplus, Locked Bag 3, Carlton South VIC Record your contribution in the table at the back of the booklet for your own reference. For spouse contributions. Step 1: Step 2: Step 3: Step 4: Complete the back of the slip. Make your cheque out to Hostplus and write the receiving spouse s membership number on the back of the cheque. Send your completed slip and cheque to Hostplus, Locked Bag 3, Carlton South VIC Record your contribution in the table at the back of the booklet for your own reference.
4 Making after-tax super contributions. The following tables briefly explain your eligibility to make personal or spouse contributions and the contribution limits that apply. Personal contributions Under 65 Over 65 and under 75 Any person may make personal (after tax) contributions. An annual limit of $180,000* 1 per member or $540,000* averaged over a rolling three year period applies to all after tax contributions made to your super account 2. Please note if you have more than one superannuation fund, all personal (after-tax) contributions are counted towards the non-concessional contributions cap. A member may make personal contributions if they have been gainfully employed for at least 40 hours in 30 consecutive days during the financial year. An annual limit of $180,000* 1 per member applies if you satisfy the work test each year 3. * This is referred to as non-concessional contribution caps. 1. Please note that before 1 July 2014 this cap was $150,000 or $450,000 over a three-year period, and if you commenced making nonconcessional contributions in excess of $150,000 in the financial year you may only contribute up to an aggregate $450,000 (and not $540,000) over your 3 year bring forward period. 2. If you are 63 or 64 you can bring forward two years of non-concessional contributions without satisfying the at work test in the following two years (see the definition of at work below). 3. The at work test means you must be in gainful employment for at least 40 hours within a 30 consecutive day period in the financial year in which you contribute. Unpaid work does not qualify.
5 Personal contributions 75 and over Self-employed Personal contributions are not allowed. If you are self-employed 5 you can make before-tax and after- tax contributions up to age 75, provided you are gainfully employed for at least 40 hours in 30 consecutive days during the current financial year 2 between age Contributions caps apply. Please refer to the Product Disclosure Statement on our website for more information on contributions caps including excess contributions tax which may be payable. 5. Eligibility criteria applies. Please refer to for more information. Once you turn 65, we ll write to you each year to confirm your continuing eligibility to make super contributions.
6 Spouse contributions. You can make contributions for your spouse if you re living together and you re both Australian residents. A spouse is someone who is legally married to you, someone who lives with you on a genuine domestic basis in a relationship as a couple or someone (either of the same or different sex) with whom you are in a relationship that is registered under State or Territory law. If you are your spouse s employer, a couple living apart or if you stop living with your spouse, you re not eligible to make spouse contributions. The receiving spouse is under 65 The receiving spouse is over 65 and under 70 The receiving spouse is 70 and over Contributions can be made at any time, regardless of the age and employment status of the receiving spouse. An annual limit of $180,000 1 per member or $540,000 averaged over a rolling three year period applies to all after-tax contributions made to your super account 2. Contributions can be made if the receiving spouse has been gainfully employed for at least 40 hours in 30 consecutive days during the financial year. An annual limit of $180,000 per member applies if you satisfy the work test each year 3. Spouse contributions are not allowed. Spouse contributions will be applied to the receiving spouse s non-concessional contributions cap.
