1 JOB DESCRIPTION Job Title: Pharmacy / GP Liaison Nurse Service/Department: Tri-borough Primary Care Support Service Location: Tri-borough Grade: Band 6 Reports To: GP Shared Care Senior Nurse/Practitioner Job Purpose The post holder will work with GP practices and community pharmacies across the Triborough (Westminster, Kensington and Chelsea and Hammersmith & Fulham) to encourage and facilitate participation in the GP shared care scheme. The post holder will also provide nursing support for an Intermediate Care Clinic held within Blenheim and support the development and provision of enhanced primary care provision for community alcohol detoxification within a recovery model. Main Duties and Responsibilities 1. Primary Care Liaison and Engagement 1.1. Coordinate the participation and engagement of GPs and community pharmacies in the primary care scheme Proactively visit all participating and interested surgeries and pharmacies to build relationships, instil confidence in the new service and troubleshoot any issues Support GP shared care arrangements, advising health care professionals in relation to opiate substitute treatment, alcohol and other drugs Provide direct nursing expertise and support/advice to the Intermediate Care Clinic Provide direct nursing expertise and support /advice to the alcohol community detox hubs Undertake comprehensive assessments, risk assessments and recovery care planning to service users within a primary care setting Support GP Shared Care Workers with client work where required Ensure high standards of patient care are maintained in line with shared care practice standards Contribute to the development of formalised pathways to wraparound and physical and mental health services.

Updating data systems including BOMIC and EMIS systems within primary care To support the coordination of community dispensing systems, in line with RCGP guidelines To attend GP meetings and GP clinic review meetings where appropriate Maintain professional links with related organisations to ensure elements of continuity of care, up to date profiling of the service, referral links and general information sharing To work in partnership with other voluntary agencies and statutory services and support the Joint Providers Forum Maintain effective working relationships with Tri-borough lead GPwSIs To attend external meetings as directed by the GP Shared Care Senior Nurse/Practitioner and GP Shared Care Manager. 2. Training and Development 2.1. Develop training programme on alcohol brief interventions and deliver to GPs and pharmacies across the Tri-borough Develop training programme on BBV interventions and deliver to GPs and pharmacies across the Tri-borough Develop and coordinate protocols on the delivery of dried blood spot testing and referral Provide support for the development of enhanced primary care provision within GP surgeries, including the development of community alcohol detox programmes in targeted clinics Provide advice and training to the whole GP shared care team on health and wellbeing, clinical issues, resolving conflict, as well as supporting staff who are working with service users with more complex needs Work in partnership with GP Shared Care Workers to co-ordinate service user pathways and to maintain positive developmental relationships with service users, GPs and pharmacies. 3. To co-ordinate the maintenance of client records in such a way that conforms to Blenheim policies and procedures and complies with medical and legal requirements Support the effective recording of interventions and activity on EMIS and BOMIC data systems To assist the Manager and administrator in the provision of information that enable data collection, input and retrieval To ensure that all interventions are underpinned by sound theoretical methods to ensure that outcomes are measurable To ensure that all communications take account of information governance policies to ensure appropriate levels of confidentiality. 4. General Duties and Responsibilities 4.1. To maintain all policies and procedures in place in Blenheim To attend supervision, appraisal and other meetings as directed by line Manager To represent Blenheim in a professional manner at all times To treat all colleagues and service users with respect and dignity To contribute to the strategic development of Blenheim by identifying skill sharing opportunities, training provision and the exchange of information To undertake any task in line with role and responsibilities as directed by the line manager. This job description is not exhaustive and may be subject to change, in consultation with the post holder and GP Shared Care Senior Nurse/Practitioner and GP Shared Care Manager, during the course of development of the post and service. Safeguarding Responsibility To be familiar with the Safeguarding Children and Adults at Risk Policies and Procedures within the wider organisation and the specific service. Financial Accountabilities Not applicable. Disclosure Requirements Enhanced DBS Disclosure every three years. Further requirement to disclose any convictions/cautions or criminal investigations incurred at any time during your employment. A minimum 12 months experience of working with people with substance misuse problems. 2. Experience of delivering training to a range of health and social care professionals. Qualifications 3. General Nursing Qualification at Band 6 with current PIN. Knowledge, Skills and Abilities 4. A good understanding of the complex needs and problems that confront those with a history of problematic substance misuse, e.g. physical, psychological, mental health, legal etc. 5. A good understanding of harm minimisation and how it can be used as an intervention to reduce the risk of harm to individuals and the wider community. 6. Able to assess the complex needs of drug users, identify risks and formulate action plans to identify how these can best be addressed. 7. Knowledge of the types of services available to people who have substance misuse issues and how they are accessed or referred into. 8. Ability to work independently and travel around the community whilst being an active team member. 9. Able to respond flexibly to meet the demands of the post. 8 Job Criteria 10.Able to establish and maintain good productive working relationships both within a multi-disciplinary team and with external partners using communication skills appropriate to the audience. 11.Good written communication skills with ability to write clear concise case notes, care plans, letters and prepare reports (including use of relevant IT). 12. A good understanding of safeguarding for both children and adults and the ability to apply safeguarding practices. 13. Able to reflect on own working practice to identify appropriate learning and development. Make effective use of supervision. 14. Demonstrates respect for the diversity, culture, values and choices of clients. Promotes equal opportunities and challenges discriminatory practice. GP Shared Care Service 15. Experience and knowledge of GP Shared Care arrangements and an ability to advise and support primary health care professionals in relation to opiate substitute treatment, BBV s, alcohol and other drugs. 16. Previous experience of opiate substitute prescribing within a health care setting. 17. Working knowledge of EMIS systems within primary care. 18.
