1 Feed-in Tariff application form Last updated 31st August 2013 General notes Please complete this form to apply for the Feed-in Tariff (FIT) scheme or make a change to an existing generation unit. We suggest you read our fact sheet and FAQs to make sure your equipment is eligible for payments. This will also help you to complete the form. Please complete a separate application form for each generator that you want to register for FIT payments. Unless indicated, please complete all sections of the application form. Solar applications must also complete two declarations from page 6 to 10. Before you send your application off, please check that you have included the following: If any of the information is missing we can t process your application which will mean a later eligibility date and new meter reading will be required to start your Feed In Tariff account. Please note that your application will be considered incomplete if the following information is not included. Valid Start Reading: Please include a meter read from the day the scanned application form is ed if not we will contact you to provide your opening meter reading. This could delay your application. Evidence of rights to Generate: Only applicable if you do not own the site at which your Generating Unit is installed. Generator Accreditation: MCS for 50kWh/Roo-fit for over 50kWh. EPC Certificate (if relevant) or valid exemption letter (for Solar PV only). Appropriate Solar PV Declaration. Please ensure you have signed two declarations (see pages 6-10). Proof of ownership (for example copy of paid invoice for generation equipment). Proof of address, if non E.ON customer. Proof of identification (for example, photocopy of passport/driving licence). Send your completed application form with supporting documentation to: fit-application-form-requests@eonenergy.com or post to E.ON UK, E.ON FIT Team, Caxton Road, Bedford, Bedfordshire, MK41 OEW. Please complete this application then scan and return by to the above address. This means your eligibilty date will not be delayed. If you need any further advice or information, call us on , 8am to 6pm Monday to Friday and 8am to 4pm Saturday. It can take 12 weeks to process your application. 2 Section 2 Your personal details Please provide us with contact details of the person who owns the generation equipment. This would normally be the person who receives the FIT payments, even if the property is occupied or used by someone else. Please supply proof of ID with your application. A copy of a passport, driving licence or equivalent identification document will be acceptable. If your energy is not supplied by E.ON, please also provide proof of address such as a utility bill, bank statement, council tax bill or equivalent document. If the Address has not been registered this may cause delays with the installation being added to the Ofgem Central FIT Register. ROO-FIT ROO-FIT accreditation number of accreditation Generating units over 50kW in size and all anaerobic digestion and hydro units must be ROO-FIT (Renewables Obligation Order) accredited. ROO-FIT accreditation means that your generation unit is registered under the Renewables Obligation Order (ROO) to receive Renewable Obligation Certificates (ROCs) and you have notified Ofgem of your intent to claim FITs. ROO-FIT accreditation is not the same as a ROO number. Section 8 Your meter details and readings (please only complete for new applications) You need to have a suitable generation meter to qualify for the FIT scheme. Do you have a generation meter? 6 Feed-in Tariff Solar PV declarations (new installations and extensions) All applications for accreditation of new solar PV installations (including extensions to existing installations), with an eligibility date on or after 1 April 2012, need to be accompanied by a copy of this document with the relevant section signed and dated. This will then be used by FITs licencees/ofgem as appropriate to determine whether or not (i) the energy efficiency requirement applies and, if so, has been met; and (ii) the multi-installation tariff rates should apply. If your application is for a new PV installation with an eligibility date on or after 1 April 2012, you must sign two of the enclosed declarations; one declaration from the energy efficiency section and one declaration from the multi-installation section. Tick one of the boxes in relation to the energy efficiency declarations and one of the boxes in relation to the multi-installation declarations. Then go on to sign the two relevant declarations. (This includes community energy or school installations) However, if your application is for an extension to an existing PV installation, you must sign one declaration from the energy efficiency section only. Tick one of the boxes in relation to the energy efficiency declarations then go on to sign the relevant declaration. Please read the following information to understand which of the declarations are relevant to you. 7 Feed-in Tariff Solar PV declarations (new installations and extensions) Energy Efficiency declarations (sign one declaration only from declarations 1-4) Declaration 1 I for the installation(s) at address certify in respect of this application for accreditation that all of the following are applicable: a. the eligible PV installation is attached to or wired to provide electricity to one or more relevant buildings; b. a valid energy performance certificate is enclosed in respect of the building (or one of the buildings) to which the PV installation is attached or wired to provide electricity; c. the enclosed energy performance certificate is the most recently issued energy performance certificate in respect of that building; d. the enclosed energy performance certificate certifies that the relevant building to which it relates has been assessed as being within bands A-D; ( e and f relevant if energy performance certificate issued in England and Wales only: if energy performance certificate issued in Scotland ignore e and f below and sign the declaration) e. (i) I am an owner or tenant of the relevant building to which the enclosed energy performance certificate relates; or (ii) I certify that I am not a person mentioned in (i) but such a person has co-signed this document; f. I: or (where this declaration is co-signed by another person, that person) consent to the disclosure of the enclosed energy performance certificate and / or the information contained within it to one or more FIT Licensees, the Gas and Electricity Markets Authority (Ofgem) and ministerial departments of the government and devolved administrations for retention and use by them for all purposes connected with administering, auditing, reporting on and performing statistical analysis on the Feed-in Tariff scheme for the duration of that scheme.
