1 Job Title: Community Nurse Mental Health (dults) Band: 6 Hours: 37.5 Department: Location: Reports to: Responsible for: Trustwide London or Kent Team Leader N/ Job Purpose: The post holder will provide effective assessment and / or case management to people with complex nonpsychotic disorders, using evidence based / client centred principles to assess, plan, implement and evaluate interventions. They will act as care co-ordinator under the CP process if indicated. The post holder will participate in the planning, development and evaluation of the service and supervise students on practice placements. They will also commit to carry out MDT activities e.g. duty responsibilities. 2 Key Responsibilities: 1) To be responsible, under supervision, for the assessment / care co-ordination for a number of cases as identified and supervised by the senior practitioner. To undertake collaborative risk assessments and risk management and crisis plans as appropriate. To work with service users to identify recovery goals as part of the overall care plan. To work as an integral part of the MDT in providing a flexible and responsive service where required (this will involve a duty system). To provide supervised interventions in evidence based practice where required. To work within policy, professional and legal frameworks at all times and have a clear understanding of ones Code of Professional Conduct, adult and child protection protocols, Mental Health and Capacity cts and CP, and Trust clinical policies 2) To effectively work as a member of the multi-disciplinary team and contribute to / implement decisions with regard to service users treatment and care. To develop therapeutic relationships which facilitate collaborative working. To actively promote relationships with colleagues that support open communication and supportive working, taking personal responsibility for one's contribution to the process of effective Team working. To maintain a high standard of written communication, recording appropriate events and observations in accordance with Trust policies and Code of Professional Conduct. To maintain confidentiality of information in accordance with Trust policies and profession s Code of Conduct. Maintain appropriate, up-to-date, written and electronic records and activity data in accordance with Professional and Trust standards. Contribute to team and CG audit through completion of outcome measures 3) To review, reflect and improve practice through effective use of clinical and managerial supervision and appraisal. To undertake the supervision and appraisal of junior staff as required. To demonstrate an ability to identify areas for personal development to improve and enhance knowledge and skills. To demonstrate an awareness of the effects of and one's own limitations in fulfilling some aspects of the role on the mental wellbeing of oneself, and demonstrate ability to seek assistance and guidance from colleagues. To show self-awareness and an ability to reflect and discuss emotions and thoughts in relation to care of service users. 4) To demonstrate an ability to initiate, plan and implement the induction, training and education of students and other staff. To regularly be responsible for the supervision and written assessment of students on practice placement within the Service. To participate in inter-professional training within the service as part of the teams training and development initiatives. Provide clinical training and supervision to clients extended support network as appropriate. 5) To comply with the code of ethics and Professional Conduct and national and local procedures and guidelines. To respect the individuality, values, cultural and religious diversity of clients and contribute to the provision of a service sensitive to those needs. Demonstrate an ability to apply increasingly complex skills and knowledge commensurate with establishing higher levels of professional practice. 6) To actively participate in and contribute to the planning, delivery, evaluation and audit of practice, clinical pathways and protocols as required. To participate in research and audit activities. To contribute to meeting the strategic targets and the needs of the borough s residents and the Trust, including outcome monitoring. 7) To exercise good time management, punctuality and consistent, reliable attendance.

Essential Requirements Desirable Requirements Core Mental Health Qualification (RMN, OT, DpSW) Experience working within a community mental health setting Experience working with people with complex mental health difficulties Knowledge of specialised evidence based interventions of people with personality disorder/recurrent depressive disorder/ dual diagnosis Experience of recovery based socially inclusive model Use of outcome measures to evaluate service effectiveness Experience working as a Care Co-ordinator (or equivalent) Clear understanding of dynamics of a team using a team approach Experience working within an integrated multi-disciplinary team udit/evaluations skills Has worked with service user/carer groups Local knowledge dependant on post Experience of supervising junior staff or students Has worked with people with challenging behaviours Knowledge of evidence based treatments for mental health disorders Knowledge of mental health legislation and national policy Knowledge and recovery model and other theoretical models related to the care of people with severe mental health difficulties Knowledge of principles of risk Completed mentorship training

SLaM employ around 5000 staff and serve a local population of 1.1 million people. We have more than 230 services including inpatient wards, outpatient and community services. Currently, provide inpatient care for approximately 5,300 people each year and treat more than 45,000 patients in the community in Croydon, Lambeth, Lewisham and Southwark; as well as substance misuse services for residents of Bexley, Bromley and Greenwich.
