6 Evidence criteria of the WCRF Report Convincing These criteria are for evidence strong enough to support a judgement of a convincing causal relationship, which justifies goals and recommendations designed to reduce the incidence of cancer. A convincing relationship should be robust enough to be highly unlikely to be modified in the foreseeable future as new evidence accumulates. All of the following were generally required: Evidence from more than one study type. Evidence from at least two independent cohort studies. No substantial unexplained heterogeneity within or between study types or in different populations relating to the presence or absence of an association, or direction of effect. Good quality studies to exclude with confidence the possibility that the observed association results from random or systematic error, including confounding, measurement error, and selection bias. Presence of a plausible biological gradient ( dose response ) in the association. Such a gradient need not be linear or even in the same direction across the different levels of exposure, so long as this can be explained plausibly. Strong and plausible experimental evidence, either from human studies or relevant animal models, that typical human exposures can lead to relevant cancer outcomes. 13 The evidence for adverse effects of alcohol is sufficient and consistent based on results from cohort studies and there is an apparent dose response relationship. The evidence that alcohol consumption of more than 30 g has adverse effects in men is convincing, probably this is also the case for women. 13

14 Practical recommendations There is abundant and consistent epidemiological evidence with a clear dose response, and evidence for plausible mechanisms that operate in humans. The evidence that greater body fatness is a cause of colorectal cancer is convincing. 14

16 Apparently, there is a clear dose response relationship from consistent cohort studies and this is supported by plausible modes of action. Confounding factors however can not be totally excluded, therefore foods rich in dietary fibre are considered as probably protective. There is consistent evidence for a probable effect of garlic from few studies mainly in form of case control studies. Both dose response relationship and plausible modes of action and data on the effects of allium species on stomach cancer have contributed to this statement. 16

17 There is consistent evidence from several cohort studies and studies on colorectal adenoma and evidence from a plausible mode of action. Therefore, calcium is attributed a probable protective effect. 17

18 Take Home Messages The optimum cancer diet? Ultimately, equal guidelines can be issued for cancer prevention (with few exceptions) as for a generally healthy diet. Supplements (particularly β carotene and iron, but also mega doses of nutrients) are generally regarded as negative. Take Home Messages The optimum cancer diet?
