1 I. SECTION 106 HISTORIC PRESERVATION SERVICES Narrative 1: Capacity (35 points) 1. Discuss your ability and past experience in completing environmental review documents that fulfill the requirements of the National Environmental Policy Act (NEPA), California Environmental Quality Act (CEQA), and all applicable U.S. Housing and Urban Development Department (HUD) regulations as cited in 24 CFR Part 58 and 36 CFR Part 800. Describe your experience with the California State Historic Preservation Officer (SHPO). Discuss your experience in preparing Section 106 review documentation. 2. Discuss the evidence of meeting the Secretary of the Interior s Historic Preservation Professional Qualification Standards (62 FR 33708, June 20, 1997) or the federal requirements for Historic Preservation as described in 36 CFR Part Discuss your knowledge of environmental regulations and requirements mandated by HOME Investment Partnerships (HOME) program, the Community Development Block Grant (CDBG) program and Lead Hazard Control and Healthy Homes (OLHCHH) program in general. 4. Discuss your past performance in meeting cost, schedule, and program requirements. Include detailed aspects of performance such as actual goals met and timeframes for completing reviews. 5. Discuss your experience in preparing, administering, and amending a Programmatic Agreement with the SHPO. In your response to this question, please discuss your experience sharing information and data with and between the SHPO and the Advisory Council; provide examples to illustrate this experience. 6. Discuss past projects, the sources of funds, and the type of services provided in the last three years. Please include a discussion on your approach and experience related to archeological/historic review costs and the typical completion time given your experience. Complete a separate copy of Exhibit III - Applicant Agency Reference Form for each of the three largest funded projects in the last three years. See Exhibits for required form. Narrative 2: Qualifications (35 points) 1. Discuss the experience and educational background of professional staff that will administer the contract and prepare historical review documents. Provide
2 evidence that staff completing historic reviews meet the minimum professional qualifications as defined in the Secretary of the Interior s Professional Qualifications Standards (48 FR ). Provide proof of qualifications in the form of résumés for principals and staff. 2. Discuss how the functions of the position(s) are appropriate to accomplishing the services. Include qualifications and experience required for the position. Provide a position description for each position. 3. Provide an organizational chart showing staff, by job title, who will be involved in the administration of the contract. Narrative 3: Work Product (15 points) Provide a sample of your company s Standard Mitigation Measures for Adverse Effects Memorandum. Narrative 4: Cost (15 points) Provide a copy of your company s fee schedule.
3 II. INSPECTION, SURVEY AND ANALYSIS FOR HAZARDOUS MATERIALS (LEAD AND ASBESTOS) Narrative 1: Capacity (35 points) 1. Describe past experience working with other municipalities, government agencies and private companies in providing inspection, survey and analysis for hazardous materials. Please include the type of services, how they were funded and whether it was for single- or multifamily housing. Please indicate if the described experience was funded by any of the following HUD-funded grants: Lead, CDBG, HOME, or any others. 2. Identify the staff positions that will be working with HCIDLA staff. Please identify the primary staff member overseeing this program and include a job description for each position identified. The description should include the responsibilities as related to this grant. 3. Provide staff resumes. Resumes should include relevant experience, qualifications, language capacity and any county, state and federal certifications. 4. Provide a chronological work plan on the Lead Inspection/Risk Assessment (LI/RA) and/or Asbestos process, from the receipt of request to delivery of the report. 5. Provide the Performance Characteristic Sheet for the X-Ray Fluorescence machine(s) that will be used during inspections, include a copy of the last leak test(s) that was/were performed on the machine(s) and the last resourcing that was conducted on the machines. Narrative 2: Qualifications (35 points) 1. How many years of experience does your company have in the field of lead and/or asbestos inspection services? 2. Provide each inspection staff members certification title, issuing agency and number, the specific certifications they have and the number of years they have had the certifications. 3. Describe your quality control process to ensure the accuracy of the results/reports. You may use a diagram/flow chart if your process is best described in this manner. 4. Provide three (3) letters of recommendation from owners, private organizations, and/or government organizations for which lead and/or asbestos inspection services have been provided.
4 5. Give at least one (1) sample report as outlined in Part II: RFP Specifications, Section B (Scope of Work), of the RFP of each of the following: a. Single-family project b. Small multifamily project (2-4 units) c. Large multifamily project (5 and above) 6. List any other services that your company can provide (i.e., project monitoring, project clearance, work specification development, etc.). Include a description of each of the additional services. Narrative 3: Work Product (15 points) 1. Provide a sample of your company s LI/RA and/or asbestos report. Please note that an example of both is required if you are applying to provide services in both areas (lead and asbestos). Narrative 4: Cost (15 points) 1. Provide a copy of your company s fee.
5 III. LABORATORY LEAD AND RADON TESTING Narrative 1: Capacity (35 points) 1. Describe past experience working with other municipalities, government agencies and private companies; include the type of work that was requested and completed by your company. 2. Identify the staff positions that will be working with HCIDLA staff. Please identify the lead member overseeing this program and include a job description for each position identified. The description should include the responsibilities as related to this grant. 3. Provide staff resumes. Resumes should include relevant experience, education, qualifications and any state and federal certifications. 4. Describe your sample processing procedure, from the receipt of the sample to the delivery of the sample results. The description should include how the chain of custody of the samples is maintained, how requests with shorter turnaround times are processed and how long it will take to deliver results after the analysis has been completed. 5. Describe your quality control process to ensure the accuracy of the laboratory results. You may use a diagram/flow chart if your process is best described in this manner. 6. Describe your equipment maintenance procedure. Narrative 2: Qualifications (35 points) 1. How many years of experience does your company have in the field of laboratory services? 2. List your company s laboratory accreditations. 3. Provide proof of your company s National Lead Laboratory Accreditation Program (NLLAP) Accreditation Certification. 4. Describe your company s continuing education requirements for the chemists and lab technicians. 5. Provide your laboratory testing schedule (i.e., what days and times different laboratory analyses are scheduled to be conducted/completed/undertaken). 6. Describe the methods used to delivery laboratory results (e.g., by phone, , fax, etc.). 7. List other laboratory services that your company can provide; include a description of each of the additional services.

7 IV. RELOCATION SERVICES Narrative 1: Capacity (35 points) 1. Describe past experience working with other municipalities, government agencies and private companies. Please include the type of relocation services provided and whether it was for single or multifamily housing. 2. Describe the your experience in implementing, applying or interpreting the Uniform Relocation Assistance and Real Property Acquisition Act of 1970 (URA), Section 104(d) of the Housing and Community Development Act of 1974, the City of Los Angeles Municipal Code (inclusive of the Rent Stabilization Ordinance (RSO) and the Tenant Habitability Ordinance, HOME Program Regulations, CDBG Program Regulations, State of California Regulation, and any other federal, state or local regulations or guidance pertaining to relocation assistance. 3. Identify the staff positions that will be working with HCIDLA staff. Please identify the lead staff member overseeing this program and include a job description for each position identified. The description should include the responsibilities as related to this grant. In addition, please provide a summary of the language capacity of the individual(s) who will be working on this contract. Provide staff resumes. Resumes should include relevant experience, qualifications and any state and federal certifications. Narrative 2: Qualifications (35 points) 1. How many individual years of experience do your staff members assigned to the project have in the field of the relocation services? 2. Provide three (3) letters of recommendation from owners, private organizations, and/or government organizations to which relocation services have been provided. 3. Provide one (1)example as outlined in Part II: RFP Specifications, Section B (Scope of Work), of the RFP of the RFP of relocation plans/analysis for: a. Temporary Relocation b. Permanent Relocation

9 V. TECHNICAL TRAINING SERVICES Narrative 1: Capacity (35 points) 1. Describe past working experience with other municipalities, government agencies and private companies. Please include the type of training provided. (i.e., Renovation, Repair and Painting Rule (RRP), Lead Inspector Assessor, Monitor, Supervisor, Lead worker, and other related CDPH courses). 2. Identify the staff positions that will be working with HCIDLA staff. Please identify the lead staff member that will be overseeing this program, include a job description for each position identified. The description should include the responsibilities as related to this grant. 3. Provide staff resumes. Resumes should include relevant experience, education, language capacity, qualifications and any local, state and federal certifications. 4. Give the ratio of students who successfully pass relevant lead or asbestos certification (i.e., EPA certified, Risk Assessor Certification, etc.) and the type of certification. 5. Provide a floor plan and photographs of your training facility. 6. Describe any additional support provided to students (i.e., refresher classes, online portal, etc.). Narrative 2: Qualifications (35 points) 1. How many years of experience does your company have in the field of technical training? 2. List your company s training accreditations. 3. Provide a description of your training courses (i.e., RRP, Lead Inspector Assessor, Monitor, Supervisor, Lead worker, Asbestos trainings and other related CDPH courses). The description should include the number of hours, class size, topics covered, language(s) the courses are offered in, supplies/materials provided and any other pertinent information. 4. Describe your company s policy and requirements for continuing education for your trainers. 5. Describe the methods used to conduct courses (i.e., online, onsite, offsite, etc.).
