2 HANDICOMP SUPPLY ORDER POLICIES 1. PRICING Items listed in this catalog are priced for laser forms. 2. TAX AND SHIPPING CHARGES Handicomp, Inc. is a corporate entity required by law to collect the appropriate sales tax for all shipments within the State of Michigan. Appropriate tax and shipping will be added to your invoice. Residents in the states of Ohio and Florida will also be charged the appropriate state use tax. 3. PAYMENT TERMS Terms are NET 30 days from date of invoice to customers. Handicomp will accept credit card payments. Accounts over 60 days are subject to credit hold, and the account will be put in the Delinquent status. No new shipments will be made until all past due balances are paid in full. 4. CUSTOM ORDERS Custom orders are guaranteed to be fully compatible with the latest version of our software. Custom order quantities are subject to a 10% over/under run as allowed by printing trade customs. Composition charges will be added as necessary. Any cancellations on special orders will be charged at cost according to materials ordered and work completed to date. 5. RUSH ORDERS For a rush order (shipped within 6 or 7 working days from time order is placed) on custom forms such as checks or statements, a flat fee of $45.00 will be charged in addition to one day shipping cost. 6. RETURNS Be sure to check all materials upon receipt. Every effort has been taken to ensure your satisfaction with your order. If you are not satisfied with our merchandise, or have received the wrong item, contact our supply department regarding the problem, and immediate action will be taken to resolve the situation. Returned merchandise may be subject to a 15% restocking fee. Imprinted items, or custom printed items cannot be returned for credit. 7. SHIPPING METHOD Shipments will be sent via UPS Ground, unless otherwise specified. Faster shipments are available at an additional charge. UPS Next-Day, 2nd-Day, 3-Day Select and Federal Express shipments can be arranged when necessary. 8. NON-CUSTOMER REQUESTS The supplies and related prices in this catalog are provided for the convenience of our customers. Others wishing to purchase an item listed in the catalog, to use with a software module other than a specific Handicomp module for which it was intended will be charged the listed price of the item. Non-customers must pay by credit card the day of the order.

5 HANDICAP CARDS PAGE 2 The plastic handicap card can be used either as a bag tag or a wallet card. The weight is comparable to a credit card. The handicap label is applied to the back side of the card.
