The position will work within prescribed professional and ethical standards & evidence based practice, under the guidance of a senior pharmacist. The key accountabilities of the role are: Deliver a base level clinical pharmacy service that may include dispensing (inpatient, outpatient and discharge), requisitions, medication reconciliation, drug information, counselling and the provision of pharmaceutical recommendations. Apply increasingly independent clinical judgement to more complex clinical decision making under clinical practice supervision, or professional guidance of a senior clinician as experience builds. Provide input into the development of clinical practices, procedures and protocols within the work area. Develop clinical practice knowledge and expertise through active learning within the team and by engaging in professional development activities and participating in education programs and competency assessments. Provide direction and support to assist staff in medication management of clients. Actively contribute to maintaining a safe workplace that values the health and safety of co-workers, clients or visitors. Actively contribute to developing and maintaining a culture where staff are vigilant to risks to themselves, their coworkers, clients or visitors (including health and safety, business and operational risks). Actively participate in the Health Service Performance Planning and Appraisal and Individual Development Planning processes. Staffing and budget responsibilities Provide clinical education, clinical practice and operational supervision to work experience students, or those involved in clinical placements. Provide direction and support to Pharmacy s if required. There is not a budget responsibility with this role. 2 Mandatory Qualifications / Professional Registration: Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association in Australia. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties. Pharmacy tertiary degree that is recognised by the Australian Health Practitioner Regulation Agency, as conferring eligibility for registration Key capabilities required for this role: Patient Focus Demonstrates honesty, integrity and respect for all patients, carers and staff. Clinical Knowledge: Demonstrates a level of knowledge, expertise and skill in Pharmacy. Demonstrates the ability to apply established principles to routine Pharmacy services. Continuous Improvement Demonstrates ability, or ability to rapidly acquire the skill, to provide guidance to less experienced staff members and education to students. Provision of information about medications to both patients and their carers, including counselling for patients about their medications. Dispensing of prescriptions for both inpatients at discharge from hospital and outpatients. Manufacturing of medicinal products, creams, mixtures, aseptic and chemotherapeutic medications.
