1 How to Register (1) Read the acknowledgement statement at the beginning of the registration form. (2) The Law Society Mentoring Program offers four different options. Select the program that you would like to join (described in Section 1 of the form). Please refer to the description of each initiative before making your selection. 2 Acknowledgement I would like to apply to be registered in the Law Society s Mentoring Program with the objective of being paired with a member of the Law Society who has volunteered to be a mentor. I understand that placement with a mentor will depend on the availability of volunteers. I confirm that I am an eligible member/student member of the Law Society of Upper Canada or a law school, secondary, or postsecondary student as described in the information I have provided in this registration form. Section 1 Program of Choice Select one by checking the box beside the program of choice. Please refer to the description of each program before making your selection. Equity and Diversity Mentorship Program This program is directed at lawyers, students-at-law, law school students, secondary, and postsecondary students. The Equity and Diversity Mentorship Program matches law school students, students-at-law and lawyers with experienced lawyers. Mentors offer advice and guidance on matters ranging from articling and the licensing process, insights in a particular area of law, to career development. The program also encourages students from diverse backgrounds - including Aboriginal and Francophone students and students from equity-seeking communities - to consider law as a career by connecting high school and university students with mentors who offer academic and career advice, job shadowing, and insights into the legal profession. Disability Mentorship Program This program is directed at lawyers, students-at-law, and law school students with disabilities. As an initiative of the Equity Initiatives Department, the Disability Mentoring Program matches lawyers and students with disabilities with lawyers who may or may not have a disability. Like the Equity and Diversity Mentoring Program, mentors in the Disability Mentoring Program offer advice and guidance on matters ranging from articling and the licensing process, insights in a particular area of law, to career development. In addition, the Disability Mentorship Program addresses the needs and challenges related to an individual s disability in entering and sustaining a career in law. Articling and Placement Mentorship Program This program is directed at students-at-law seeking articling positions. The Articling and Placement Mentorship Program provides students-at-law seeking articling positions with a support link by pairing them with a member of the profession who will provide advice, support and encouragement in their search for an articling position. Practice Mentoring Program This program is directed at practicing lawyers. The Practice Mentorship Program connects lawyers with experienced practitioners in relevant areas of law to help deal with complex substantive legal issues or specific procedural issues outside of the Law Society's advisory mandate. Currently, mentors represent eight areas of law including: Corporate/commercial, Bankruptcy and Insolvency, Environmental, Estates, Family, Immigration, Criminal and Civil litigation.

6 Section 4 Objectives and Expectations The purpose of this section is to identify your learning objectives as a mentee, as well as your expectations of the mentoring program in order to facilitate an effective mentoring relationship between you and your mentor. Please be as specific as possible in answering the following questions: Question: What are your professional objectives or goals? Description: Your goal is your vision of where you would like to be in a certain period of time. For example, if you are a recent call, perhaps your goal is to become a Crown counsel in three years. It is suggested that goals be set as a series of short term accomplishments, which may or may not successively progress towards a broader long-term objective Answer: Question: What are your objectives in a mentoring relationship? Description: Identify what you are seeking to achieve in a mentoring relationship. Describe the key learning objectives or competencies you would like to develop. Your objectives should be specific, measurable, action-oriented, realistic, and time-specific. For example, if you are a law school student, one objective might be to get advice and know-how on effective strategies for finding a summer position in a law firm. You should indicate the time period that is required and the methods or actions necessary to achieve the objective. Answer:
7 Question: What are your expectations from a mentor? Description: Describe what you expect from your mentor, and how your mentor can assist you in achieving your objectives. How should you and your mentor communicate with each other, and how often do you expect to meet? When should the relationship be completed? As you describe what your expectations of your mentor, consider the following: would you like a member that shares on or more of you communities (e.g. race, religion) do you want a mentor of a particular gender? do you want a mentor for specific practice area advice would you prefer to have a mentor with a disability? 8 Confidentiality Any information provided below is strictly voluntary, and is used solely for the purpose of ensuring that mentees who self-identify as being members of certain communities are able to be matched with mentors from similar communities, as the mentees requests. Self-identification will assist in bolstering the Law Society s commitment to promoting equity and diversity in the legal profession, and encourages the participation of members from Aboriginal, Francophone, and equality-seeking communities in its programs, including mentoring.
