1 UDC : EVALUATION OF EFFECTIVNESS OF TREATING CHEMICAL WOUNDS IN CONDITIONS OF MOIST ENVIRONMENT WITH USING CREAMS THAT CONTAIN CERAMIDES Butko Y. O. Wound and inflammatory injuries of the skin are on the top of the list in structure of traumas therefore their treatment and prevention remains an actual problem of modern medicine [5]. Despite a significant amount of medicines the problem of necrotic wounds treatment remains unresolved. It is caused by irrational local therapy which slows down reparative processes and can promote development of cicatricial deformations. [3, 7]. In this regard, one of the perspective approaches to optimization of regeneration processes is to provide their course in the conditions of the moist environment, which prevents excessive dryness of tissues and necrosis aggravation that in its turn prevents development of cicatricial deformations. Intercellular interaction, transport of substances (histamine, serotonin, prostaglandins etc.), growth factor (epidermal, fibroblast), hormones (aldosterone, corticoids etc.) are facilitated in such environment. In the moist environment immunocompetent cells (neutrophils, lymphocytes, monocytes etc.) and derma cells carry out intercellular interaction, excretion of territorial matrix and cytokines easier, also proliferation of kerationcytes and endotheliocytes is accelerated. All in all, in conditions of moist environment water balance of skin remains better, also excessive perspiration is prevented [3, 9, 10, 11]. To provide a course of wound process in moist environment creams which contain ceramides were offered. Lately ceramides received a broad application because they support water balance, fill contents of endogenous ceramides, fill space between cells of epidermis, fasten them, thanks to them flexibility and elasticity of skin increases, all this promotes skin regeneration, restoration of a hydro-lipidic cloak and barrier properties of skin [9, 11, 12]. In regard to this, creams containing a combination of ceramides and dexpanthenol which is widely used for local treatment of wounds were developed.
2 The objective of this study was to assess the efficiency of treatment with creams containing ceramides in model of chemical wounds with expressive alterative processes of skin and hypodermic cellulose in rats. Materials and methods. The experiment was lead using 36 rats weighing g that were divided in groups: 1 st group intact animals (IA), 2 nd peak of pathology (PP) (ulcer formation), 3 rd control pathology (CP), 4 th treated with cream Ceramides, 5 th treated with cream Dexpanthenol with ceramides, 6 th treated with cream Bepanthen (preparation of comparison produced by the company Bayer, Germany). The technology of creams with ceramides was developed in State Expert Centre for Drugs and Medical Products under the leadership of professor N.O. Lyapunov. Aseptic acetic ulcers of skin were reproduced according to the technique [2]. Treatment by preparations was carried out once a day till the complete healing. The efficiency of the treatment was estimated with the help of planimetric and biochemical rates of animal s blood. Planimetric rates were ulcer area (S) and healing area (mm 2 ), healing speed (V, mm/day) and healing coefficient (k) [2]. Biochemical rates in blood serum were defined: general protein (GP) and creatinin, which characterize intensity of destructive changes in tissues using sets Filisit-Diagnostic. Also the level of lipid peroxidation (LP) (by the concentration of TBA-products, technique of I.D. Stalnoy) and state of the antioxidant system (AOS) by the level of the reduced glutathione (RG) were defined [4, 6]. All the interventions and euthanasia of animals were carried out according to standards of the treatment of laboratory animals (Strasburg, 1986). Results of the experiment were processed using program Statistica 8 at probability level р< 0.5 [8]. Results and discussion. Simultaneous introduction of phlogogen and acetic acid led to formation of hypodermic ulcers of skin which break occurred on the 7th day of the experiment and that was the day when the treatment began. The initial area of ulcers was mm 2. Results of the experiment showed that the healing speed on the 5 th and 15 th day in the group CP was 2.70 and mm/day; in the group treated with cream Ceramides 6.63 and mm/day, with cream Dexpanthenol with ceramides and mm/day, with cream Bepanthen
3 10.97 and mm/day respectively (tabl. 1). According to the healing speed coefficient there was reduction of the wound area on the 5 th, 9 th and 15 th day of the experiment for group CP by 5%; 18% and 29%; for the group treated with cream Ceramides by 13%; 48% and 97%; with cream Dexpanthenol with ceramides by 22%; 40% and 100% (that was complete healing), with cream Bepanthen by 17%; 20% and 80% respectively. Thus, complete healing in group treated with cream Dexpanthenol with ceramides was observed on the 15 th day of treatment, in other groups that received treatment on the 17 th day. To assess systematical changes in the process of reproduction of pathology and treatment the biochemical rates were studied (fig. 1.) According to the literature, decrease in level of GP indicates intensive inflammation in organism. It was supported by our experiment. After modeling of pathology the level of GP significantly decreased by 1.4 times compared to intact. After treatment with cream Ceramides normalization in level of GP was observed, however it didn t gain intact values. In groups treated with creams Dexpanthenol with ceramides and Bepanthen the level of GP significantly increased by 1.3 times compared to the peak of pathology and approached to intact values that indicates depression of inflammation. Significant increase of creatinine level in rats serum in the peak of pathology by 2 times indicates the intensity of destructive changes during the inflammation. On the 15 th day of the experiment in the group CP this rate remained almost the same that indicates preservation of necrotic processes. Decrease of creatinine level was observed after the treatment of animals that indicates termination of the necrotic phase of the wound process. At the end of the experiment creatinine level decreased as much as possible in animals treated with creams Dexpanthenol with ceramides and Bepanthen - by 1.3 times compared to the peak of pathology and was significantly lower than in the group of animals that were not treated. LP processes characterize intensity of destruction of cytoplasmic membranes of cells and prevalence of catabolic processes. Level of LP in the peak of pathology increased twice. In the groups that were treated level of TBA-P decreased by 1.2 times
4 and was significantly lower than in the group CP, however it didn t gain intact values. The highest membrane stabilizing activity was shown by cream Dexpanthenol with ceramides as in this group level of the TBA-P was the lowest. Increase of LP level is accompanied by compensatory increase of RG level that indicates exhaustion of the system of antioxidant protection AOS [8]. The level of the RG decreased by 1.5 times after the disclosure of ulcers. In all groups after treatment the normalization of RG level was observed. In animals that were treated with cream Dexpanthenol with ceramides RG level values had a significant prevalence by 1.3 times to values of the group CP and gained intact values. Thus, treatment with creams Dexpanthenol with ceramides and Bepanthen promotes normalization of biochemical rates, decrease of LP level and reduction of AOS system in wound pathology in animals. Conclusions: 1. Reparative activity of creams Ceramides, Dexpanthenol with ceramides and Bepanthen in acetic wound model in rats was studied. 2. Cream Dexpanthenol with ceramides promotes decrease of healing terms and normalization of biochemical rates, processes of LP and state of AOS in animals with pathology. In the group treated with experimental cream Dexpanthenol with ceramides healing occurred two days faster than in groups treated with creams Ceramides and Bepanthen. 3. Thus, creams that contain ceramides are perspective preparations for acceleration of healing of wounds and for purpose of their use in pharmacocorrection of wounds.
