2 Message from the Chair Welcome to the summary version of our annual report. We hope this booklet is useful in providing you with an overview of what we have achieved during 2015/16. This is our third year in operation as a CCG. It has been an exciting and challenging time as we continue to deliver improvements to health services for 94,000 people living locally. We are passionate about involving patients, carers and the wider community in our work. By understanding their experiences and listening to their feedback, we are able to drive forward better and more integrated services. You are probably aware that health and social care are facing significant challenges, both locally and nationally. Pressure on services is building and costs are rising faster than our financial allocation. Transformational change is needed. The Five Year Forward View sets out a new direction of travel for the NHS of better health, better patient care and improved NHS efficiency. We look forward to working with our partners, patients and local communities as we continue to move towards our mission of improving health outcomes and reducing health inequalities for our patient population. 3 Who we are and what we do GP Practices in Nottingham West About us NHS Nottingham West Clinical Commissioning Group (CCG) is responsible for planning, funding and monitoring local health services for our population. We are made up of 12 GP practices across the area. Our practices have worked together since 2007, bringing high quality care closer to home through strong clinical relationships and partnership working. We work with wider partners in the NHS (community and mental health providers, hospitals and pharmacists), councils, patients and community groups to make the best use of our money to meet patient needs. Our values Our values are at the heart of our decisionmaking. 4 Spotlight on patient involvement April June September December Our summer events season kicked off in June. We spoke with hundreds of people at Hemlock Happening and Broxtowe Community Celebration Event. We also celebrated national carers week and took part in a stand at the Carers Federation. More than 1,000 people shared their views during our formal public consultation on gluten free foods on prescription. In September we held two drop-in events at Beeston and Stapleford libraries for patients to share their views. The consultation sparked a national and local media debate and was covered by the Independent, Daily Mail, Mirror and East Midlands Today. A development session was held for our Patient Reference Group (PRG) in December. The PRG is made up of patient representatives from all 12 GP practices and it meets on a monthly basis. The group agreed their work plan for 2016/17 which included IT and data sharing and adult mental health as areas of focus. July October We heard stories from patients from the Breathe Easy Nottingham West support group which offers support, advice and rehabilitation to people affected by lung conditions. Thanks to support from the CCG, the group has been selected to be part of a major national study called NESTA. The study is looking at the impact of self-help groups in helping people to become more independent with their lung condition. May National dementia awareness week took place in May. During the month we visited Beeston Memory Cafe and saw what a difference the cafe is making to people living with dementia and their carers. They offer support, advice, guest speakers and activities such as quizzes and singing. It was great to see the impact of the CCG s Lifestyle Funding in our community. Patient representatives and partners also helped us to launch a leaflet of local support for dementia patients. 6 January We met expectant mothers and new mums with their babies at an event at Eastwood Children s Centre. They spoke to us about isolation, breastfeeding, antenatal classes and the importance of children s centres. A consultation was launched by Linden Medical Group on the proposed closure of their branch surgery in Wollaton. More than 830 responses were gathered during the eight week consultation through surveys, public meetings and telephone conversations. The practice worked closely with the CCG and their PPG during the consultation period. November February Thanks to help from the Patient Reference Group, more than 3,200 views were captured during the annual Nottingham West patient survey on access to primary care. This year s highlights included patients being seen quicker when waiting for their appointment in the practice and the helpfulness of receptionists. Nottingham Pride is always a colourful and vibrant occasion. We surveyed more than 100 people from Lesbian, Gay, Bisexual and Transgender (LGBT) backgrounds about their views on access to GP practices. A key aim of our equality and diversity work is to ensure all services are accessible, appropriate and sensitive to the needs of individuals. March August Patient representatives and service users took part in the procurement process for the 177.5m community services contract. The contract focused on a new model of integrated care in line with the NHS Five Year Forward View and went live in April NHS Nottingham West Clinical Commissioning Group More than 90 people attended a Cancer Services Awareness Event which was held by Nottingham University Hospitals in collaboration with the African Caribbean National Artistic Centre. The drive for this event was to bring cancer professionals into the community to engage directly with community members. Black and Minority Ethnic (BME) cancer inequalities remain a significant area of concern for NHS Cancer services and local providers. We spoke with soldiers and their families at the Chetwynd Barracks Health Fayre in March. We were joined by Healthwatch Nottinghamshire, mental health support charities and drug and alcohol services. 7

5 Achievements in 2015/16 GP Practices Mental Health Domestic Violence Dementia Key highlights from the 2016 national GP survey include: Spending on mental health services in 2015/16 has increased in real terms. A new national requirement is the introduction of new access standards for mental health, as part of the ambition to achieve parity of esteem between mental and physical health by The IRIS (Identification and Referral to Improve Safety) Domestic Violence Advocate Educator role is supported by a local GP and has provided training to clinical and non-clinical staff at all GP practices. This heightened awareness has led to an increase in identifying, providing one on one support with the named advocate educator and signposting for patients. We have the highest proportion of people over 65 with a formal diagnosis (5.62%) in the country. Also, 86.56% of our new patients with dementia have had a blood test recorded, which is also the highest in country. 2nd in country for helpfulness of our receptionists 2nd in country for convenience of appointments In the vast majority of other results, Nottingham West is consistently ranked in the top 10% - 20% nationally. In 2015/16 we have: n Increased spend in line with the CCG s funding allocation increase of 5.7%. Older People Local Care Teams Within primary care, we have funded the Engaged Practice Scheme, which aims to harmonise and improve access to GP Practices. The requirements of the scheme include: n Invested in 111 mental health project, Personality Disorder Services and Children & Adolescent Mental Health Services (CAMHS) Eating Disorder Service. All 12 practices open Monday to Friday 8am to 6.30pm n 24/7 crisis care for adults established via service transformational change. Practices required to offer urgent appointments on the day and routine appointments within 3 days n Joint working with Nottinghamshire Police to eradicate detentions in police cells for children and adults detained under section 136. Our Local Care Teams work to avoid hospital admissions for vulnerable patients with complex physical or mental health needs who are aged 75 and over, or in the top 2% risk register for admissions. The team includes health and social care workers which includes Clinical Care Coordinators, Administrators, Therapists and Social Workers. Overall capacity has increased by 600 additional face to face and telephone appointments per week. n Delivery and continued improvement of take-up of Primary Care Psychological Therapies. Care Homes Carers and Family Support The CCG has funded a Carer Support Worker post to increase identification and support for carers through engaging with member practices. We continue to work alongside our colleagues in Nottinghamshire and Bassetlaw CCGs in partnership with Nottinghamshire County Council to commission services to identify, support and signpost carers. The Compass Worker provides practical and emotional support to carers of people with dementia. It was commissioned by Nottinghamshire County Council to work within Nottingham West and is working well and proving popular. We continue to promote awareness campaigns to support timely diagnosis, living well with dementia and dementia friendly communities. We remain a member of the Dementia Action Alliance. An information leaflet specifically for carers of people with dementia has been developed and being used locally. Projects run by local and community groups such as dementia cafés and reminiscence groups have been supported using the Lifestyle Fund and continue to grow and develop across Nottingham West. We are pleased the Residents Representative Service provided by Age UK Notts has begun within Nottingham West. The service offers independent support to residents living in an older person s care home, their relatives and carers. The Health Partnerships Care Homes Team has identified tissue viability champions in care homes. The React to Red pressure ulcer training has commenced. The team are using data to target and help inform care homes and the Care Home forum has been re-launched. We continue to fund a seven week information programme for carers of people with dementia through Radford Care Group. This programme aims to help improve understanding of dementia and the support available for carers. 8 NHS Nottingham West Clinical Commissioning Group 9

6 Our finances We have been allocated 130m this year to plan, buy and monitor healthcare services. We are pleased to report that we have met all of our financial duties for the year, including delivery of the planned surplus target of 1.3million. However, 2015/16 has been a tough year financially with a savings target (QIPP) of 2.3m. How we spend our money Local Authority BCF, 2.9m, 2% Running Costs, 1.8m, 2% GP Primary Care, 11.5m, 9% Planned surplus, 1.3m, 1% This year also saw us take over delegated budgetary responsibility for GP primary care contracts and the Better Care Fund (for health and social care integration). Both these areas presented a degree of uncertainty and financial risk to the CCG in an already challenging financial environment.
