Also, please sign the Waiver of Liability form for CABC, the Consent to Treat form for CABC, the Publicity Authorization and the Guidelines and Agreement Form which follow. Please turn in this completed application along with your deposit to the church office (or the Welcome Center on Sunday mornings) by July 1 or earlier. Space is limited at the Ensenada Outreach Center so reservations will be accepted on a first-come, first served basis. 1. Have you been on a CABC mission trip to Mexico in the past five years? yes no 2. Have you traveled internationally before? yes no 3. Do you have a valid U.S. Passport? yes no. If no, do you know how to apply for one? yes no 4. Do you (or other members of your family who will be going) have experience in: a. construction? b. children s ministry? c. speaking Spanish? d. driving in Mexico? (Are you willing to take your car or truck? ) Financial information: The registration fee for this trip is $375 per person (adults 12 and up). Children are $225 per person. Families of four or more will receive a 10 percent discount on these rates. The registration fee covers your transportation costs, housing, food and program fees at Ensenada Outreach Center in Mexico. The registration fee does not cover the cost of the house building project or personal expenses you may have in Mexico. 1. Are you able to pay a deposit of $100 per person by July 1? yes no. 2. Will you be able to pay the balance by August 1? yes no 3. Are you also able (or willing) to contribute additional funds to help with construction and VBS materials? yes no. Note: If you have friends or family members who would like to support you and your family s participation on this mission trip, we can provide you with a sample support letter that you can send to them with instructions on how to do that. 4 WAIVER OF LIABILITY (CABC) I am fully aware that there may be risks and hazards connected with MexiGO! 2016, sponsored by College Avenue Baptist Church. I hereby elect on behalf of myself to voluntarily participate in said activity, knowing that the activity may be hazardous to participant and/or participant s property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by participant, or any loss or damage to property owned by participant, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF College Avenue Baptist Church or otherwise. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS College Avenue Baptist Church from any loss, liability, damage or costs, including court cost and attorney s fees that they may incur due to participation in said activity, WHETHER CAUSED BY NEGLIGENCE of College Avenue Baptist Church or otherwise. CONSENT TO TREAT I consent to any medical or dental diagnosis or treatment deemed necessary for myself and/or my minor child (children) listed below. Further, I understand that all efforts will be made to contact me prior to treatment. In the event I cannot be reached in an emergency, I give permission to the activity leader to make the decisions necessary for treatment to my child (children). Should there by no activity leader available, I give permission to the attending physician to treat my minor child. I further understand that the doctors, dentist, & other providers attending to my child will take all reasonable safety precautions during their care. Further, as parent or legal guardian, I am responsible for the health care decisions for my minor child and agree that my insurance plan is the primary plan for the dental, medical or hospital care or treatment that is given to my child. Any policy of the church or organization sponsoring this event will be used as the secondary coverage. PUBLICITY AUTHORIZATION Any pictures or video taken of participants listed below in this activity may be used in publicity, including articles in newsletters, magazines, pictures on the College Ave. Baptist Church website, or presentations during Worship Services, Community Classes, Ministries and event promotions pertaining to College Ave. Baptist Church.

5 GUIDELINES AND AGREEMENT MexiGO! 2016 We re excited that you are going with us to Mexico this summer! By filling out these registration forms, you agree with and commit to the following guidelines: 1. I will get all my application forms and money turned in on time. 2. I will help raise the money (or contribute myself) to pay for the cost of the house we are building and other financial needs of the trip. 3. If I have to cancel after August 1st, I will contribute the money I have paid or raised so far to the fund-raising efforts for this trip. 4. I will make sure I obtain a passport or passport card for myself and my family members, at my own expense. 5. If I take my own vehicle into Mexico, I will do so at my own risk and my own expense and will make sure I obtain Mexican liability insurance before I enter Mexico. 6. If possible, I will contribute my expertise and resources that I have to help make this trip successful. That may include such things as construction knowledge, tools, ability to serve as an interpreter, teach children, play a musical instrument, etc. 7. I will be willing to submit to the authority of YUGO Ministries and College Avenue Baptist Church leadership while serving in Mexico. 8. While serving in Mexico, I am willing to honor and respect the cultural and national customs and values of the Mexican people. 9. While serving in Mexico, I agree to refrain from using alcohol or tobacco in any form or engaging in other behaviors which might be offensive to others or inappropriate. I will not bring any weapons, firearms or illegal drugs into Mexico. 10. I will be a team player and attend team meetings and all scheduled activities unless illness or other emergencies occur. 11. I will have a flexible attitude because I realize that not everything will go as planned or go my way. I will not complain because things aren t going exactly the way I think they should be going. I acknowledge that this trip is not for the primary benefit of me or my family members, but for the benefit of Christ and those whom we are going to serve. 12. I will pray diligently for this trip, for those who are going with me, and for the families in Mexico whom we will serve. I will do all that I can to help make this trip a positive experience for everyone involved.
