1 Welcome to the Village School Performing Arts Private Lesson Policies CONTACT: Debbie Martin, Director of Performing Arts Phone: (239) x208 REGISTRATION: Fill out the attached form to hold a spot for private lesson with a $40 NON- REFUNDABLE payment. Checks should be made payable to The Village School Performing Arts. LESSON TIMES: All lesson schedules will follow the Village School academic year. Lessons are during or outside school hours as scheduled with the instructor. STUDENTS RESPONSIBILITY: Regular attendance and practice is expected of all students. If a student forgets his or her books and/or instruments needed for a lesson, that lesson will be forfeited. Each student is allotted one absence per term that will be made up by the teacher. Any missed or cancelled lessons, after the first absence, will count as a completed lesson and will NOT be made up by the teacher. If the teacher cancels a lesson at any time, he or she must make up the lesson. 24 hours is needed for a lesson cancellation, or there will be a charge. STUDENT RECITALS: Students have the opportunity to participate in recitals and programs that occur throughout the school year. Selected advanced students will participate in the Honor s Recital in the spring. MUSIC LESSON SCHEDULES: Music lessons begin the week of September 6 th, 2016 through December 9 th, Second semester lessons will begin the week of January 9 th, 2017 through May 12 th, Make- up lessons will be the week of December 12 th, 2016 for the fall semester and after May 12 th, 2017 for the spring semester. There are no lessons given Thanksgiving week, November th, 2016 or Spring Break March th, 2017.

4 Withdrawal without penalty may only occur between the Fall and Spring semesters. Should discontinuance be elected by the student for any reason, the Director must be notified in writing by December 9, Students ceasing private music lessons prior to the end of the Fall or Spring semesters are still responsible for that semester s full tuition. FAILURE TO MAKE TIMELY PAYMENTS will lead to that student s termination in the Performing Arts Program. A $40 NON REFUNDABLE REGISTRATION FEE MUST ACCOMPANY THIS FORM. I ACKNOWLEDGE AND AGREE TO THE PROGRAM REQUIREMENTS / POLICIES AND WILL BE RESPONSIBLE FOR ALL FEES. Responsible Party Signature and Date: Date: Private Lesson Payment Information Lessons are typically 30-minute sessions once a week, meeting on the campus of The Village School, 6000 Goodlette Road North. Payment Plans: Village School Student Or NNUMC member Unaffiliated Student Group A Group B Semester: Two payments for each semester due Sept. 1, 2016 & Jan. 1, 2017 A1 = 2 payments of $600 OR B1 = 2 payments of $650 Monthly: Nine monthly Payments due on the 1 st of each month Sept. 1 st May 1 st A1 = 9 payments of $140 B2 = 9 payments of $150

5 Music Payment Schedule *Any payments more than 10 days late WILL BE CHARGED an additional $30 fee. **All returned checks will be assessed a $35 fee without exception. ALL CHECKS SHOULD BE MADE PAYABLE TO: The Village School Performing Arts with the student s name on the memo line. Please submit to the school office to the attention of Debbie Martin.
