2 What do you get with BCBSTX? Blue Cross and Blue Shield of Texas (BCBSTX) has been bringing health care services to Texans for more than 80 years. BCBSTX serves STAR and CHIP members in Bastrop, Burnet, Caldwell, Fayette, Hays, Lee, Travis, and Williamson counties. With BCBSTX STAR or CHIP, you get great health care from our hospitals, doctors and pharmacies PLUS we offer helpful Value-Added Services at no cost to you. With BCBSTX STAR or CHIP, you get all the health care you know today plus we offer all of these Value-Added Services: Free Rides: Free rides to non-emergency doctor visits, health classes, therapy and BCBSTX member meetings. 24 Hour Nurse Advice Line: Talk to our nurses at any time, 24 hours a day to get help with non-emergency medical questions. Multilingual Glucometers for STAR Members: Diabetic STAR members can get a glucometer that gives test results in four languages. Sports and Camp Physicals: Free sports and camp physicals for children 18 and under. Child Safety Helmets: Free safety helmets for kids ages 3 to 18 for biking, skateboarding or other activities. Children s Safety Booster Seats: Car booster seats for kids ages 2 to 12 and who weigh between 30 to 100 pounds. Well-Child Incentive Gift Cards: BCBSTX offers gift cards for taking your child to his or her Texas Health Steps or yearly checkups.

3 Upgraded Eye Glass Frames: Children 18 and under can get up to $175 each year to buy stylish eye glass frames. Pregnancy, Childbirth and Newborn Care Classes: Help knowing what to expect during pregnancy, getting ready for labor and delivery, and taking care of your new baby. For pregnant mothers or newly delivered mothers who enroll and attend approved prenatal education programs, we offer a free diaper bag that includes useful baby items. Prenatal and Postpartum Gift Cards: BCBSTX offers STAR members who are pregnant or those who have just delivered their baby, gift cards for prenatal and postpartum visits. Free Baby Safety Car Seat: Pregnant women can get a free baby car seat by getting prenatal care before the baby is born. Breastfeeding Support: We offer breastfeeding coaching at home after the baby is born and a free hands-free pumping bra. In-Home Wellness Visit (HWV) for Moms and Newborns: Home visit by our In- Home Wellness Nurse after the baby is born and delivery of a breastfeeding support kit. CHIP Perinate moms will also receive a breast pump. Dental Care for Pregnant Women (STAR members 21 and over, CHIP Perinate members 19 and over): Checkups, cleanings, fillings and more. Vouchers for the Sustainable Food Center (SFC) Farmers Market Locations: Vouchers to get free fresh fruits and vegetables two times during pregnancy. FOR ALL MEMBERS Free Rides Using BCBSTX Transportation: Schedule free rides to non-emergency doctor visits, therapy, approved health classes and special BCBSTX member meetings. We can also send a wheelchair van if you need one. All members will need to call to schedule rides at least three days before scheduled appointments, and 24 hours before a sick child visit. STAR Members: STAR members can still get non-emergency medical transportation through the Medical Transportation Program (MTP) by calling MTP at If MTP can t help you with getting a ride, you can call BCBSTX Customer Service to schedule a ride with our transportation vendor, MTM. We ask that you schedule your ride at least three days before your scheduled appointment, or 24 hours before a sick visit. Limitations: BCBSTX transportation is for STAR members when MTP is not available. BCBSTX transportation requires one to three days minimum advance scheduling and prior authorization.

4 CHIP Perinate Members: BCBSTX can help CHIP and CHIP Perinate members find a ride for day to day approved visits. Call BCBSTX Customer Service at then choose the transportation option. If you have hearing or speech loss you can call the TTY line at Limitations: BCBSTX transportation for approved rides in the service area and less than 75 miles applies to CHIP and CHIP Perinate members and requires authorization. BCBSTX transportation more than 75 miles including lodging and food allowances are limited to CHIP members. BCBSTX transportation requires three days advance scheduling. BCBSTX transportation may be approved with less notice on a case by case basis. Lodging and food allowances are limited to CHIP members and one parent, guardian or authorized caregiver only. They do not apply to CHIP perinate members. All lodging and meal expenses for CHIP members will require three days prior approval. Approval for BCBSTX transportation overnight lodging and transport may be approved in less than three days on a case by case basis. They will only be approved for prior approved medical appointments more than 75 miles from the member s home. There is a daily limit of $120 per night for lodging and $50 per day for meals with a total maximum amount of $1,000 per 12 month period. 24 Hour Nurse Advice Line: Talk to our nurses at any time, 24 hours a day/seven days a week to get help with non-emergency medical questions. Our nurses can help if you have questions or concerns about any health topic. They can help you choose the right place for care if you are not sure what to do. We have Spanish-speaking nurses and translators for other languages. There is also an Audio-Health Library with recorded messages on hundreds of health topics. You can talk to our nurses on the 24 Hour Nurse Advice Line at or TTY There are no limitations. Members may access the 24 Hour Nurse Advice Line at any time. FOR STAR MEMBERS WITH DIABETES Free Multilingual Glucometers: STAR members with diabetes who need help monitoring their blood sugar can get a multilingual glucometer that tells the test results in English, Spanish, French and Arabic. To get the monitor, the member must be willing to join the BCBSTX Diabetes Condition Care program or Case Management for diabetes. Diabetic members who are already in these programs can also get the free multilingual glucometer. You must be a STAR member. Call Member Outreach at or TTY Limitations: The Multilingual Glucometer speaks English, Spanish, Arabic and French. Refills of testing strips may be obtained through Longhorn Health Solutions or other durable medical equipment (DME) providers and is a covered benefit program under STAR. BCBSTX Medicaid diabetic members who are willing to enroll in the BCBSTX Condition Care program must request the multilingual glucometer and have a linguistic need.

5 JUST FOR KIDS Free Children s Safety Booster Seat: STAR and CHIP children between the ages of 2 to 12 who have outgrown their baby car seat, and weigh 30 to 100 pounds, can get a safety booster seat. Get the booster seat by doing these things: Take your child to the doctor for a Well Child checkup or Texas Health Steps checkup within 90 days of signing up with BCBSTX, or Take your child to the doctor for a yearly Well Child checkup or Texas Health Steps checkup You can get a booster seat form from our website at or call Member Outreach at Fill out the form with your doctor and have the doctor sign it and add the office stamp. Fax the form to The booster seat will be sent to the address you put on the form. Limitations: The booster seat is for kids 2 to 12 who have outgrown their infant car seat and meet the height and weight requirements for the booster seat. The booster seat is for kids 30 pounds to 100 pounds and a maximum height of 57 inches. Parents or guardians are responsible for ensuring their child meets the height/weight criteria that must be met to safely use the booster seat. Child Safety Helmet: Help your children stay safe while riding bikes, skate boarding or doing other outdoor activities. Kids ages 3 to 18 can get a free safety helmet every two years. Get the safety helmet by doing these things: Take your child to the doctor for a Well Child checkup or Texas Health Steps checkup within 90 days of signing up with BCBSTX, or Take your child to the doctor for a yearly Well Child checkup or Texas Health Steps checkup. Go to our website at to get the safety helmet form or call Member Outreach at Fill out the form with your doctor and have the doctor sign it and add the office stamp. Fax the form to The helmet will be sent to your address on the form. Limitations: Limitations to this program include limiting members eligibility for receiving new safety helmets to every other year. Members will be required to complete the appropriate paperwork and submit it to demonstrate that they have met the criteria in order to receive a helmet.

6 Well-Child Incentive Gift Cards: BCBSTX offers gift cards for taking your child to his or her Texas Health Steps or yearly checkups. This is how you get the $50 gift card: Take your baby to the doctor at least six times from birth to 15 months old. Take children ages 3 to 6 to Texas Health Steps or annual checkup by the end of the calendar year. Teens, tweens and young adults, ages 12 to 20 for STAR and 12 to 19 for CHIP, should also see their PCP or OB/GYN once a year. Limitations: Parents or guardians of child members must ensure their child member completes Well Child checks by the defined criteria. Members or a parent or guardian of underage members have to complete the documentation and send it in to request the gift card. Members must be active on the Plan to receive the gift card. Sport and Camp Physicals: We help STAR and CHIP kids take part in sports and fitness activities by giving free physicals to kids 18 and under who are going to camp or playing sports. Physicals can be done once a year. Just schedule an appointment with your child s primary care provider (PCP) to get the physical. No authorization is required. Limitations: STAR and CHIP members must see an in-network STAR and CHIP provider. Sports and Camp Physicals are limited to STAR and CHIP kids ages 18 and under. Limitations: Sports and Camp Physicals are limited to STAR and CHIP members and physicals must be done by a STAR or CHIP provider. Upgraded Eye Glass Frames: We offer stylish frames to STAR and CHIP kids. Children 18 and under can get up to $175 each year to buy stylish frames. Your child must get an eye checkup from a Davis Vision eye doctor. You do not need an OK from your child s PCP. For more information, call Customer Service at or TTY Limitations: For STAR members, STAR kids ages 18 and under will be limited to one pair of stylish frames every year after completion of an eye checkup. The maximum value of the frames will not exceed $175. Limitations: For CHIP members, CHIP kids ages 18 and under will be limited to one pair of stylish frames every year after completion of an eye checkup. The maximum value of the frames will not exceed $175. Enhanced frames are restricted to CHIP members. The enhanced frames do not apply to CHIP Perinate.

7 FOR PREGNANT WOMEN Pregnancy Incentive Gift Cards for STAR members: BCBSTX offers gift cards to STAR members who are pregnant or who have just delivered their baby, for going to prenatal and postpartum visits. You can get a copy of the forms online at or call Member Outreach at We will send your gift card to the address you give us on the form. $25 Prenatal Visit Reward for STAR members: If you are pregnant and you are on our STAR plan, you can get a $25 gift card for going to your first prenatal visit within the first three months of your pregnancy or within 42 days of joining our plan. To get your $25 reward, you will need to have your doctor fill out a BCBSTX Prenatal Visit Postcard and sign it. Limitations: Members must be active on the plan to receive the gift card. $50 Post-Partum Visit Reward for STAR members: If you see your doctor for your post-partum visit between 21 and 56 days after you deliver your baby, you will get a $50 gift card reward. Have your doctor fill out the BCBSTX Post-Partum Visit Postcard and sign it. Limitations: Members must be active on the plan to receive the gift card.

8 Free Infant Car Safety Seat: Pregnant women can get a free infant car seat by doing a combination of these things: Visit your doctor in the first three months you are pregnant, or in the first month after signing up with BCBSTX. Go to the number of doctor visits you need based on how far along you are in your pregnancy at the time you join BCBSTX. Join our free Special Beginnings program. Find the Car Seat Form on our website at or call Customer Service at Take the form to your doctor to start earning points. Once the form is completed it can be faxed to Member Outreach at The car seat will be sent to the address on the completed form. Limitations: Members must be pregnant and complete a combination of activities specified in the description in order to receive a free infant car safety seat. Pregnancy, Childbirth and Newborn Care Classes: Pregnant members can learn up-to-date information about pregnancy and caring for a new baby in these free classes. You must be pregnant to join. Here are some of the things you will learn: Pregnancy Labor and Birth Baby Care and Breastfeeding How your body changes Your birth plan New baby care and safety How baby grows and changes Taking care of yourself Aches and pains of pregnancy Birthing choices Breathing and pushing skills C-section birth Umbilical cord and circumcision care Choosing your baby s doctor How to know if your baby is sick Your checkups and tests Pain relief choices Vaccines your baby needs Knowing signs of early labor Recovery and postpartum care Breastfeeding Feeding positions Expressing and storing milk Going back to work Classes are offered at various locations. If you need a ride, BCBSTX will help you with the free rides BCBSTX transportation described in the Free Rides Section. Call Member Outreach at to find all the locations, days and times of our prenatal classes. You can also learn which classes will be offered in Spanish.

9 Free Diaper Bag Filled with Baby Items: If you are pregnant and you go to a BCBSTX approved prenatal education class; you can earn a diaper bag full of helpful stuff for you and your new baby. After you take your prenatal education class, we will send you an invitation to come to a prenatal class graduation celebration and baby shower. You will get your diaper bag full of gifts when you come to the celebration. You must still have BCBSTX as your plan to be able to come to the shower celebration. Limitations: Members must be on the plan to be eligible, Members also need to have attended one BCBSTX approved prenatal education class to be eligible to request the gift. In-Home Wellness Visits: For STAR and CHIP members, BCBSTX offers a home visit for new moms and babies after the baby is born. Our In-Home Wellness Nurse can visit three to five days after birth or when you ask. The nurse will make sure you and your baby are doing well and will answer any questions you may have about your new baby and how to keep your baby safe. Call Member Outreach at and our staff will help set up your free visit. You do not need a doctor s referral. If you are breastfeeding, the nurse can help and coach you. See below for more about breastfeeding support. Our in-home wellness visiting nurse also delivers many of the other Value-Added Service items such as the breast feeding support kit, the pumping bra and breast pump for CHIP Perinate moms. Limitations: Members must have delivered a live baby while on the plan. Breastfeeding Coaching: Breastfeeding is the best start you can give your baby. You can get breastfeeding coaching from the In-Home Wellness Nurse, after the baby is born (See In-Home Wellness Visit above). Limitations: Enrolled pregnant or newly delivered members who are interested in breastfeeding may receive breastfeeding coaching. The member must have delivered while on the plan and must be willing to have an in-home wellness visit.

10 Hands-Free Breast-Pumping Bra: BCBSTX is offering a hands-free pumping bra to help you better manage your life while you are pumping. This bra allows you to pump your milk and leaves your hands free to do other things at the same time. The hands-free pumping bra will be delivered to you by your in-home wellness nurse after you have your baby. During the visit, the nurse can help you with breastfeeding support and teach you how to use the hands-free pumping bra. Call Member Outreach at to find out how to get your free in-home wellness visit and hands-free pumping bra. Limitations: Members must be on the plan to be eligible. Members must have also delivered their new baby on the plan and be willing to meet with the in-home wellness nurse to have the bra delivered. Breast Feeding Support Kit: Pregnant members who are breast feeding will be eligible for a breast feeding support kit gift. This gift will be delivered by the home wellness visiting nurse. The kit includes breast milk disposable bags, soothing breast pads, breast soothing cream, a breast milk storage tracking refrigerator magnet and nipple protectors and case are provided to members who can benefit from use of the protectors after working with the Home Wellness Visit nurse. Limitations: For CHIP members, members must have delivered while on our plan and notify BCBSTX upon delivery. Members have up to 30 days post-delivery to contact the plan to request support kit. Breast feeding members must be willing to have the home wellness visit to receive the breast feeding support kit which will be delivered during the visit. Breast nipple protectors are handed out to members by the HWV nurse as needed. Limitations: For STAR members, members must have delivered while on our plan and notify BCBSTX upon delivery. Members have up to 30 days post-delivery to contact the plan to request support kit. Breast feeding members must be willing to have the home wellness visit to receive the breast feeding support kit which will be delivered during the visit. Breast nipple protectors are handed out to members by the HWV nurse as needed. Dental Services for Adult Pregnant Members: BCBSTX offers dental services for pregnant STAR members age 21 or older, and CHIP Perinate members age 19 or older. These dental services include a mouth checkup, X-rays, cleanings, fillings, sealants, scaling and planning, and extractions (pulling a tooth) based on what you need. You must go to a Liberty Dental provider in the network. Call BCBSTX Customer Service toll-free at to find a dentist near you. Limitations: For STAR members, BCBSTX will provide dental care for pregnant women not covered by the STAR Dental Program. Adult Pregnant women over age 21 enrolled in STAR. Members must see a participating dental provider in the network; benefits for treatment services per scheduled treatment up to a maximum of $500. Limitations: For CHIP members, BCBSTX will provide dental care for pregnant women not covered by the CHIP Dental Program. Adult Pregnant women defined as CHIP Perinate women greater than age 19. Members must see a participating dental provider in the network; benefits for treatment services per scheduled treatment up to a maximum of $500. Dental Services are limited to CHIP Perinate women older than 19 years of age, they do not apply to CHIP members.

11 Sustainable Food Center (SFC) Farmers Markets: Pregnant members can get 10 coupons worth $2 each to use for fresh fruits and vegetables at any of the SFC Farmers Markets located in and around the Austin area. You can get the vouchers two times while you are pregnant. They can only be used for fresh fruits and vegetables. Call Member Outreach at to get your SFC Farmers Market coupons. For STAR and CHIP Perinate members, vouchers are redeemable for fresh fruits and vegetables at the select market locations listed below: SFC East Market, 51st Street & 183 (YMCA), SFC Downtown 4th and Guadalupe, SFC Triangle 46th and Lamar SFC Sunset Valley 3200 Jones Rd/Tony Burger Center Limitations: No cash is provided if the member does not use the full value of the voucher. Members must provide their own transportation to the market. Vouchers are for pregnant moms - 10 vouchers, up to two times per pregnancy. Upon request and validation of membership, the vouchers will be mailed to the member within five days of request. Members may request vouchers on four separate occasions with a minimum of two weeks between requests.
