1 , Inc. Pharmaceutical Program is a provider of off-site waste services to the medical marketplace. We offer several options for a tailored medical waste disposal system and program to the medical communities in which you operate each day. Our full time professional staff offers over 150 years of regulatory experience in the proper management of medical and RCRA waste materials and customizing compliance programs to achieve your compliance goals. EWS will consult and identify each of your facility disposal needs by performing a walk through audit of each medical location, or, specifically respond to the needs you identify. We will structure a compliant waste program for hazardous, universal, recyclable, and regulated wastes shipped off-site. We also offer education to your personnel on the proper segregation and waste minimization techniques that the law allows and mandates. EWS offers assisted waste determinations and formulary review for the correct identification, profile building and choice for your disposal considerations. We are a fully insured medically regulated waste management company (MRW) maintaining a PART B RCRA status, maintaining environmental, professional, general, and automobile liability coverage. Maintaining your facility compliance requires routine management of off-site waste shipments, facility inspections, establishing satellite accumulation areas in strategic locations, and temporary storage within each facility. 2 , Inc. Below is program information on hazardous waste disposal, education your employees in collection and compliance to biannual recordkeeping as well as other support services we provide. EWS has selected services for which I hope you will find of interest. We offer the following services by either contract based or a monthly fee based program using a purchase order: Medical Facility Audit & Review- consult to identify and measure compliance with the hazardous waste regulations, as well as mandatory implementation of compliance programs with EPA, RCRA, DOT, TSCA, JSCA, OSHA and AHA Hazardous Waste Disposal Options- chemotherapy pharmaceuticals, packing samples, obsolete pharmaceutical products, specialty solutions, solvents, lab packing services, mercury, and spill removal, specific lab waste and universal waste collection. Non Hazardous/Non-Regulated Waste Disposal Options- oils, greases and cleaners, lab samples, obsolete pharmaceutical products, and electronic removal (e-waste) which falls under HIPPA requirements. Green Recycling Option- implementation of ZERO LANDFILL program Universal Waste Recycling Options- battery sorting, collection and removal, lamp(s) and lighting, mercury metal recycling by retort, and recycling of computer components. Provide MRW & RCRA waste removal services of materials that are hazardous, non-hazardous, and universal waste as described on Exhibit A waste profile forms. 2. Provide as requested & when required for off-site shipment/container management service (Exhibit E) to allow for the proper collection of waste handling, segregation, packaging & labeling to regulatory standards. 2. Consult & assist in the proper profile completion and waste identification to EWS, manifesting & land band completion, provide labeling systems for all containers shipped. 3. Print & provide off-site shipment reports, whenever requested, of all wastes arriving at EWS facility. 4. Pick up each waste stream properly identified by profile, offering the best available technology (BAT) including but not limited to incineration, chemical treatment, alternate fuel blending, reclamation, reuse, recycling & landfill of non-land band materials. 5. Offer emergency response services-after initial containment; manage complete cleanup and removal of response material based on Exhibit B pricing, eliminating the eminent harm. 6. Offer specified training (RCRA education) of facility personnel to RCRA compliance standards (included with a contract). Purchase order basis includes training with a separate fee structure. 7. Monthly service fee includes all the above plus training on-site, technicians traveling to listed facilities (Exhibit D) for a walk through audit and inspections, as needed, preparing reports annually and updating specified paperwork such as profile updates, manifests by year reporting, land ban forms of record & certificates of disposal or recycle (COD, COR) verification on file. 8. 4 , Inc. TERMS & CONDITIONS The Service Customer/Generator agrees to the attached terms and conditions herein. The Service Customer/Generator agrees to the attached service pricing schedule marked as Exhibit B. The Service Customer/Generator agrees to sign off on the pricing acknowledgement and has reviewed all prices and service as they apply marked Exhibit C. Contract price schedule attached does not reflect a volume discount. Universal Waste price schedule and Lab Pack price schedule attached in Exhibit B are invoiced as listed without discounts. Payment of invoice is due 30 days from the invoice date. All late charges are 1.5% per month added to the statement. The State of Alabama shall have final jurisdiction over legal matters requiring law determinations. Inc. is a service & disposal company that provides custom waste disposal programs to the medical community, commercial and industrial sectors for business. The Generator signs all waste profiles with completions assistance from EWS personnel. The Generator understands that non-conforming waste or off specification waste streams are subject to additional charges. The Service Customer agrees to pay additional fees incurred for non-conforming wastes. The Service Customer agrees to collect, store, and transport all waste in a safe, responsible manner, not jeopardizing human health, life and/or the environment. Waste containers not packaged by EWS personnel are subject to charges for repacking at the TSDF, if not in compliance with DOT regulations. All Scheduling must be made in advance, preferably 7-10 working days. Delays in loading the truck on a waste pickup can incur demurrage charges of $95.00 per hour after the first hour on-site. Firm scheduled dates for waste pickup must occur, or you will be subject to a no load fee of $ for cancellation without 24 hours notice. Transportation pick up cost & stop fee is based on a 4'x4'x3' (one cubic yard of material) pallet dimension. Pallet pricing is each, per drum. Stop fee is per facility. Service routes are established for LTL (less than truckload quantities) in location(s). In order to increase service pickup, the following fuel surcharge is required: per pallet, per drum. Monthly LTL transport pick up of waste is no charge unless the national fuel index climbs past present freight publication standards. EWS reserves the privilege to postpone work for any unsafe conditions that could pose a threat to drivers, technicians, the medical facility, environment or consultants. This agreement is valid for a 3 year, 5 year, or monthly charge. Cancellation of the agreement is permissible with 60 days written notice by either party, however, work in progress, including schedules in route will be charged out at published rates from Exhibit B. 5 , Inc. EWS will maintain all permits, insurance, disposal contracts and service agreements for the proper management of your wastes. EWS and the Service Customer, in good faith, enter into this agreement for service as listed herein and confirm by hand signature a full understanding of this written agreement. EWS offers a limited compliance service for part of the RCRA regulations for generators and understands that the customer has other responsibilities as well affecting facility compliance. EWS offers information and suggestions that are based on regulatory requirement, not cost value to the generator. It is the generators duty to weigh out the information it collects and advise EWS as a service provider, as to the course of action that shall be followed for facility compliance. This facility, at a minimum, is under the regulatory control of RCRA, DOT, JCHA, SARA, OSHA and the Clean Water Act. This agreement is not transferable and required updated terms and conditions upon mergers and acquisitions by either party. We will GUARANTEE a 1 year fixed disposal price with a signed contract.
