1 ADMISSION PROCEDURE PHYSICAL THERAPIST ASSISTANT Fall Semester, 2017 Revised: April 2016 How do I apply to the Physical Therapist Assistant program? The following items must be completed and be part of your records at North Central State College before acceptance to the clinical portion of the program can be determined: 1. Application to the college. The general enrollment process to North Central State College must be completed and submitted. The enrollment process can be completed in the Admissions Office in Kee Hall, or online at A letter of acceptance to the college will be sent to you once this step is completed. Please note that the enrollment process to the college is not complete until you have met with an Admission representative. Previous students of North Central State College who have not attended classes in at least one year will need to update their application to the college. Students who desire acceptance to the Physical Therapist Assistant program will be first accepted as pre-physical Therapist Assistant (pre-phta) students. This status neither implies nor guarantees eventual acceptance into the Physical Therapist Assistant program. Pre-PHTA students may register for developmental, general, and basic courses only; this excludes the courses with the PHTA prefix. 2. Official copy of high school transcript or GED. An official copy of your high school transcript must be submitted to the Office of Admissions in Kee Hall. This needs to be ordered by you directly from your high school. High school seniors must have their final transcript on file with the college by June 15, If you have not graduated from high school, General Education Development (GED) scores must be submitted with passing scores. 3. COMPASS Assessment Test. You must take the COMPASS Assessment Test (or ACT) before registering for classes. Contact the Student Success Center in Kee Hall ( ), or visit for more information. 4. Apply for Financial Aid (if applicable). Contact the Financial Aid Office in Kee Hall ( ), or visit for more information. 5. READ0080 (grade of C or above) or qualifying COMPASS/ACT score. You must complete one of the following: READ0080 College Reading with a grade of C or above, or obtain transfer credit. A grade of at least C or transfer credit in a higher level English course (such as ENGL1010 or ENGL1030) will also meet this requirement. Complete the COMPASS Assessment test with a Reading score (RD) of 80 or higher. Complete the ACT with a Reading score of 18 or higher. 6. WRIT0090 ( passing (P) grade) or qualifying COMPASS/ACT score. You must complete one of the following: WRIT0090 Basic Writing with a passing (P) grade, or obtain transfer credit. A grade of at least C or transfer credit in a higher level English course (such as ENGL1010 or ENGL1030) will also meet this requirement. Complete the COMPASS Assessment test with a Writing score of 81 or higher. Complete the ACT with an English score of 22 or higher. 7. MATH0086 (grade of C or above) or qualifying COMPASS/ACT scores. You must complete one of the following: Complete MATH0086 Beginning Algebra with a grade of C or above, or obtain transfer credit. A grade of C or transfer credit in a higher lever math course (such as MATH1010, STAT1000, etc.) will also meet this requirement. Complete the COMPASS Assessment test with a Numerical Score of 61 or higher and an Algebra Score of 52 or higher. Complete the ACT test with a Math score of 22 or higher.

2 PTA Admission Procedures Page 2 8. High school chemistry (grade of C or above) or CHEM1010 (grade of C or above). You must have completed one unit of high school chemistry with a grade of C or above, or complete CHEM1010 Introduction to Chemistry with a grade of C or above or transfer credit. CHEM1010 Introduction to Chemistry has a prerequisite of MATH0074 Beginning Algebra. A grade of C or transfer credit in a higher level chemistry course (such as CHEM1030) will also meet this requirement Grade Point Average (GPA) A 2.5 cumulative grade point average (GPA), based on 12 semester credit hours of NCSC college coursework or a high school grade point average (whichever is most recent) is required prior to consideration for acceptance into the program. 10. Application for Consideration and Application for Selection to the Physical Therapist Assistant program. The Application for Consideration must be completed by end of fall term 2016 to be considered for the fall semester, 2017, class. Once the above requirements (steps 1-9) are met (qualifying grades, scores, etc. on file), you must complete the Application for Consideration for the Physical Therapist Assistant Program by the end of Fall Term This is NOT the same as the general enrollment process to the college. This application is available in the Health Sciences Office, Room 201-HS, and must be completed there. It cannot be mailed to you, nor is it available online. After you complete this application, it will be forwarded to the Physical Therapist Assistant selection committee. Upon completion of the Application for Consideration, you will receive the Application for Selection, which must be returned by the first Monday in February High school seniors applying to the program who will be graduating in the spring of the entering year may complete the Application for Consideration and the Application for Selection provided all the above requirements have been met and an official partial high school transcript is on file in the Student Records Office. Important Additional Requirements! You must maintain at least a 2.5 grade point average (GPA) prior to beginning the Physical Therapist Assistant program. You will be required to maintain a current CPR certification for Health Care Provider throughout the Physical Therapist Assistant program. College offers HLST1010 CPR/First Aide on a limited basis. It is your responsibility to notify the Student Records Office of any address or phone number changes. Failure to keep contact information updated could prevent you from receiving important communication about acceptance to the program, which will result in your name being removed from the Physical Therapist Assistant applicant list. These pre-requisites are reviewed on an annual basis and may be revised for the next academic year. How are students accepted into the Physical Therapist Assistant program? Only students who meet the preceding qualifications are eligible to complete the Application for Selection to the Physical Therapist Assistant program. Selection into the Physical Therapist Assistant program is based on a point system. Points are awarded in the following categories: Grade Point Average (20 points maximum) High school or North Central State College grade point average, whichever is higher. North Central State College grade point average can be used only if at least 12 credit hours of course work has been completed. Other college transcripts will be utilized when deemed necessary by the program selection committee. The North Central State College Selection Committee reserves the right to refuse consideration of any transcripts more than seven (7) years old. This includes both college and high school transcripts. Courses Completed (15 points maximum) Specific courses completed in college with a grade of C or above including: BIOL2751, BIOL2752, HLTH1150 (points applied based on grade received; A=3, B=2, C=1 for a max of 9 points) and STAT1010, PHYS1010, PSYC1010, PSYC2010 (1 point each for grade of C or better for a max of 3 extra points). Courses completed in high school with a grade of C or above: Biology II, A & P, Physics. (1 point each for a max of 3 points)

3 PTA Admission Procedures Page 3 Knowledge of Health Field (30 points maximum) Complete one or more of the following requirements. A maximum of 30 points will be awarded for any combination of experience included below. A list Minimum of 6 months full-time work experience, within the last 5 years, having direct patient care in physical therapy clinic. (30 points maximum) Minimum of 40 hours/month for not less than 12 months, within the last 5 years, having direct patient care in a physical therapy clinic. (25 points maximum) Student applicants are required to have a minimum of 60 observation hours (maximum 120 hours for points) with a licensed PT or PTA divided between Inpatient and Outpatient setting (minimum of 30 hours in each setting with no more than 4 different facilities). It is acceptable to complete both observations at one facility if it is large enough where you can observe another PT or PTA in a different setting. Observation with a relative is not accepted or at a facility where you are currently employed. (5 points awarded based on each 30 hour increment) Minimum 6 months full-time work experience within the last 5 years or a minimum of 40 hours/month for not less than 12 months within the last 5 years, in a non-physical therapy setting with direct patient care in an accredited health care agency. (15 points maximum) Minimum of 3 months full-time work experience, within the last 5 years, in a non-health care facility such as private clinic, summer camp, transportation or school for special needs children. (10 points maximum) Minimum of 6 months full-time work experience, within the last 5 years or minimum of 40 hours per month for not less than 12 months within last 5 years, in a non-patient care experience in a health care facility. (10 points maximum) The observation and volunteer experience must be under the supervision of physical therapist or physical therapist assistant. As an example, the following facilities may allow an individual to volunteer or observe. You may observe in any facility under the supervision of a physical therapist or physical therapist assistant. Please contact a health care agency's Physical Therapy Department by phone prior to your visit to make an appointment to discuss this matter. Any valid physical therapy department which is convenient and agrees to host the student is acceptable. Below are some possible options: Avita Therapy - Bucyrus Avita Therapy - Galion Dale Roy School, Ashland Fisher-Titus Medical Center, Norwalk Mifflin Care Center Ohio Health Med Central Mansfield Hospital Avita Summit Therapy Observation/Employment Reference (15 points maximum) Mercy Hospital of Willard Kingston of Ashland Morrow County Hospital Newhope School Oak Grove Manor Advance Therapy and Fitness Samaritan Medical Center, Ashland Evaluation of applicant s volunteer or work experience in physical therapy clinic or other healthcare setting. The form for this recommendation/evaluation is included in the Application for Selection packet. Form must be completed and signed by a PT/PTA or Supervisor. Character Reference (5 points maximum) One personal character reference letter may be submitted (this should not be from an employer, PT/PTA or family member). Previous Application for Selection (10 points maximum) Persons completing an application for selection to the PTA program for two or more consecutive years are eligible for points. Graduate of Health Tech Program (5 points maximum) Applicants are then considered based on total points achieved. Those students with the highest total points are accepted into the Physical Therapist Assistant program. In the event of a tie, as a result of the selection process, the date of the student's most recent and current Application for Consideration to the Physical Therapist Assistant Program. All application for selection materials must be submitted to the selection committee by the first Monday in February in order to be considered for the upcoming class. This includes completion of courses for points, work experience/observation verification, and evaluation of high school and college transcripts for transfer credit. Transcripts from another college must be submitted by November 14, 2016, as they can take up to 30 days to be evaluated before transfer credit is awarded. The program selection committee reserves the right to decline review of any transcripts older than 7 years.

4 PTA Admission Procedures Page 4 Students accepted into the Physical Therapist Assistant program will receive an official acceptance letter from the Program Director to which the student must respond, accepting or declining the appointment. Those students not accepted into the Physical Therapist Assistant program will need to reactivate their Application for Consideration and Application for Selection to be considered for the next year's class after April 1, While materials may be preserved for consideration, points for selection are re-assigned each year. This procedure is reviewed on an annual basis and may be revised for the next academic year. What abilities/skills should an applicant to the Physical Therapist Assistant program be expected to demonstrate? Success as a Physical Therapist Assistant student depends on many variables. Among them are academic abilities and certain technical abilities or competencies. An applicant for Physical Therapist Assistant must have the abilities and skills necessary for the practice of physical therapy. These skills and abilities include observation, communication and motor ability. Technological compensation can be made for some handicaps in certain areas, but an applicant should be able to perform in a reasonably independent manner. An applicant must possess the emotional health required for full utilization of his/her intellectual abilities. Applicants must be able to tolerate physically taxing workloads and to function effectively under stress. The above is not intended to discriminate against persons who have physical handicaps that limit their ability to perform these tasks in an adequate manner. This policy protects and insures the safety of both the applicant and his/her clients. What else should I know about the Physical Therapist Assistant Program? Physical Examination An acceptable health physical exam along with verification of required vaccinations will be required prior to enrollment in the Physical Therapist Assistant program. Students accepted to the Physical Therapist Assistant program will receive physical examination materials from the department at the college. Each student must pass a complete physical with verification of immunizations/immunities and an acceptable Bureau of Criminal Identification and Investigation (BCI&I) report/fbi background check and drug screening before being permitted to enter clinical areas. Cardiopulmonary Resuscitation Certification (CPR) All Physical Therapist Assistant students must have a current valid certificate in CPR before they will be allowed to start the clinical education portion of the program. This certificate can be obtained through the American Heart Association or Red Cross. The course should be equivalent to North Central State s non-credit offering CPR for the Health Care Provider. Successful completion of HLST1010 CPR/First Aid will also fill this requirement. Current certification must be maintained throughout the program. Background Check An acceptable Bureau of Criminal Identification and Investigation (BCI&I) report/fbi background check and drug screening is required in order to begin the education portion of the Physical Therapist Assistant program. The BCI&I check, FBI background check, and drug screening will be done at the college during the program and the cost is included in the lab fees of the first PTA course. Academic Year and Schedules A new class is started fall term of each year. Lecture and demonstrations are given at the college. Practicum experience is concurrent with the classes and is scheduled in different health care facilities during the year. Students must provide their own transportation. Academic Standards A new class is started each fall term. Lectures and labs are given at the College and at local facilities. Clinical experiences begin with the third semester (second year Summer term) of the program. These experiences are scheduled in various facilities throughout Ohio. PTA students are required to follow the specific curriculum sequence provided in the college catalogue and PTA handbook to enhance learning by building a foundation of education to advance learning levels. If for any reason the student is unable to maintain the curriculum sequence, dismissal from the program may be necessary. Candidates selected into the PTA program must have a 2.5 grade point average before entering the program in Fall I term. Students must maintain a 2.0 grade point average to remain in the program. PTA students must receive a minimum course grade of 77% or higher in all core education courses (BIOL2751, BIOL2752, STAT1010, HLTH1150) and all PHTA courses in order to meet prerequisite and graduation requirements.

5 PTA Admission Procedures Page 5 Liability Insurance Each Physical Therapist Assistant student must carry liability insurance which must be in effect the entire time the student is enrolled. This price of the insurance is included in fees for the first and third term PTA courses. Student Health Services There are no health services available on campus at this time. All health related issues will require either private or emergency transport to the local health facility of choice. Health Insurance Requirement The PTA program does not require that students maintain a health insurance policy but it is highly recommended that students obtain minimal health coverage to cover catastrophic incidents. At times during clinical education the clinical site will require health insurance as a requirement for students. In such cases the student may obtain health insurance or may have to be placed at another clinical site. Student health insurance information is available from the College Admissions Office. The low cost insurance program is designed for full-time students who traditionally are inadequately insured. Additional student insurance is available for the dependents of students upon request. Students will be responsible for any expenses incurred for health related emergency care in off-campus educational experiences.
