7 The driving force for our problems is progress Tremendous scientific progress has side effects Individuals cannot know all they need to know Medicine has become team sport for patients with: Chronic diseases (e.g., diabetes, heart failure) Multiple complex conditions The potential result: Chaos Physicians don t know safest, best, most efficient strategies Physicians don t know what other colleagues have done Patients get conflicting advice Important issues slip between the cracks How healthcare is financed is important, but the fundamental problems exist at clinical level, and will require clinical leadership. 7

12 What Can Be Done to Improve Care and Prevent Admissions? Population-based programs: Improve the reliability with which all patients receive interventions known to beneficial Example: Registries to track all diabetic patients, which help physicians identify which ones have not received key interventions Personalized interventions: Identify the highest risk individuals, and connect those patients with programs that coordinate care between visits Examples: Nurse-based CHF program; telephonic coaching of highest risk patients 12

13 A Systematic Approach to Improving Care and Preventing Admissions for Six Key Conditions Asthma CAD CHF COPD DM Renal Population-oriented programs: Registries to track population Decision support to enhance reliability of MD interventions Disease management programs Patient education interventions Individual-oriented programs for highest risk patients: Information systems to identify highest risk patients Follow-up phone calls after ED visits or hospital admissions Telephonic health coaching for high risk patients Hospital-based or PHS Home Care disease management program 13

15 Disease Management Averts a CHF Admission MD notified of weight gain. Patient called, and MD learned she had stopped taking furosemide twice daily. Regular regimen restored 15

16 Examples of the Challenges for Physicians Integration into groups that can take on population management Recognition that they are members of teams even if they are the leaders of those teams Willingness to be good team member (meet other team members expectations, work at communication with colleagues) Adoption of EMRs and other systems to help track populations 16
